Immunoglobulin profiles and blood variables in 50 patients with hairy cell leukemia. Effects of splenectomy.

The immunoglobulin profile in patients (50) with hairy cell leukemia (HCL) was assessed on the basis of quantitative estimation of IgG, IgA, IgM, IgD and IgE serum levels. In HCL patients IgG, IgA and IgE mean levels were found higher than in healthy persons. No significant difference was found in serum IgM and IgD levels as compared to the reference group. Considering three major immunoglobulins, in no class and no patient with HCL immunoglobulin deficiency has been recorded. The serum immunoglobulin profile might be useful in differentiating HCL from chronic lymphocytic leukemia or lymphocytic lymphomas. However, no difference in immunoglobulin profiles in all five classes under study could be found between splenectomized and nonsplenectomized patients. Four percent PEG precipitation technique was used for the detection and characterization of IgG immune complexes in sera. The percentage of IgG precipitated in 4% PEG from serum was significantly higher in patients with HCL than in healthy persons; similarly as in immunoglobulin study no significant differences between splenectomized and nonsplenectomized patients were recorded. The beneficial effect of splenectomy was documented by the improvement in hemoglobin concentration, platelet and neutrophil counts and erythrocyte sedimentation rate.

Hairy-cell leukemia and toxoplasmosis.

Investigation for Toxoplasma gondii infection using complement fixation test and microprecipitation method in agar gel was performed in fifteen patients with clinically and morphologically typical hairy-cell leukemia. Positive complement fixation test was found in four patients. In three patients an initially high complement fixation titer or its considerable increase associated with positive microprecipitation in agar gel suggested a recent toxoplasmosis. The importance of search for Toxoplasma gondii infection in hairy-cell leukemia patients especially before splenectomy and the necessity of reinvestigation after splenectomy is stressed.

Congenital dyserytropoietic anaemia, type II (HEMPAS) in three siblings.

These siblings of a Czech family aged 21, 19 and 6 years, respectively, with congenital dyserythropoietic anemia, type II, (HEMPAS) are reported. In two elder siblings ferrokinetic studies revealed a rapid plasma 59Fe clearance, markedly decreased erythrocyte incorporation and shortened 51Cr red-cell survival. Direct anti-globulin test was found positive in one of them. Further investigations revealed low values of blood plasma cholesterol, total lipids, beta-lipoproteins, beta-carotine and vitamin E and A as well as low values of the prothrombin complex. Liver biopsy demonstrated siderosis and disseminated intravascular coagulation in the liver in both patients. The possible reasons for these humoral aberrations are discussed.
