Metastatic choroidal choriocarcinoma: a clinicopathological study.

A previously healthy 33-year-old woman presented with decreased vision and pain in her left eye. Funduscopic examination revealed a mass in the posterior pole of the eye, and chest radiography showed a lesion in the lower lobe of her left lung. The eye became painful and blind over 12 days and was enucleated. The tumour had the typical histopathological and immunohistochemical features of choriocarcinoma: sheets of cytotrophoblasts with pleomorphic nuclei and lacy cytoplasm intermingled with multinucleated syncytiotrophoblasts. The neoplastic cells stained positively for cytokeratin and the beta subunit of human chorionic gonadotropin (beta-HCG). The serum beta-HCG level was markedly elevated, at 82,000 (normally less than 5) IU/L. The patient was treated with combination chemotherapy, and normal serum beta-HCG levels were obtained. Thirty months after presentation the serum beta-HCG level was normal and there had been no clinical evidence of recurrence. To our knowledge this is the second case of metastatic choriocarcinoma in the eye in which the ocular tumour caused the presenting symptom.

Depot triamcinolone-induced glaucoma.

The authors report two cases of recalcitrant ocular hypertension persisting 6 1/2 and 10 months following subconjunctival injections of 40 mg of triamcinolone acetonide. In both instances, the glaucoma resolved promptly after the whitish plaque of residual steroid was surgically removed.

Vitrectomy in retinal detachment surgery.

Pars plana vitrectomy has provided an approach to those situations in which treatment of a retinal detachment becomes impossible due to vitreous pathology. The records of the first 207 pars plana vitrectomies performed by one of us (M.S.) were reviewed and attention was focused on the combined vitrectomy-scleral buckle procedures, particularly with regard to anatomical reattachment and visual improvement. This group was then divided into those in which a one-handed technique was used (29 procedures on twenty-eight eyes, involving 27 patients) and those in which a two-handed technique was used (22 procedures on twenty-two eyes of twenty-two patients). A combined vitrectomy scleral buckle approach has succeeded in reattaching the retina in a number of cases and provided a number of patients with worthwhile visual improvement.

Energy: a useful criterion in analysis of human visual evoked response.

Visual evoked responses from a diffuse Xenon flash were studied for seven normal and 14 abnormal subjects. Amplitude and latency measurements of the waveforms were compared with an energy function calculated from the area beneath a curve of the VER voltage squared and summed over the interval from 0 to 125 msec. This energy function proved superior to latency in detecting asymmetrical responses and similar to latency in detecting symmetrically reduced waveforms.
