Changes in apolipoprotein AI after treatment with high-dose medroxyprogesterone acetate.

21 women (mean age 60 years, range 48-72) were given depot medroxyprogesterone acetate (DMPA), 1,000 mg/week parenterally for 6 months, as part of the treatment for endometrial carcinoma in either clinical stage I or II. Before treatment and after 1, 3 and 6 months of treatment serum apolipoprotein AI was analyzed by electroimmunoassay. There was a significant decrease in apolipoprotein AI after the administration of DMPA, compared to the value before treatment. A low level of apolipoprotein AI is considered a risk factor for the development of atherosclerosis and cardiovascular disease. Such a risk might therefore be anticipated if the period of treatment was extended to several years.

Primary adenocarcinoma of the uterine cervix. A clinical study.

In a retrospective clinical study, 67 matched cases of primary adenocarcinoma and squamous cell carcinoma of the cervix were compared. The mean age, weight, and menopausal age of women with adenocarcinoma of the cervix were in excess of the corresponding figures for squamous cell carcinoma. There was a significantly (P less than 0.05) larger number of diabetics and nulliparous women amongst the patients with adenocarcinoma of the cervix. Blood group A was significantly (P less than 0.05) more frequent in patients with cervix cancer. Treatment choice was independent of the tumors histological type. The five-year survival rates for adenocarcinoma of the cervix were 83.3% (Stage IB), 60.0% (Stage IIA), and 45.5% (Stage IIB). The corresponding five-year survival rates for squamous cell carcinoma were 90.0%, 73.3%, and 40.9%. There were indications that adenocarcinoma of the cervix was less radiosensitive than squamous cell carcinoma. Estrogen substitution following treatment of adenocarcinoma of the cervix did not adversely affect the outcome.

Muscle substrate levels, muscle enzyme activities and muscle morphology in the vastus lateralis and deltoideus muscles in normal children and in children with coarctation of the aorta.

Muscle biopsies from the deltoideus dx and vastus lat. dx muscles were taken in 17 children with coarctation of the aorta, aged 5.0 to 13.8 years, prior to surgery. Higher concentrations of glycogen, ATP and CP were found in the vastus lat. muscle compared to the deltoideus muscle. The same differences between these two muscles were also found in healthy controls. No differences were found between the patients with coarctation of the aorta and the control group. Nor were any differences found for the other variables studied; glucose, glucose-6-phosphate, lactate, muscle enzyme activities (SDH, LDH and phosphorylase), muscle fibre composition or fibre sizes. It seems reasonable to assume that the differences in muscle substrate levels found between the vasus lat. and the deltoideus muscles in the two groups were due to a higher degree of activity during daily life for the legs as compared to the arms. Patients with coarctation of the aorta do not seem to be influenced by the altered haemodynamic situation with regard to the studied variables.

Comparisons of muscle substrate levels, muscle enzyme activities and muscle morphology in arm and leg muscles in normal subjects and in patients operated upon for coarctation of the aorta in childhood.

Muscle biopsies from the upper and the lower part of the body (m. deltoideus dx and m. vastus lat dx) were taken in young men, 17-28 years of age, who had been operated upon for coarctation of the aorta in childhood. Significant differences regarding SDH-activity and some muscle substrate levels were found, with higher values in the leg muscle. The same differences were found in ten healthy controls. It is concluded that arm and leg muscle differ in these respects but that the haemodynamic difference remaining after coarctectomy does not seem to influence this situation.

Do estrogen and progesterone receptors (E2R and PR) in metastasizing endometrial cancers predict the response to gestagen therapy?

In a prospective investigation biopsy specimens were obtained of 150 women with primary, untreated endometrial carcinoma for determinination of the content of estrogen and progesterone receptors (E2 and PR). After a varying interval 13 who developed metastases despite routine surgical and radiation therapy received treatment with gestagens. The clinical effect of the hormone was studied for any correlation with the initial content of E2 and PR. Eight of the women (61--71 years) proved E2R positive (greater than 10 fmoles/mg cytosolprotein), 2 had no PR and the remaining 6 had PR in a concentration of 7--892 fmoles/mg cytosolprotein). 7 responded clinically to gestagen therapy (tumors poorly differentiated in 3 and moderately in 4). Five of the women (50--84 years) were E2R negative; they had no PR, they did not respond to gestagen therapy and died after 2--8 months (tumors poorly differentiated in 2 and moderatley in 3). Determination of E2R and PR in endometrial carcinoma appears to be a clinically useful test for predicting whether a patient is likely to respond to treatment with gestagens.

On receptors for estrogens (E2) and androgens (DHT) in human endometrial carcinoma and ovarian tumours.

Some human endometrial carcinomas contain receptors for estrogen (E2) and dihydrotestosterone (DHT). Analysis of 50 primary endometrial carcinomas by agar gel electrophoresis revealed a varying receptor pattern, absence of both receptors, sole presence of E2- or DHT- receptor, or presence of both. Highly differentiated tumours often had receptors of both types (7/18) and poorly differentiated tumours most often lacked them (1/10). Primary tumours with metastases (Stages III-IV) seldom had combined receptors, only in 2 cases out of 9. Growth of the tumour down in the cervical canal (Stage II) implied absence of combined receptors in all of the 13 cases studied. In 8 different ovarian tumours, presence of either E2- or DHT-receptors was found in 6. The findings are discussed in relation to hormonal treatment of the disease.
