A model state-level approach to family survival for caregivers of brain-impaired adults.

This profile of a statewide program to support family caregivers of brain-impaired adults identifies the major needs and problems of these family caregivers and delineates service interventions to meet those needs. Demographic data on the major characteristics of over 3,000 family caregivers and their cognitively impaired patients are reported. A discussion on addressing brain impairment as a broad category of disability needing an array of policies and programs regardless of individual diagnostic categories, is provided.

Geographic differences in the use of Veterans Administration hospitals.

This study contrasts the determinants of community hospital utilization with Veterans Administration (VA) hospital utilization using traditional planning variables. The comparisons had some expected and some unanticipated findings. Regional differences in non-VA hospital admissions and bed days are fairly well explained by measures of medical need, provider supply, community alternatives, and sociodemographic characteristics (other than those used as proxies for case mix). However, regional variations in the VA are explained less well by the same classes of variables and the unexplained differences between the two systems do not correspond geographically. This suggests that the two systems have different reasons for regional variation. Further, contrary to expectation, when other predictors are held constant, excess bed capacity in the area does not correlate with lower VA utilization. The study is important as the VA comes under increasing pressure to contain costs. It may well be that the rational planning model attributed to the public sector is less likely to overcome maldistribution than the private sector 'invisible hand'. Policy analysts need to give more attention to the political, bureaucratic determinants of resource allocation before changing eligibility criteria or merging the two systems.

Simultaneous strategies for solving the nursing shortage.

An overview of the nursing employment system is presented: recruitment, education, hospital careers, licensure, compensation, and alternative careers. Simultaneous changes in pay and licensure/education are advocated for developing a career structure attractive to contemporary workers.

The nursing shortage: do we dislike it enough to cure it?

The current, and rapidly worsening, nursing shortage is likely to continue in the absence of fundamental changes in nursing education and salaries. Because both hospitals and nurses themselves seem to benefit in the short term from the current education and salary structure, I suggest that the fundamental change necessary to ensure an adequate supply of high-quality nurses in the future will have to come from outside these two constituencies. In this paper, I propose a long-range solution that comprises three basic proposals: 1) Create a nursing education system that clearly differentiates, in terms of salary and certification, between programs that educate registered nurses and associate degree nurses and programs that educate bachelor's and advanced-degree nurses. 2) Expand the nursing salary structure to reward long-term, career-track nurses. 3) Assign nursing responsibilities according to nurses' educational and certification qualifications.
