Recovery of specific antigenic characteristics using microwave treatment of paraffin sections of tissues fixed by formalin allows to extend spectrum of antibodies for immunohistochemical diagnosis. Microwaves enable the reaction on the material prepared according to the standard technique, and macropreparations long stored in formalin and archive blocks.
Neuroendocrine tumors of the pancreas were studied by histology, immunohistochemistry and electron microscopy. Tumors associated with organic hyperinsulinism showed heterogeneity in their ultrastructure, functional and proliferation activity. More than half of these tumors are multihormonal and 15% of them developed from polypotent cells which are capable for several types of differentiation.
The paper is devoted to immunohistochemical (IHC) investigation of various benign and malignant proliferating lesions in the thyroid with a special focus on borderline lesions--atypical adenomas (AA) and focal adenomatous areas (FAA). All papillary carcinomas (PC) and FAA in Hashimoto thyroiditis demonstrated strong immunoreactivity for CK-19. More than 50% of PCNA-positive cells were detected in most of the investigated carcinomas; in FAA, and in AA PCNA expression was not so prominent but higher than in other adenomas and nodular goiters. Strong expression of vimentin by follicular epithelium was detected in PC. In FC, AA and FAA it was focal from moderate to strong. In other cases immunoreactivity to vimentin was weak or absent. Thus, it is worth to form on the basis of histological and IHC results the group of thyroid proliferating processes with possible malignant transformation. AA and FAA with more than 10% PCNA-immunoreactive cells and vimentin expression from moderate to strong should be included in this group. Further observation of our patients will enable to assess malignant potential of this processes. Besides, CK-19 expression is an additional criterion for the diagnosis of early PC.
Surgical biopsies from 21 patients with syndrome of organic hyperinsulinism were studied. Precise quantitative characteristics of "typical" and "atypical" beta-granules in tumor cells were obtained by means of quantitative and qualitative analysis. The criteria for establishing the tumor type (mono- or polyhormonal) and the degree of hormonal activity are established. The validity of these criteria is confirmed by positive correlation between the secretory activity of the tumor and insulin level in the patient's blood.
Literature data and three cases observed by the authors are described. Extrapulmonary inflammatory myofibroblastic tumor (EIMT) presented with proliferation of spindle cells with myofibroblast phenotype. Frequent recurrences and indefinite prognosis make to treat these lesions as true tumors. EIMT malignancy hyperdiagnosis is possible as morphologically it and some soft tissue sarcomas are alike.
Invasive potential of ductal tumors (adenocarcinoma, mucinous-cystic tumors, gastrinoma and undifferentiated tumors) positively correlated with the degree of metalloproteinase-2 expression and negatively correlated with stromal content of type IV collagen.
Epithelio-myoepithelial carcinoma was singled out as a separate nosological unit in the second revision of the International Histological Classification of Tumors of the Salivary glands (WHO, 1991). For the first time in domestic literature the article describes morphological evidence obtained at investigation of epithelial-myoepithelial carcinoma of the parotid gland using modern technologies.
Certain differentiation-specific antigenic determinants are characteristic of pancreatic cells at each stage of embryonic development. Using immunohistochemical markers, we have demonstrated that the cells of several pancreatic tumors express some fetal proteins characteristic of embryonic cells of the pancreas. We propose that the presence of fetal antigens and proteins in the tumor is a sign of a lower tumor differentiation and a less favorable prognosis.
Oncocytic adenomas have primarily follicular structure; trabeculas, solid areas, necrosis are rare. They may possess malignant potential as their malignant transformation occurs in 35% cases against 5% in adenomas of follicular cells. Oncocytic follicular carcinomas can be hardly distinguished from oncocytic adenomas. Tumors larger than 4-5 cm in diameter are considered to be malignant. Main difference with adenomas is invasion into the capsule surrounding thyroid or into the vessels. They can be well or poorly differentiated or anaplastic. Oncocytic papillary carcinoma and oncocytic medullary carcinoma are rare. The clinical course of oncocytic tumors is more aggressive than that of tumors from follicular cells. Of key importance in differential diagnosis is electron microscopy (EM) and immunohistochemistry with antimitochondrial antibodies. EM may be also useful in determination of the degree of oncocytic tumors maturation.
15 cases of thyroid insullar carcinoma in patients aged from 10 to 65 years are described. It is shown that the tumor has aggressive histological signs and represents a prognostically important form of thyroid carcinoma.
In spite of a large size, mesenchymal tumor of the small pelvis was not diagnosed clinically, and hypoglycemia was considered as a manifestation of hyperinsulinism. The tumor was identified as mesenchymal, most likely fibrosarcoma, immunohistochemically. The use of antibodies against insulin, glucagon and somatostatin showed the disturbance in the balance of cells in the hyperplastic islands of the pancreas. Multifocal hyperplasia of the islands is considered as a compensatory adaptive reaction to paraneoplastic extrapancreatic hypoglycemia.
