Does the surgeon's annual case load make a difference in the quality of peripheral vascular surgery? A report of the mortality, morbidity, and long-term results of 101 procedures performed over 93 months.

The outcome of 101 peripheral vascular procedures performed during 93 months was reviewed to determine the quality of the results. Quality was defined as the following: series mortality rate, frequency and significance of complications, cumulative patency rate for bypass grafts, amputation rate, survival following abdominal aneurysm surgery, and neurologic events associated with carotid procedures. The series mortality rate was 4%; 44 procedures were free of complications; patency rates were not significantly different from published reports; the initial amputation rate was 7.4%; and only one out of ten patients was known to have died following aneurysm surgery. Neither of the two carotid procedures included neurologic complications. These data suggest that factors other than a surgeon's annual case load may influence the quality of the results and point out the need to further study this question in light of current admission criteria for the vascular surgery examination.

Target-like skin lesions in primary amyloidosis.

A case of primary amyloidosis is described, during the course of which transient, purpuric haloes appeared around long-standing Campbell de Morgan spots, forming target-like lesions of the skin.

Ultrasound-guided fetal blood transfusion for severe rhesus isoimmunization.

Four fetuses with severe rhesus isoimmunization were transfused with packed red blood cells directly into the umbilical vein. The outcome was successful in three. In one infant, this ultrasound-guided technique resulted in resolution of severe fetal hydrops at 27 weeks, allowing delivery of a healthy nonhydropic infant at 33 weeks, and in the other three infants in prolongation of the pregnancy. The last four transfusions were performed after fetal neuromuscular blockade with curare. The procedure would appear to be associated with a low risk of complication and to provide an excellent chance of a successful outcome of a fetus with severe rhesus isoimmunization even when fetal hydrops is present.

Self-report and heart rate responses to a stressful task.

Data were obtained from a risk task, the zipwire, in order to examine self-report and heart rate responses of males and females. Using this task, heart rate data demonstrated similar patterns to those found in earlier parachuting studies. ANOVAs demonstrated significant differences between sexes in self-report measures, but no significant differences in heart rate. In addition, examination of the relationships between heart rate and self-report showed a positive correlation for the women, while there was a negative correlation for the men.

Immunological abnormalities in bronchiectasis with chronic bronchial suppuration.

Fifty-three patients with bronchiectasis (chronic bronchial suppuration) of unknown cause, in whom the chronic production of purulent sputum was the prominent clinical feature, were investigated for possible immunological abnormalities. They were compared with two control groups comprising 50 patients with chronic bronchitis and emphysema and 33 patients with bronchial asthma. Forty-two patients with bronchiectasis(79%) had at least one abnormality of immunoglobulin, usually elevation of IgA, IgG or IgM. Eight patients had all three immunoglobulin levels raised and this was related to severity of disease. Similar increases in immunoglobulin levels were observed in the control groups, but the frequency and severity of these changes were significantly greater in the bronchiectasis patients. Two patients had IgA deficiency. There was a very high prevalence of rheumatoid factor (52%) and an increased prevalence of antinuclear factor (10%) in the bronchiectasis patients compared with the control groups. The presence of these autoantibodies did not correlate closely with severity of disease. Ten patients with bronchiectasis (19%) had one or more autoimmune disorders, and the association of severe bronchiectasis, Hashimoto's thyroiditis and pernicious anaemia in one patient is described in detail. The immunoglobulin changes, high incidence of autoantibodies and association with autoimmune disorders raises the possibility that in some patients with bronchiectasis (chronic bronchial suppuration) of apparent unknown cause abnormal immune mechanisms may be important in causing or perpetuating the condition.

Erythema gyratum repens with pulmonary tuberculosis.

A 63-year-old man presented with erythema gyratum repens of 7 months' duration. A cavitating mass at the right lung apex was resected and proved to be tuberculous. Following the resection, the skin lesions cleared within a few days. Erythema gyratum repens has not previously been described in association with non-malignant visceral pathology. The pathogenesis remains obscure but cannot be related specifically to a response to tumour cells or their products in view of the association reported here. The condition bears no resemblance to any known tuberculide.
