Fine needle aspiration biopsy in the management of palpable breast lesions.

During a 34-month period 2050 fine needle aspiration biopsies of solid or cystic palpable breast lesions were performed. Of the 1182 patients with solid lesions, 384 underwent 405 surgical procedures. Carcinoma was the histologic diagnosis of 118 lesions. Of these, aspiration cytology had identified 92 (78%) as malignant; in 13 (11%) a malignant condition was suspected; 9 (7.6%) had been falsely assessed as benign. Cytology reports were accurate in 237 (83%) of the 287 histologically confirmed benign lesions. There were no false-positive reports. The 243 patients with cystic lesions underwent 572 aspirations for diagnosis and as treatment. Four lesions were excised and found to be benign. Needle aspiration biopsy is a most valuable tool. It is safe, simple, quick, repeatable and easily accepted by patients. Its positive predictive value is excellent (100% in our series). Cytology reports negative for malignant change should be disregarded if cancer is suspected clinically.

Survival in breast cancer: study of 283 patients.

Records of 283 women with breast cancer diagnosed from 1965 through 1973 at l'Hôpital du Saint-Sacrement were studied for survival at 3 and 5 years. These were all patients who had not previously received cancer-directed treatment. Life-table methods based on full intervals only were used to analyze survival. Adjustment for deaths from other causes was provided by computing the expected and relative survival rates. The 5-year relative survival rates (%) were 60.2+/-4.1 for the total group of 283 patients, 87.9+/-4.5 for the 142 with localized disease and 44.9+/-59 for the 118 with regional spread. Of the patients with disease that was either localized or regional, 84% underwent total mastectomy. The age of patients at entry did not differ from that of cases reported to the Quebec Tumour Registry. The significant increase over time in the proportion of patients with localized disease would be expected to reduce the Quebec death rate from breast cancer in future if the better survival of these patients is not simply a function of earlier diagnosis.

Evaluation of variables in immunofluorescence procedures for the detection of antibodies against human herpesvirus 6 (HHV-6).

Ten human sera were used to study different parameters, namely, methods of smear preparation and fixation, and age of infected HSB-2 cells in order to optimize indirect immunofluorescence assay (IFA) and anticomplement immunofluorescence (ACIF) procedures to measure antibody levels against HHV-6. Results showed a greater sensitivity of rapid smear drying and methanol fixation over conventional acetone smear preparation. Cells harvested 6 days after infection and fixed with methanol exhibited a sharper and more intense fluorescence. IFA titers were higher than those obtained with ACIF, although the latter procedure enabled the distinction between three fluorescent sites. Reactivity pattern of individual sera against infected cells was variable and indicated that the human immune response to HHV-6 is directed against different antigens. An easier interpretation and a better definition of the fluorescence of HSB-2 cell line infected with HHV-6 strain Dv is obtained with the following conditions: cells should be harvested at 5-8 days after infection (at the giant cell stage of infection), cell smears have to be dried quickly before fixation with methanol at -20 degrees C, and finally, they should be stained by IFA.
