Is the stability of the oxyhaemoglobin dissociation curve in stored blood a function of the way the cells 'pack'?

The results in this report show that the oxyhaemoglobin dissociation curve (ODC) of blood when stored at 4 degrees C under special storage conditions was stable for at least 7 days. The stability of the ODC was reflected in both the haemoglobin p50 (the partial pressure of oxygen at 50% saturation of the haemoglobin) and n (the Hill slope of the ODC) values and was associated with constant blood pH and 2,3-diphosphoglyceric acid (2,3-DPG) concentrations. The ODC was stable whether heparin, ethylene diamine tetra-acetic acid (EDTA) or citrate phosphate dextrose (CPD) were used as anticoagulants, but the latter was associated with higher p50 values than those observed in fresh samples. The most critical storage requirement for the ODC stability appeared to be some function of the area of the cell-plasma interface as orientation of the collection tubes in other than a vertical position, or storing blood in larger containers, resulted in a marked fall in p50. These results have important implications in that they suggest that the relative packing of the cells or cell-cell and/or cell-plasma interfaces can significantly affect the ODC stability. Further, they may be of potential importance in blood banking practice.

Valve replacement in active infective endocarditis.

To assess the role of emergency valve replacement in patients with active infective endocarditis (IE), we reviewed 30 patients who underwent valve replacement within 3 months of the diagnosis of IE. Eighteen patients fulfilling the criteria for active IE underwent emergency surgery. The aortic valve was involved in all cases and was previously thought to be normal in 12 (67%). There were 5 early deaths (27,7%), 4 due to circulatory failure and 1 due to rupture of a mycotic cerebral aneurysm. Prosthetic valve dehiscence (paravalvular leak) developed in only 1 patient, and prosthetic valve endocarditis did not occur. The results confirm the place of early valve replacement in otherwise moribund patients with IE. We recommend immediate valve replacement for active IE in the presence of: (i) progressive haemodynamic deterioration; (ii) failure to obtain early control of infection; (iii) significant emboli; and (iv) conduction disturbances.

Gibson's invariance hypothesis and the Ames oscillatory effect.

Previous explanations of the Ames oscillatory effect are evaluated by means of a functional-evolutionary theory of perception. Use of stored interpretations, static elements, and other explanatory ideas are rejected in favor of adaptive response to ambiguity as the essential factor for the effect. It is emphasized that Ss report perception of rotation, oscillation, reversal, and flapping, which are all correlates of horizontal contraction and expansion of the retinal image, when viewing rotating objects. The provision of all possible correlates of the transforming array is described as supportive of a functional-evolutionary approach, and Gibson's theory of perception.
