Small cell carcinoma of lung simulating pleural mesothelioma. Report of 4 cases with autopsy confirmation.

Four cases of small cell carcinoma characterized by a striking involvement of the pleural sheets were investigated. The patients, four adult males with a long history of cigarette smoking, presented with symptoms common to pleural tumors, including cough and chest pain. Malignant cells were identified in pleural effusions of all cases and in the bronchial aspirate of one patient. Antitumoral treatments included chemo- and/or radiotherapy. Survival ranged from three to 17 months. At autopsy, the serosal sheets resulted to be extensively involved by thick tumor rinds, associated with massive hilar metastases. Histologically, the tumors were composed of small epithelial cells, expressing carcinoembryonic antigen and keratins and occasionally neuroendocrine markers. Our findings indicate that small cell carcinoma should be added to the group of pseudomesotheliomatous lung carcinomas and, above all, that it deserves distinction from ordinary mesothelioma, especially the small cell variant, because of its medico-legal implications and possible antitumoral treatment.

Paget's disease of the nipple without clinically and radiologically detectable breast tumor. Histochemical and immunohistochemical study of 44 cases.

Paget's disease of the nipple is a rare lesion nearly always associated with an underlying breast cancer, clinically impalpable and radiologically undetectable in about 40% of the patients. Fourty-four cases (28 mastectomies and 16 biopsies of the nipple) of Paget's disease of the nipple without clinically and radiologically detectable breast tumor were retrospectively studied by means of histochemistry and immunohistochemistry. Histochemical study showed that Paget cells were PAS positive and diastase resistant, and alcian blue positive at pH 2.5 in 32% and 18%, respectively. Immunohistochemical study showed that Paget cells were EMA and c-erbB-2 positive in 100% and 84%, respectively. Four of the six EMA positive and c-erbB-2 negative cases of Paget's disease of the nipple in which the underlying tumor could be pathologically analyzed were associated with ductal carcinoma in situ of cribriform or mixed types. These findings are helpful for differentiating Paget's disease from other lesions of the nipple, namely Bowen's disease and eczema which do not react with both antibodies, and from nipple adenoma which exhibits a positive staining with anti-EMA antibody and no reactivity with anti-c-erbB-2 antibody.

Accuracy of bronchial aspiration cytology in typing operable (stage I-II) pulmonary carcinomas.

The accuracy of bronchial aspiration cytology in typing resectable (stage I-II) lung cancer has been investigated in 100 cases, comparing preoperative cytologic features with pulmonary tumor histology seen at surgery. The accuracy has been 100% for small-cell carcinoma (two cases), 98.8% for squamous-cell carcinoma (86 cases), and 91.6% for adenocarcinoma (12 cases). The overall accuracy rate has been 98%. No case of undifferentiated large-cell carcinoma has been identified. It is suggested that the high accuracy in cytologic typing of operable lung cancer is basically related to adequate preservation of differentiation features, thus allowing for correct identification of most non-small-cell carcinoma. Moreover, the absence in this study of any large-cell carcinoma, compared with its frequency in advanced stage series, would indicate that such a histotype reflects excessive dedifferentiation of an original squamous or glandular form.

Aggressive "breast-like" adenocarcinoma of vulva.

A case of vulvar adenocarcinoma seen in a 42-year-old woman is reported. The tumor involved the right labium majus and diffusely spread into the surrounding soft tissues as well as the inguinal lymph nodes and was histologically composed of nests, cords, and tubular formations recalling an aggressive duct carcinoma of breast. Likewise, tumor cells exhibited positivity for common breast tumor markers, such as epithelial membrane antigen, carcinoembryonic antigen, and glandular keratins. The possible origin of the neoplasm from mammary ectopic tissue in vulva or from vulvar skin adnexa is briefly discussed.

Metastatic tumors to the female breast. An autopsy study of 12 cases.

Twelve cases of metastatic tumors to the female breast are reported. Ten of them were incidentally discovered at autopsy; two cases were diagnosed ante mortem but one was misinterpreted as primary. The patients' mean age was 58 years. The metastasizing tumors included cutaneous malignant melanoma (four cases); ovarian, renal and gastric adenocarcinoma (two cases each); and individual cases of pulmonary and pancreatic carcinoma. The patients with melanoma were younger than the others (49.7 vs. 62.7 years). The results of the present study indicate that breast metastases, although rare, are not exceptional especially in large autopsy series. Their recognition in surgical material would result in more adequate treatment against the primary tumor, thereby avoiding unnecessary radical surgery to the breast.

Comparison between diagnoses on death certificates and autopsy reports in Trieste: gynaecological cancers.

We have compared autopsy results with the diagnosis reported on 759 death certificates for gynaecological cancers. High levels of discrepancies were noted for all sites. Surprisingly, complete agreement between death certificates and autopsy reports was found for only 30% of cervical and corpus tumours and for 50% of ovarian tumours. This low level of agreement was due partly to the poor quality of death certificates of older women. No difference in accuracy was found over the 15-year period considered. The most obvious causes of the discrepancies were of two types. The first was erroneous interpretation of codes; confusion between 180 (cervix) and 182 (corpus) was very frequent. The second type was associated with clinical factors, such as confusion of anatomical site, adjacent organs or metastases being diagnosed as primary tumours. A relatively frequent cause of inaccuracy on death certificates was consideration of patients who had been cured of a cancer as having died of the disease.

[Echography and cytology in the study of spreading pathology of the salivary glands].

The contribution of the combined use of US and cytology is evaluated in the diagnosis of masses in the salivary glands and adjacent structures. US had 87.2% sensitivity in locating the mass; its accuracy in defining both physical structure and benign/malignant nature of the lesion was 91% and 74%, respectively. Thus US, after demonstrating a lesion, does not always allow the exact definition of its characteristic. In many of these cases, other imaging modalities do not help either. In our series of cases, cytology allowed an unquestionable diagnosis to be made in 87.2% of cases, and the combined use of US and cytology rose the figure to 97%. The only limitation is the evaluation of the deep extent of large masses: in such cases CT or, if available, MR imaging are recommended.

[Effect of hysterectomy and ovariectomy on the incidence of malignant neoplasms of the female genital tract].

The true risk population for uterine and ovary cancer have been studied, according to the prevalence of histerectomy and oophorectomy in the Trieste area female population. The calculation of the females at risk for these tumors permits to recalculate the incidence rates that are usually underestimated. This adjustment increases the rates in all genital tract tumors from 14% to 18%, but does not modify the relative frequency per age groups or the incidence in the temporal trends. An exception is represented from the uterine corpus cancer in which the correction of the population modifies the temporal trend for the age group over 60 years. The incidences tend in fact to increase significantly more after the adjustment.

[Cervical cytologic screening in the province of Trieste].

The frequency of the Pap test and the incidence of dysplastic and neoplastic lesions of the uterine cervix in the province of Trieste from 1/7/85 to 30/6/88 have been analysed. Over this period, the annual frequency of screening was 11% of the resident female population, with considerable differences from one age class to another. In the age group between 15 and 29, in which CIN 1 incidence is at its maximum, the frequency of screening is much lower than it is among subjects of average age. It is considered that it would be more advisable to detect the cervical lesion in the earliest stage and so bring forward performance of the first Pap test to a younger age.

[Infection of Chlamydia trachomatis and dysplasia lesions of the cervix uteri (CIN)].

With the aim of evaluating the correlation between dysplastic lesions of the uterine cervix associated or not with condylomatosis and Chlamydia T. infection, we carried out a serological study of anti-Chlamydia species-specific antibodies and a direct and/or a cultural investigation on a sample of 320 women who performed a Pap-test. The serological examination showed positivity (titer greater than or equal to 1:32) in 49 control subjects (with negative cytology) (23.7%); in 27 cases (43.5%) with actual diagnosis of condylomatosis/CIN associated or not with condylomatosis and in 28 cases (54.9%) with previous diagnosis of condylomatosis/CIN associated or not with condylomatosis. The cultural investigation did not show any significant difference among the groups above mentioned. Finally, the frequency of positive cases for anti-Chlamydia antibodies was estimated for each grade of actual or previous CIN: a greater frequency of positivity was noted in cases with CIN 3 (75%).

[Endometrial hyperplasia in the province of Trieste].

1103 cases of endometrial hyperplasia were diagnosed between January 1, 1978 and December 31, 1987 at the Department of Anatomy and Pathological Histology of the University of Trieste. Of these, 58% were cases of cystic glandular hyperplasia, 38% of adenomatous hyperplasia and 4% of atypical hyperplasia. Subdividing the period examined into two five-year periods, it was found that the rate of incidence of endometrial hyperplasias was more than doubled, this increase having been caused above all by cases of cystic hyperplasia. Distribution by age classes of subjects suffering from different hyperplastic forms suggests that cystic glandular hyperplasia is not a precursor of the adenomatous form, whereas the latter and atypical hyperplasia might constitute a continuous spectrum of potentially precancerous lesions.

A clinical and epidemiological study of 245 postmenopausal metrorrhagia patients.

The authors report the incidence of endometrial adenocarcinoma and atypical hyperplasia in 245 women who had undergone uterine curettage for post-menopausal bleeding. In 4 cases a stenosis of the cervix precluded the curettage. Of the remaining 241 patients, 71.3% had negative histology; in 24.4% histology was compatible with adenocarcinoma or atypical endometrial hyperplasia; in a third group of 10 patients a different type of gynecological neoplasia was diagnosed. Obese, nulliparous women were more significantly affected by endometrial adenocarcinoma. The highest incidence was noted among women over 60 years of age. The authors describe some epidemiological and clinical characteristics of the population under study.

Correlation between cervical intraepithelial neoplasia and invasive cervical carcinoma on the basis of their frequency in the Province of Trieste (1977-1986).

The Authors report the results of a study on the diffusion of CIN and of invasive carcinoma of the portio in the Trieste population over the 1977-1986 decade. In order to achieve this aim all the bioptical specimens analyzed in the Institute of Pathological Anatomy and Histology of Trieste University were reviewed. Over this period 313 cases of CIN 1-2, 314 cases of CIN 3 and 289 of invasive carcinoma were diagnosed. The average age for CIN 1-2 lesions was 33.5 (range 17-78), for CIN 3 39.9 (range 18-81) and for invasive carcinoma 58.6 (range 26-89). The distribution by age group was markedly different in the three types of lesion dealt with, showing a substantial presence of preinvasive lesions under the age of 50. During the period considered a significant decrease in the incidence of invasive carcinoma was not recorded, in contrast with other reports. This datum may be explained by the fact that the number of women undergoing cyto-oncological screening has not increased over the last decade.

[Condyloma and intraepithelial cervical neoplasms. Epidemiological contribution to their interpretation].

The frequency of condylomatous lesions and of intraepithelial neoplasms of the cervix (CIN) and their possible association in the female population of Trieste have been evaluated. The analysis was carried out on the basis of biopsy samples of the uterine cervix examined at the Department of Pathological Anatomy and Histology of the University of Trieste in the period between 1-1-1983 and 31-12-1987. In the five-year period considered, 272 papillomavirus lesions and 451 cases of CIN were diagnosed. The condylomatous infections were associated with CIN in 48% of cases. An association with CIN 1-2 was encountered in 32.4% and with CIN 3 in 15.4% of cases. CIN 1-2 was associated with condylomatous lesions in 35.2% and CIN 3 in 20.9%. The average age of patients with cervical condyloma was 29.5 years, while that of patients suffering from CIN 1-2 and CIN 3 was respectively 31.9 and 38.7 years. The patients who presented the association between the two pathologies were younger than those suffering from CIN alone: the average age was in fact 28.7 years among women suffering from CIN 1-2 and viral infection and 35.7 in those with condyloma and CIN 3. In the five years examined, a marked tendency was encountered towards an increase in both pathologies examined and this was seen exclusively in the associated lesions.

Cervical intraepithelial neoplasia with conization: early complications and follow-up.

The authors made a review of the last 100 cases who underwent a conization of the cervix during the period 1980-1986. Cyto-histological evaluation was compatible in 78% of the cases with a cervical intraepithelial neoplasia of third degree and in 22% with a CIN of first and second degree. Early post-operative complications consisted of a hemorrhage in 3% of the cases, easily stopped with a vaginal packing. In 2% of the cases a risuture under general anaesthesia was required. In 1 case the stitches loosened and in another a urinary infection was present. Twenty-six patients underwent total hysterectomy after conization for different indications; only one patient had a relapse after six months as there was a moderate dysplasia localized in the vaginal vault. During the follow-up, 14 patient became pregnant. Among these, 5 underwent legal termination during the first trimester and 9 delivered at term without any complication. During these follow-up of 7 years in the older cases and 6 months in the more recent ones, 6 cases displayed a persistence of the disease after 3 months, (CIN of variable degree) and 2 cases relapsed at the second colpocytological control. These patients, according to the age and the presence of clinical indications, underwent total hysterectomy or diathermic coagulation with resolution of the disease. A relapsing invasive tumor was never observed.)

Frequency of cervico-vaginal infections (Trichomonas vaginalis; Chlamydia trachomatis -CHL-; herpes simplex virus -HSV-; human papilloma virus -HPV-) in cervical intraepithelial neoplasia.

The Authors have carried out a cyto-hystologic study on 533 cervical intraepithelial neoplasias (CIN) devoted to identifying the association frequency between cervical dysplastic lesions and cervico-vaginal infections caused by Trichomonas vaginalis, Chlamydia Trachomatis, Herpes Simplex virus and Human Papilloma virus. HPV lesions have revealed the pathology found more frequently in CIN lesions (33.2%) compared to 0.19% in the normal population, while the other infections have not shown significant differences between dysplastic lesions and normal control. In patients with CIN, the assumption of the estro-progestinic pill does not seem to contribute to the increase of frequency of cervico-vaginal infections.
