The results of 143 ultrasound investigations (UI) of the mammary glands (MG) in 86 patients with lactic mastitis (LM) are presented. The techniques for performance of common UI of MG, and as well in combination with pharmacologic pituitrin assay are described. The echographic signs of the different forms of LM are systemized, their dynamics traced in the process of treatment, peculiarities of UI of MG in common and complicated course of the postoperative period are reflected.
In the experiment on 75 white rats, the reactogenicity and strength of catgut, dexon, kacelon and PGL (native analogue of vicryl) were studied 10, 20 and 30 days after their implantation into parenchyma of the lactating mammary gland (MG). Around a catgut thread, the cellular-tissue reaction according to the type of purulent inflammation occurred. A danger of oncogenic effect of catgut which manifested itself in proliferation and atypical hyperplasia of epithelium of the MG ducts was noted. Dexon, kacelon and PGL were characterized by similar low reactogenicity, but kacelon strength reduced considerably in moistening. Dexon and PGL due to their reactogenicity and strength properties are the materials of choice in surgical treatment of MG diseases.

The results of treatment of 6681 patients with acute pancreatitis (AP) were analysed. Operated on were 554 patients with AP, among them, 260-with acute destructive pancreatitis (ADP). Total lethality was 1.6%, the postoperative one-20.2%. In ADP, the postoperative lethality was 28%. Mild forms of the AP course were characterized by constancy of the clinical symptoms and their rapid involution, severe forms of the disease (destructive pancreatitis)--by progressive periodicity and intensity of clinical signs. A morphologic pattern of the different forms of AP is described. It was established that at the early period of the disease, patients died mainly from posthemorrhagic anemia, anemia with intoxication, at the late period--predominantly from different purulent and necrotic complications.
The method of extracorporal connection of the swine spleen (ECSS) was used in the complex treatment of 32 patients with acute pancreatitis. Three patients died. It was established that ECSS effectively reduced hyperfermentemia and increased the phagocyte activity of blood leukocytes which was an important component of complex treatment of patients with acute destructive pancreatitis.
The dynamics of activity of lysosomal enzymes in the blood, morphometric indices in the parietal peritoneum was studied in 217 patients with acute diffuse peritonitis depending on the severity of the process. It was established that patients suffering of acute diffuse peritonitis through showing to a certain extent similar clinical picture differ, however, by their morphochemical indices. Comparing the activity of catepsin-D, relative specific volume of erythrocytes (V), density of distribution of polymorphonuclear leucocytes (N) it may be suggested that catepsin values over 16.9 ast. un (min, mg), V--over 0.4 conv. un., N over 4.04 conv. un indicate irreversible changes in the tissues.

Intensification of specialized aid to the patients with acute pancreatitis, improvement of the existing methods for diagnosis and treatment, and development of the new ones permitted to reduce the total, postoperative lethality and mean duration of hospital stay of the patients. It is of great social importance, as more than 70% of them are of a productive age.
On the basis of the analysis of 217 records of acute diffuse peritonitis peculiarities of the postoperative process of that complication in persons treated with 2 antibiotics (161) and 4 antibacterial drugs (56) were discussed. 4 risk levels were defined with an account of the inflammation process severity. The use of poly-antibiotic therapy was validated. The technique of its application is described in detail. The technique allowed to lower the frequency of postoperative complications and lethality 5-fold.
