Biochemical screening programme in general practice: a clinical follow-up.

Follow-up studies of findings obtained during a short programme of biochemical screening conducted on 1,041 patients attending their general practitioners in January 1967 showed that this yielded 64 results that led to a change in diagnosis or method of treatment. These results were obtained in 57 patients (5.5% of patients screened), whereas only 15 abnormal results would have been revealed on 14 of the 1,041 patients if the tests performed had been restricted to those requested on the basis of clinical features. Hence a screening programme can make an important contribution to the recognition of otherwise unsuspected disease.

Equipment for linking the AutoAnalyzer on-line to a computer.

An Elliott 903 computer with 8K central core store and magnetic tape backing store has been operated for approximately 20 months in a clinical chemistry laboratory. Details of the equipment designed for linking AutoAnalyzers on-line to the computer are described, and data presented concerning the time required by the computer for different processes. The reliability of the various components in daily operation is discussed. Limitations in the system's capabilities have been defined, and ways of overcoming these are delineated. At present, routine operations include the preparation of worksheets for a limited range of tests (five channels), monitoring of up to 11 AutoAnalyzer channels at a time on a seven-day week basis (with process control and automatic calculation of results), and the provision of quality control data. Cumulative reports can be printed out on those analyses for which computer-prepared worksheets are provided but the system will require extension before these can be issued sufficiently rapidly for routine use.

Association of inorganic-pyrophosphatase activity with human alkaline-phosphatase preparations.

1. The inorganic-pyrophosphatase activity of alkaline phosphatases prepared from human liver and small intestine was investigated at different stages of purification. 2. Both liver and intestinal preparations possessed pyrophosphatase activity at all stages of purification, and the two types of activity were not separated by gel filtration or by anion-exchange or cation-exchange chromatography. 3. After starch-gel electrophoresis of the tissue extracts, the zones of pyrophosphatase activity coincided exactly with alkaline-phosphatase zones. 4. Hydrolysis of each type of substrate was inhibited by the presence of the other, and a constant ratio of alkaline-phosphatase activity to pyrophosphatase activity was maintained during inactivation of the enzymes by incubation at 55 degrees . 5. These results are consistent with the view that alkaline phosphatases are also inorganic pyrophosphatases.

Serial laboratory reports and records for chemical analyses.

A system of reporting laboratory data is described which provides serial and composite presentation of the results of chemical analyses as reports in the patients' notes in the form of copies of serial and composite records which are prepared and kept in the laboratory. The system has been used for seven months, providing reports for patients in 250 general medical beds, which generate an annual workload of approximately 90,000 chemical tests.
