Digestive permeability to different-sized molecules and to sodium cromoglycate in food allergy.

Intestinal absorption of mannitol (used as a marker of normal monomer passage) and intestinal absorption of lactulose (used as a marker of macromolecule exclusion) were measured in 15 healthy subjects and in 20 patients with food allergy. When studied in the fasting state, there was no apparent difference in the absorption of mannitol between control subjects and allergic patients although the absorption of lactulose was more marked, but not significantly, in the latter group. Ingestion of an allergen by these patients led to a significant increase in the absorption of lactulose (intestinal passage increased by a factor of two). Administration of 300 mg cromoglycate 1/4 hour before the provocation test completely averted abnormal intestinal permeability. Digestive absorption of cromoglycate was also studied in the same 15 control subjects and 20 allergic patients. The peak plasma concentration and area under the curve were significantly greater in healthy subjects when the study was carried out in the fasting state. The application of an oral provocation test with an allergen also significantly increased the plasma concentration of cromoglycate in allergic patients. Contact between an allergen and the digestive tract in allergic patients therefore leads to an increased intestinal absorption of macromolecules but also to cromoglycate passage, which inhibits allergic reactions.

[Congenital atrioventricular junction tachycardia. Favorable response to anti-arrhythmia agents].

The congenital junctional ectopic tachycardia, is an unusual tachyarrhythmia, with early clinic manifestation and poor antiarrhythmic drugs response with a great infant mortality reaching rates of 35%. It deserves a special attention in its timely detection as well as in its appropriate handling with different modalities of pharmacological and nonpharmacological therapy. We reported two cases in which age of presentation of tachyarrhythmia was at three months and whose particularity was the good response to antiarrhythmic drugs; verapamil and later propafenone, used for the acute stages and a combination of propafenone plus propranolol initially for the chronic stage. Due to intolerance it was necessary to change the treatment after a year for sotalol and digital with good response. We review the literature about this topic.

[Usefulness of the thoracic circle in localizing the pre-excitation zone in Wolff-Parkinson-White syndrome].

In this study thoracic circle lead electrocardiogram were recorded during sinus rhythm in 50 patients with Wolff Parkinson White Syndrome. We analyzed: delta wave polarity, QRS axis in the frontal plane, ventricular preexcitation, the pattern of precordial R wave transition and QRS morphology in the unipolar leads, also concordance between electrocardiographic patterns and the site of the accessory pathway determined during electrophysiological study. Electrocardiograms from patients with left lateral sites showed negative delta waves in leads LI or a VL, V7 to V9, positive delta waves in V3R to V9R, a normal QRS axis and early precordial R wave transition (20 of 23 patients). Left posterior sites manifested negative Delta waves in L3, a VF, V7 to V9, V7R to V9R and a prominent R wave in V1 (4 of 5 patients). Left posteroparaseptal sites had the same pattern plus negative delta waves in L2, a superior QRS axis, and RS or Rs morphology in V1 (3 of 3 patients). Right posteroparaseptal sites had negative delta waves in L2, L3, a VF, V3R to V9R, positive delta waves in V7 to V9, a superior QRS axis and an R greater than S in V1 (10 of 11 patients). Right free wall locations manifested negative delta waves in L3, a VR, V3R to V9R, positive delta waves in V7 to V9, a normal QRS axis and R wave transition in V3 to V5 with QS morphology since V3R to V9R (6 of 7 patients).(ABSTRACT TRUNCATED AT 250 WORDS)
