Gastric cancer in the Korean-American: cultural implications.

Although the overall incidence of gastric cancer in the United States is decreasing, Korean-Americans are increasingly at risk of developing this type of cancer. The epidemiology of gastric cancer in the Korean population is associated with environmental and genetic factors. Current treatment modalities include surgery, chemotherapy, and radiation. A case study is presented of a Korean patient who elected to participate in an investigational research protocol at the University of Southern California/Kenneth Norris Jr. Cancer Hospital. The paper also contains a discussion of transcultural nursing focusing on the background, values, and healthcare practices of Korean-Americans and the associated nursing implications.

Independent regulation of mouse VL30 retrotransposon expression in response to serum and oncogenic cell transformation.

The nucleotide sequence of the long terminal repeats (LTRs) of retrovirus-transmissible mouse VL30 cDNA clones, NVL-1 and NVL-2 were determined and compared with that of the prototype NVL-3. Both shared the typical U3 R U5 structure together with unusual features of redundancy in the tRNAgly primer binding site and adjacent inverted repeat. NVL-1 and NVL-2 LTRs were almost identical and differed from the NVL-3 LTR in the U3 domain harbouring transcriptional regulatory determinants. S1 nuclease analysis of cellular and virus-encapsidated RNA suggested that NVL-1/2 and NVL-3 elements retrotranspose with comparable efficiency but that in contrast to transformation-regulated VL30 expression which affects all types of NVL element, only NVL-1/2 elements were found to be serum responsive. Both modes of VL30 regulation were found to be coupled through protein kinase C-independent pathways. Expression of N-ras transactivated U3 enhancer determinants in all classes of LTR. However the same region of NVL-1/2 LTR did not confer serum responsiveness implying that cis regulatory determinants of VL30 elements mediating growth factor responsiveness are at least in part dissociable from those responsible for cell transformation-regulated expression.

Enhancement of early human E rosette formation by cholinergic stimuli.

The effects of the cholinergic stimuli carbamylcholine (carbachol) and dibutyrl cyclic guanosine monophosphate (DBCGMP) were determined on both 'early' and 'total' E rosette formation. Ficoll-Hypaque-separated lymphocytes were preincubated with either carbachol or DBCGMP over a 10(-3) M to 10(-13) M dose range. Both agents significantly enhanced 'early', but not 'total' E rosette formation. Peak enhancement above control values occurred at 10(-7) M (72%) and 10(-9) M (69%) for carbachol and 10(-5) M (70%) and 10(-7) M (70%) for DBCGMP. Kinetic studies showed a rapid onset of enhancement (2.5 min) for carbachol, whereas DBCGMP required 15 min for significant enhancement to occur. The muscurinic nature of carbachol enhancement of E rosettes was demonstrated. Atropine at 10(-7) M completely abolished the carbachol effect while showing little inhibition of the DBCGMP effect on rosette formation. These studies indicate that the cholinergic stimuli carbachl and DBCGMP significantly enhance the 'early' E rosette former in man. Human T lymphocytes appear to have functional cholinergic receptors that can be blocked by the muscurinic antagonist atropine. The role of the cyclic nucleotides and their stimulants on the immune system is incompletely understood, but it would appear that they are extremely important in the differentiation and function of the T lymphocyte. E rosette formation may be a useful model in man for studying the effects of the cyclic nucleotides on the human T lymphocyte.

Nursing in prisons. Health and young offenders.

This article focuses on a recently opened unit, Lancaster Farms, which houses over 300 young inmates, a proportion of whom cannot read and write. Nursing staff at the unit have cooperated with colleagues in other settings to produce an innovative programme designed to promote healthy lifestyles. Courses are available in areas ranging from parenting skills to sexual health. More importantly, perhaps, the unit is fostering an ethos that discourages bullying--arguably an experience which makes victims' lives so miserable that they would be unreceptive to health promotion initiatives.
