Outcome of rubella during pregnancy with special reference to the 17th-24th weeks of gestation.

The consequences of rubella during pregnancy were studied with special reference to infections occurring during the 17th to 24th weeks of gestation. Laboratory confirmed rubella infected pregnancies were followed and the infants were examined clinically and serologically at a mean age of 20 months. In addition children born to mothers with verified rubella in earlier epidemics were examined clinically at the age of 4 or 7 yr. 491 cases of rubella in pregnant women from 1978-1980 were reported and 118 children were followed-up. Intrauterine transmission occurred in greater than 50% during the 16 first weeks of pregnancy compared to in 17% during the 21st to 24th weeks. Rubella defects appeared in a declining rate from 5/6 congenitally infected during the first 12 weeks to 1/7 at weeks 15 and 16. Three cases with deafness were found among the 65 children from earlier epidemics. Only 1 child with hearing impairment was found among children whose mothers were infected after the 16th week. Although rubella infections during the 17th to 24th weeks of pregnancy result in transmission to the fetus in about one fifth, sequelae seem to be a rare event.

Early assessment of prognosis in Perthes' disease.

A new method or early prognostication of Perthes' disease is presented. It is based entirely on radiological assessment and was developed from a review of 69 radiologic examinations performed during the first year of the disease in 27 patients with unilateral Perthes' disease. A simple mathematical function was constructed with the aid of discriminant analysis of four parameters, viz the metaphyseal width of the femur, the position of the femoral head in relation to the acetabulum, and the acetabular height and breadth.

Supernumerary soleus muscle.

The findings made in cases of supernumerary soleus muscles are reviewed, and the first radiologic observations on this malformation are reported. The value of CT is emphasized.

Voluntary habitual dislocation of the hip in children.

The clinical and radiologic findings in a child with habitual voluntary dislocation of the hip are reported. Observations made in this case and in 6 others on record suggest that this rare condition is a specific pediatric entity with a good prognosis.

Open reduction and pin fixation of severely displaced supracondylar fractures of the humerus in children.

Out of 87 displaced supracondylar fractures of the humerus in children, 17 were treated by open reduction and internal fixation, in eight cases because of vascular complications (with or without neurological complications) and in nine cases after unsatisfactory closed reduction. In the eight cases with vascular impairment, evacuation of the haematoma, reduction of the fracture and division of the fascia of the forearm caused a return of the radial pulse. Median nerve paralysis, in the four cases with neurological complications as well, recovered within 7 months. Fractures fixed with one pin only redislocated and needed an additional reduction--this procedure was associated with impairment of the range of motion. In spite of the selection of the most severe cases for surgery, the end result was excellent in all cases expect one and considerably better than previous results of closed reduction. Rotational deformity, however, did not decrease with age and growth.
