[The value of acetazolamide provocation combined with blood flow tests in the diagnosis of ischemic cerebrovascular diseases].

In 28 (3 normal, 11 TIA, 14 completed stroke) patients 99mTc-HMPAO rCBF SPECT studies were performed at rest and after acetazolamide administration. For the investigations, a one-day protocol (the stress study directly followed the rest investigation) was used. The reconstructed and normalized slices were evaluated visually and semiquantitatively with a side difference analysis method. In the group of TIA patients, the abnormal results of the rCBF investigations increased from 55% to 82% after acetazolamide provocation. The corresponding results in the completed stroke group were 80% and 87%. After acetazolamide stimulation, hypoperfusion appeared or become more pronounced in the majority of the TIA group patients in contrast with the completed stroke patients with an unchanged or decreased perfusion abnormality. The semiquantitative evaluation method was mostly effective in the TIA group, where predominantly a one-sided cerebrovascular disorder was suspected. It was concluded that the 99mTc-HMPAO studies allow demonstration of the acetazolamide-induced cerebral perfusion alterations, and this method can be useful in the diagnosis and management of cerebrovascular disorders.

[SPECT follow-up of cerebral blood circulation after stroke].

rCBF SPECT investigations with 99mTc-HMPAO were performed in 22 completed ischemic stroke patients on average 2.5, 16.8 and 38.0 days after stroke onset. As control group, 12 neurologically healthy volunteers were examined with the same method. The overall sensitivity of the rCBF SPECT method in the detection of cerebral blood flow abnormalities was 91%, with a specificity of 83% and an accuracy of 88%. The rCBF study was highly effective, particularly in the acute phase of the disease. Regional increased tracer uptake or a false normal 99mTc-HMPAO distribution due to the developing hyperemia mostly complicated the rCBF image interpretations in the subacute period. In the chronic phase, the spatial resolution of the SPECT system limited the detection of the continuously retracted stroke lesions.

Blood Flow Velocity

[Effect of intra-arterial chemotherapy of head and neck tumors on cerebral blood circulation].

A combination of Cisplatin and Epirubicin was administered through the external carotid artery system according to a standard protocol as preoperative or palliative therapy in 14 oral cancer patients. With the aim of clarifying the subacute side-effects of this chemotherapy on the intracerebral circulation, rCBF SPECT investigations with 99mTc-HMPAO were performed before and after the cytostatic treatment. The interval between SPECT studies was 24 (SD +/- 12) days. In concordance with the neurological condition, visual assessment of the rCBF SPECT reconstructed transversal slices revealed no perfusion abnormalities and no changes on the follow-up. No significant changes were found in the side activity ratios in 22 regions of each hemisphere (unpaired t-test) in the whole group of patients. It was concluded that within the investigated time interval this highly effective combination of cytostatics administered perfectly through the external carotid artery system causes no serious disturbances in the cerebral circulation in oral cancer cases.

[Ergometric stress test using Tl-201 myocardial perfusion. Tomographic or planar imaging?].

The clinical performance of the planar vs. tomographic ergometric stress 201-Tl myocard perfusion scintigraphy was compared. The results of the investigations of 80 patients were analysed retrospectively. In 59 patients planar, in 21 tomographic imaging was performed. As a consequence of clinical decision making cardiac catheterization was made in all of the patients, within 3 months after the perfusion scintigraphy. The sensitivity of the planar method was 23/27 (85%), the specificity 18/32 (56%), the accuracy 41/59 (69%). The respective values of the SPECT technique were: Sensitivity: 12/12 (100%), specificity 6/9 (66%), accuracy 18/21 (86%). It is concluded, that in comparison to the planar imaging the SPECT technique is superior in the detection of the perfusion abnormalities of the myocard due to significant coronary narrowings.

[The role of combined SPECT studies of regional cerebral blood flow and volume in the detection of intracranial vascular malformations].

Brain perfusion (rCBF) (99mTc--HMPAO) and blood volume (rCBV) (99mTc--RBC) investigations were performed by SPECT on 13 patients with intracranial AVM. In each cases the rCBF revealed a decreased tracer uptake at the site of the AVM. In the rCBV SPECT, abnormal AVM structures with increased radioactivity were detected in 92% of cases. In all of the patients surgical resection of the AVM was also carried out. In the cases where the rCBF disturbance was equal or larger than the rCBV abnormality the surgical complications were more frequent it was concluded that combined rCBF--rCBV method is suitable to detect the AVM-s, and to clarify their haemodynamic effects. It is helpful in planning the surgical intervention, and in the follow-up.

Three-dimensional presentation of the Fourier amplitude and phase: a fast display method for gated cardiac blood-pool SPECT.

The routine clinical use of gated SPECT is inhibited by sophisticated, time-consuming processing techniques. The present paper describes a new technique for the simultaneous three-dimensional presentation of the amplitude and phase of the first Fourier harmonics, with the aim of obtaining detailed information about the ventricular motion in a relatively short time, from each angle of view of three-dimensional space. The method is simple and robust, and processing is automatic. It does not need carefully elaborated techniques for surface determination, because the cardiac surface is merely used as a reference skeleton onto which the functional information of amplitude and phase is mapped. The Fourier analysis before reconstruction results in running times shorter than 15 min and may further open the way for the routine use of gated SPECT.

[Effect of extra- and intra-cranial vascular anastomosis on regional blood flow, studied by SPECT using 99m-Tc HMPAO].

Thirty-two patients suffering from occlusive cerebrovascular disease were investigated by means of 99mTc-HMPAO brain SPECT before and one month after EC-IC bypass operation. The regional cerebral blood flow was unchanged postoperatively in 19 patients, increased in 7 cases, and deteriorated in 6 patients. One patient died in the postoperative period and in another no follow-up SPECT could be performed. It is concluded that the regional cerebral blood flow, even in the absence of a morphological deficit on CT, was not improved by the operation in the majority of cases.

[Isolated right ventricular infarction].

Generally, right ventricle infarction is accompanied by left ventricle infarction. Isolated right ventricle necrosis is rare. Among 1707 patients with 6-week-old myocardial infarction there were 2 cases with isolated right ventricle infarction. Diagnosis based on pathologic Q waves in right precordial chest wall leads, radionuclide and echocardiographic observations. In one case, diagnosis was supported by coronary angiography, too.

[Bone scintigraphy in breast cancer: a ten-year follow up study].

The value of bone scanning in the primary staging and clinical course of breast cancer was studied in a 10-years follow-up. Bone metastases (BM) were detected at the time of primary diagnosis in 6.8% of the patients. After primary therapy, the skeleton was the first site of the relapse in 42% of the cases. During the follow-up, BM were revealed in 25% of 623 patients a mean of 22.3 months after the operation. BM were manifested in 17% of the cases with small primary (pT1) tumors and negative axillae. Forty-three percent of the patients had no symptoms at the time of BM diagnosis. The mean survival after the appearance of BM in 163 patients was 18.2 months. Thirteen patients survived more than 5 years after the diagnosis of BM. The routine use of bone scanning is suggested at the time of primary diagnosis, and then yearly in a minimum 5-years period, independently of the symptoms.

[The role of SPECT-supported three-phase scintigraphy of red blood cells in the diagnosis of liver hemangioma].

The roles of planar and SPECT 99mTc labeled red blood cell (RBC) scintigraphy in the diagnosis of hepatic cavernous hemangioma were evaluated. Out of the 29 patients with various focal intrahepatic lesions 13 were diagnosed as having hemangiomas: 6 proven by surgery, and 7 proven by maintaining stable clinical course ranging from 6 to 12 month with no change in the size or appearance of the lesion on US. In the 13 patients a total of 16 hemangiomas were found. 12 of these detected by planar imaging, 2 detected by SPECT only. The sensitivity of planar RBC scintigraphy was 75%, the specificity was 100%. Although planar imaging is probably sufficient for all large or superficial hemangioma, delayed SPECT should be used with small (2-3 cm) or deeply seated lesions.

[Pheochromocytoma in Recklinghausen neurofibromatosis].

The authors describe the case of a 38-year-old hypertensive woman who suffered from neurofibromatosis, pheochromocytoma, scoliosis and diabetes mellitus. Because of the residual pheochromocytoma surgical intervention was repeated. According to the available literature this is the first case in Hungary where pheochromocytoma was associated with neurofibromatosis.

[Internal mammary lymphoscintigraphy in breast cancer].

Clinical importance of the internal mammary radionuclid lymphoscintigraphy (IM RNLS) have been studied in 203 breast cancer patients at the primer staging in 94 unilateral, 7 bilateral independent breast tumor, 13 inflammatory breast cancer, and in the frame of the clinical follow-up in 41 local/regional recurrence, 9 soliter sternal lesion and 2 metastatic bilateral breast tumor. Based on the results the routine use of the IM RNLS is suggested beyond the primer staging in the course of the clinical follow-up at the appearance of the above mentioned entities related to the lymphogen tumor spread.

Breast Neoplasms

[Radionuclide imaging of parathyroid adenoma using the 201TI-99mTc substraction scintigraphy method].

Primary hyperparathyroidism is the most frequent parathyroid disease. Parathyroid adenomas account for the majority of primary hyperparathyroidism (81%) while carcinoma and diffuse hyperplasia occur less frequently (4% and 15%). Ten per cent of adenomas are located substernally. The difficulties of preoperative localization of parathyroid adenomas may lead to incomplete surgical removal. Therefore an accurate technique for preoperative localization of the parathyroid gland is of utmost importance. Various imaging methods are available (ultrasonography, computer tomography, angiography, etc.), each with their own limitations. Thus, a nuclear technique performed via 201Tl-99mTc dual isotope subtraction scintigraphy may well contribute to the safety of diagnostics. The authors present a case of parathyroid adenoma diagnosed by this method and confirmed histologically. The role of non-invasive techniques in preoperative localization of parathyroid adenomas protecting the patient from invasive procedures and repeated surgical explorations is highly emphasized.

[Cardiolite (99m-Tc-isonitrile) SPECT studies in patients prior to coronary bypass surgery].

The authors studied with 99m-Tc Cardiolite (methoxy-isobutyl-isonitrile) SPECT examination the effect of coronary bypass operation on the myocardial perfusion. One week before and 3 weeks after the operation examinations were performed first with dipyridamol loading and 24 hours later in rest. The regional perfusion of the myocardium was studied on the basis of transversal section-pictures of corrected axis position. In the examinations preceding the operation the global sensitivity of the method was found to be 100%. In the demonstration of the structures of the ramus descendens 95%, of the ramus circumflexus and of the right coronary artery 100% sensitivity was reached. In postoperative examinations following the bridging of the ramus descendens anterior the improvement of the perfusion of the corresponding ventricular segment was found in 15/20 during the loading and in 7/8 cases following the bridging of the ramus circumflexus and right coronary artery. Improving perfusion was demonstrated in 17/20 and in 6/8 cases in rest. On the basis of the results the authors consider their method suitable for the study of the regional blood perfusion and for the examination of the changes taking place upon coronary bypass operations.

[Regional cerebral blood flow studied by single photon emission computed tomography (SPECT)].

Clinical experience based on regional cerebral blood flow investigations with 99mTc HMPAO SPECT in 164 patients is reported. The pharmacokinetics of the 99mTc HMPAO are summarised, and the important indications of the investigations are interpreted in case reports (stroke, surgical solution of intracavernous aneurysm, Alzheimer and multiinfarct types of dementias). The literature data suggest that the diagnostic possibilities with this method are good in other diseases too, this area is surveyed briefly.

[SPECT--single photon emission computer tomography. A new imaging method in isotope diagnosis].

The paper gives an introduction to the basis and quality control of a relatively new imaging method. First the different kinds of SPECT instruments, and then the aspects of diagnostic application are described. Details are given on examinations of the central nervous system, the abdomen, the hearth and the skeletal system, on the possibilities of immunoscintigraphy, and also on the indications of SPECT studies and the clinical performance. Finally, the state and future of this technique in Hungary are considered.

Forecasting

[Accurate detection of hemangioma of the liver by SPECT].

The hemangioma cavernosum is a frequent, benign tumor of the liver. Generally the hemangioma becomes suspected during the US examination as a secondary result. From the point of view of the differential diagnostics the three-phase blood-pool scintigraphy, a new very sensitive isotope diagnostic method, has a great importance in these cases. The images taken at the beginning, the early and the late-phase show the kind of alteration unambiguously. Since the conventional planar scintigraphy is not always able to detect the small (1-2 cm in diameter) lesions and the lesions surrounded by intact parenchyma, the SPECT has big advantage increasing the resolution of images and introducing the three-dimensional imaging. The authors illustrate the possibility of specific detection of liver hemangioma in a case by SPECT.
