[The clinical picture in shunt nephritis].

Report on three children with hydrocephalus in whom an immune complex nephritis ("shunt-nephritis") had occurred in consequence of an infected atrioventricular shunt. Besides the clinical course, the histological findings which are of particular significance with regard to pathogenesis are discussed as well as the serum complement system on the basis of the laboratory data. Removal of the valve system led to a complete normalization of the laboratory parameters and to a subsidence of the clinical symptoms in all patients. The necessity of a perioperative antibiotic prophylaxis in the insertion of an atrioventricular shunt is explicitly emphasized. Narrow-interval follow-up investigations should enable early detection of cases of shunt nephritis and thus prevent functional residues or even fatalities. Knowledge of this immune complex nephritis is also very important because it is one of the few forms of nephritis in which a causal therapy is available with removal of the infected shunt.

Results of the modified Hartl gracilis plasty.

Follow-up results of patients who had a gracilis plasty for incontinence after rectal atresia showed that in 8 of 11 patients there was marked improvement. 6 children had good continence and 3 sufficient continence. Similar results have been reported by others and are encouraging. The high percentage of infected wounds did not seem to influence the results. The results in operations for incontinence with myelomeningoceles were different. Only 5 times was there sufficient continence and in only 2 patients was subjective improvement recorded. It must be pointed out that the function of the gracilis muscle was tested preoperatively by electromyographic examination. These unsatisfactory results can undoubtedly also be achieved with conservative therapy. Incontinence with myelomeningoceles is therefore no indication for a gracilis plasty. These varying results appear to indicate that success can only be obtained with a gracilis plasty if there are still functioning parts of the pelvic musculature present and if parts of the anorectum are able to mature. If this is true, then there is a direct connection between the results we obtained and those achieved by free muscle transplantation or transposition of a denervated muscle. No doubt, the gracilis plasty is not the only available method to achieve continence, but it will often improve the function of the sphincter in patients with incontinence following rectal atresia operations.

[Clinical findings and diagnostic problems in a case of shunt nephritis. (author's transl)].

A report is presented on a nine year old girl in whom a Spitz-Holter valve had to be implanted after operation on a meningomyelocele while a neonate. Because of a macrohematuria and a hypochromic anemia, she was admitted as an inpatient. In addition, there was a pyuria with bilateral hydronephrosis. The clinical picture was initially misinterpreted as hemorrhagic cystitis with ascending pyelonephritis in neurogenic bladder. Only the reduction of serum complement suggested the presence of a shunt nephritis. The diagnosis was verified by kidney biopsy. After removal of the infected valve system, there was a prompt normalization of all laboratory parameters. Besides description of the case history, above all the diagnostic problems of this rare syndrome, which is nevertheless of practical importance, are dealth with.

[Application of acerbine, a mixture of organic acids containing propylene glycol as lotion or ointment in paediatric surgery (author's transl)].

Acerbine has been available for clinical trial since three years ago. Promotion of epithelialization, as well as antiphlogistic, antibacterial and fungicidal actions have been attributed to this preparation. This paper presents a critical appraisal of our own results with Acerbine; initial scepsis was soon dispelled by the evident efficacy of this preparation, both in the form of a lotion and ointment, and its wide scope of application. Moreover, our own studies revealed an excellent effect on keloids, which became smoother and visibly paler. A very good prophylactic and curative effect was observed in respect to all types of skin transplantation. So far Acerbine has been successfully applied in 117 patients who have completed their course of treatment. Acerbine is being currently employed in the case of 16 further patients who are still in hospital. No untoward side effects were recorded. Our findings confirm the opinion of other authors that hospitalization is shortened by the use of this preparation. The availability, more recently, of the new preparation in form of an ointment enables treatment to be continued on an ambulant basis or at home in many cases. Certain new fiies. The results of further explorative investigations into these aspects are under review at present.
