Airway-like inflammation of minor salivary gland in bronchial asthma.

T-lymphocytes are important components of the inflammatory cell infiltrate in bronchial mucosa in asthma. Because activated lymphocytes migrate through the thoracic duct and the general circulation to remote glandular and mucosal sites, we initiated this study to evaluate the histologic abnormalities of a minor salivary gland (MSG) associated with bronchial asthma. Fifty-eight asthmatic patients were prospectively studied (37 women, 21 men; mean age, 54 +/- 6 yr). Twenty-nine patients had allergic asthma (AA) and 29 had nonallergic asthma (NAA). Results were compared with those obtained from 15 healthy control subjects and 15 nonasthmatic patients with chronic obstructive pulmonary disease (COPD). MSG was normal in 14 of 15 patients with COPD and in 14 of 15 control subjects. Forty-three of 58 (74%) asthmatic patients presented MSG abnormalities. These were significantly more frequent in asthmatics (74%) than in patients with COPD and healthy control subjects (6%, p < 0.001 versus asthmatics) and in NAA (97%) than in AA (52%, p < 0.01). The main pathologic features of MSG in asthma were T-lymphocyte infiltration (57%: AA nine of 29; NAA 24 of 29), partly degranulated mast cells (64%: AA, 11 of 29; NAA, 26 of 29), basement membrane thickening (64%: AA, 11 of 29; NAA, 26 of 29), and endothelial cell changes (26%: AA, one of 29; NAA, 14 of 29). Eosinophils were found only in two cases. Expression of ICAM-1 was demonstrated in nine of 16 patients with NAA, whereas VCAM-1 and E-selectin were negative in all of them.(ABSTRACT TRUNCATED AT 250 WORDS)

[Periosteal osteosarcoma of the jaw. Apropos of 2 cases].

The authors report two cases of periosteal osteosarcoma of the jaws, a tumour which is one of the two variants of juxtacortical osteosarcoma. Periosteal osteosarcoma involving the jaw bones is extremely rare. Because of its chondroblastic differentiation, sometimes predominant, this tumour may pose some problems of differential diagnosis with chondrosarcoma or pleomorphic adenoma: immunochemistry may be necessary. This localization does not change the clinical, radiologic, histologic and prognostic features of periosteal osteosarcoma.

[Treatment with intermittent high dosage corticotherapy in chronic interstitial pneumonia in an infant. A case report].

Chronic interstitial pneumonia (PIC) are rare in infants. We report a case of an infant who presented with a chronic interstitial pneumonia following an infection with a respiratory syncytial virus. The diagnosis was confirmed histologically (open lung biopsy). The failure of oral steroid therapy (Prednisolone 2 mg/kg/day) led to the use of boluses of methylprednisone (30 mg/kg/day, daily for three days in three series one month apart). The treatment led to a progressive improvement and thirty months after the last bolus the respiratory examination was normal as was the radiology. The efficacy of bolus steroid therapy observed by other authors should be confirmed in multicentre trials using surgical biopsy to precise the pulmonary lesions.

[Primary choriocarcinoma of the ovary: a case report].

A case of primary choriocarcinoma of the ovary in a 32-year-old woman, 1 month after a voluntary interruption of pregnancy, is reported. A review of the literature shows that this tumour is rare and the distinction between a gestational and a non-gestational choriocarcinoma is difficult. The distinction between these two diagnoses and specific treatment are discussed.

Tetrachloroethylene and trichloroethylene fatality: case report and simple headspace SPME-capillary gas chromatographic determination in tissues.

We describe a simple, precise, and sensitive assay of tetrachloroethylene and trichloroethylene in tissues, suitable both for emergency cases and forensic medicine. The method employs headspace solid phase microextraction-capillary gas chromatography and electron capture detection. The case is relative to a 45-year-old woman discovered unconscious in a laundry area. The concentrations of the solvents in tissues were determined and compared to other previously published fatalities.

[Congenital human cytomegalovirus infection: value of human cytomegalovirus DNA quantification in amniotic fluid].

A quantitative PCR assay (RS Elosa CMV, Lambdatech) was used to quantitate HCMV DNA in maternal amniotic fluid of 12 fetuses with congenital infection (group 1) and of 10 fetuses without congenital infection (group 2). HCMV detection was performed for both groups using culture and qualitative PCR. Histologic examinations of fetal tissues and placenta were carried out for 9 patients from group 1. The amniotic fluid viral loads were negative in all patients of group 2. In group 1, all viral loads were high (from 1.105 to > 107 cop/mL) and no difference was observed between symptomatic and asymptomatic foetuses. Further evaluation on larger samples is needed to define more precisely the pronostic value of HCMV DNA quantification in amniotic fluid.
