Based on a personal observation and an analysis of the recent literature, the characteristic findings of Pseudoxanthoma elasticum are illustrated and the syndrome its diagnosis, differential diagnosis, pathogenesis and prognosis are summarized. Although the major dermatological and ophthalmological signs are highly characteristic, dermatologists or ophthalmologists are quite often late in diagnosing the condition.
A 73-year-old patient suddenly became confused and comatose due to excessive hypercalcemia. He developed acute respiratory distress syndrome (ARDS) and died even though the hypercalcemic state had been brought under control with mithramycin. Autopsy disclosed an adenoma of the parathyroid gland and calcifications in lung and kidney tissues as well as hyaline membranes. The case illustrates the poor prognosis of hyperparathyroid crisis whenever organ damage is present.
A case of Campylobacter jejuni meningitis with associated septicemia and positive stool culture in a normal host is presented. Clinical findings, course of the illness and serologic data are compared with data from the literature. The possibility of infection due to a serum-resistant strain of Campylobacter jejuni is raised. Retrospective analysis of immunity revealed normal humoral and cellular immunity in a healthy patient one year later.
