[Generalised fecal peritonitis. Etiopathogenic and therapeutic considerations in 30 cases (author's transl)].

The authors analyse a series of 30 cases of generalised fecal peritonitis which represents a less frequent form of peritoneal infection. The etiology was mainly diverticular perforation (50 p. cent) or neoplasia (25 p. cent). The mortality was high (63 p. cent). It seems, in the light of the results, that the best treatment consists of operating in two stages, the first being the eradication of the colonic lesion which should be carried out as a routine without attempting immediate anastomosis. Wide drainage with peritoneal lavage and the use of "noxythioline" may improve the results of a disease which always remains very severe.

[Postoperative enterocutaneous fistulae of the small gut (author's transl)].

Postoperative enterocutaneous fistulae are fortunately rate (1 : 1000), but they carry a mortality of about 50%. The authors analyse the development and treatment of fistulae in 30 patients. This depends on whether the cause of the fistula is connected with an anastomosis or not, on a variety of pathologic-anatomical criteria and in particular on the amount of fluid lost daily. If this loss is less than 500 ml/24 h the fistula can be regarded as relatively benign and in all probability conservative treatment alone will succeed. Fistulae with a fluid loss of more than 500 ml/24 h have a worse prognosis and require a combined conservative and surgical therapy. The key to this problem is the optimal time for surgical intervention, it lies around the 4th week. In case an early operation is necessary one must be prepared for an enterostomy or by-pass procedure.

Drainage↗

[Surgical treatment of acute pancreatitis].

In the initial stage the treatment of acute pancreatitis must be medical. If there is not positive improvement after 2 to 6 days or in cases of associated cholelithiasis, surgery is indicated. The first step in surgery is a search for necrosis and hematomas, and their exact topographic localization. These findings will determine the final tactics, which may be drainage only, partial or exceptionally, total resection, or necrosectomy (which is not to be confused with a sequestrectomy !). Indications for these different types of operation are outlined and discussed. They are supplemented by large-scale drainage/lavage of the abdomen, choledocostomy or at least cholecystostomy, and temporary exclusion of the upper portion of the digestive tract. 30 personal cases treated according these principles are discussed.

Acute Disease↗

Appendicular mucocele and peritoneal pseudo-myxoma.

From 2 personal cases the authors present a pathological appraisal of appendicular mucoceles. They distinguish between a benign kind with an atrophic mucous membrane and a malignant type with a hyperplasic epithelium arranged in papillary and vegetating structures; this is a neoplasm with local malignancy. This variety remains the most liable to become a peritoneal pseudo-myxoma representing the most serious complication of the appendicular mucecele. Treatment of the mucocele, which consists in an appenicectomy or a right bemi-colectomy, usually yields good results provided operative rupture can be avoided. Contrarily the peritoneal pseudo-myxoma remains a serious disease with a disappointing treatment.

[The radiologic aspects after highly selective vagotomy (author's transl)].

The purpose of this study is to report the early radiologic modifications observed among 72 patients operated for duodenal ulcer. Each patient has been subjected to a double control, the first eight days and the second eight weeks after the operation. The first transit permits to judge the efficacity and quality of the highly selective vagotomy and shows the permanent and transistory modifications of the cardia, the stomac, the duodenum and the small bowel; the second permits to study the condition of the ulcer.
