Evidence for highly charged ion Coulomb crystallization in multicomponent strongly coupled plasmas.

Multicomponent non-neutral ion plasmas in a Penning trap consisting of Be(+) and highly charged Xe ions, having different mass-to-charge ratios than Be(+), are cooled to form strongly coupled plasmas by applying a laser-based collisional cooling scheme. The temperature of the plasma was determined from a Doppler broadened transition in Be(+). For the Xe ions, which are centrifugally separated from the Be, the Coulomb coupling parameter was estimated to be approximately 1000. Molecular dynamics simulations of the ion mixture show ordered structures, indicating crystallization of the Xe.

Assessing intraocular lens calcification in an animal model.

PURPOSE: To describe an animal model used to evaluate the propensity of various biomaterials to calcify intraocularly. SETTING: Research Department, Allergan Inc., Irvine, California, USA. METHODS: Intraocular lens (IOL) optic materials were implanted intramuscularly and/or subcutaneously in rabbits for up to 90 days. The materials included silicone, poly(methyl methacrylate) (PMMA), hydroxyethyl methacrylate hydrogel, and several hydrophobic acrylic materials. Scanning electron microscopy (SEM) and energy dispersive x-ray spectroscopy (EDS) were used to detect calcification demonstrated by characteristic discrete nodules containing both calcium and phosphate. Histological methods were used to evaluate tissue reactivity. Disc lenses fabricated from the experimental material were also bilaterally implanted in rabbit eyes that were monitored by slitlamp biomicroscopy. The lenses were explanted at 1, 2, 5.5, 10, and 20 months for SEM/EDS analysis. RESULTS: No calcification was noted in the intramuscularly or subcutaneously implanted silicone, PMMA, and acrylic optic materials. Calcification was noted on the intramuscularly, subcutaneously, and intraocularly implanted experimental acrylic and the intramuscularly implanted hydrogel material; the calcification was more extensive on the hydrogel. Signs that suggested intraocular calcification were first noted on the experimental IOLs at 4 months, but calcification was not confirmed until 10 months. CONCLUSIONS: Material calcification occurred more quickly in an intramuscular or subcutaneous environment than in an intraocular environment. Intramuscular and subcutaneous implantation appears to be an excellent model for screening materials for calcification potential. However, calcification is both host environment and material dependent. Using intramuscular or subcutaneous implantation in animal models to predict intraocular calcification in humans must be done with caution.

Using artificial neural networks to model the urinary excretion of total and purine derivative nitrogen fractions in cows.

A dataset of 177 individual nitrogen balances from dry and lactating cows was split in two independent groups: training dataset (n = 130) and challenge dataset (n = 47). The training dataset was used to develop multiple linear regressions (MLR) and artificial neural networks (ANN) aimed at predicting the urinary excretion of total (NURI) and that of purine derivative nitrogen (PDN). Input variables for the prediction of NURI were crude protein (CP) intake, effective degradability of non-protein dry matter (DM), neutral detergent fiber (NDF) content of the diet, live weight and milk yield. Live weight, total carbohydrate intake, the ratio of non-protein DM degraded to CP degraded and milk yield corrected for DM intake were entered to predict PDN. The regression between predicted and observed values for the training dataset showed a better statistical accuracy of ANN than did MLR models, especially for PDN. The evaluation of the two models on the challenge dataset showed similar determination coefficients, either when predicting total nitrogen excretion (0.623 and 0.614 for ANN and MLR, respectively) or PDN (0.688 and 0.666, for ANN and MLR, respectively). Moreover, both approaches were affected by a tendency to under-predict both targets at high levels of NURI and PDN. However, with the ANN approach, it is possible to study the response of the model to modifications of individual inputs by the so-called response analysis. This unique feature could be used to study the effect of different physiological situations as well as providing hypotheses for additional research.

Vitreoretinal surgery through multifocal intraocular lenses compared with monofocal intraocular lenses in fluid-filled and air-filled rabbit eyes.

PURPOSE: To compare vitrectomy procedures and visualization of posterior segment structures through multifocal silicone intraocular lenses (IOLs) with the same procedures through monofocal silicone IOLs in rabbit eyes. DESIGN: Experimental study. PARTICIPANTS: Twelve eyes of six rabbits. METHODS: Each rabbit eye underwent phacoemulsification of the lens and posterior chamber implantation of a silicone multifocal or silicone monofocal IOL. The type of IOL (monofocal vs. multifocal) implanted in the first eye of each rabbit was randomly decided. The fellow eye then received the other IOL type. Vitrectomy procedures were performed through the IOLs by using a flat contact lens (part 1) or wide-angled contact lens (part 2) for visualization through fluid-filled (parts 1 and 2) and air-filled (part 2) eyes. MAIN OUTCOME MEASURES: Image quality, stereopsis, and contrast were subjectively graded on a scale of 0 (none) to 4 (excellent) for each eye by each surgeon. RESULTS: In part 1, image quality averaged 4 for the monofocal IOL and 3.6 for the multifocal IOL. Stereopsis averaged 4 for the monofocal IOL and 4 for the multifocal IOL. Contrast averaged 4 for the monofocal IOL and 3.9 for the multifocal IOL. Vitrectomy with retinal surface maneuvers was successfully performed in both pigmented and nonpigmented rabbit eyes through both IOL types. In part 2, image quality, stereopsis, and contrast were rated as 4 for both multifocal and monofocal silicone IOLs. Air-fluid exchange was performed without difficulty. Image quality, stereopsis, and contrast were rated as 4 for air-filled eyes. CONCLUSIONS: Visualization of posterior segment structures through multifocal silicone IOLs was sufficient for retinal surface maneuvers during vitrectomy procedures in both fluid-filled and air-filled rabbit eyes.

Analysis of health insurance cover for reproductive immunology.

The objective of this study was to document the current state of health insurance cover for reproductive immunology in the USA. A survey of couples who had given birth to a child within the last 2 years while they were being treated with lymphocyte immunotherapy at the Reproductive Medicine Clinic of the Finch University of Health Sciences/The Chicago Medical School (FUHS/CMS), North Chicago, IL, USA, produced 61 completed questionnaires from couples in 16 states, representing a response rate of 55%. The Reproductive Medicine Clinic at the FUHS/CMS is a major centre that treats couples with reproductive immunological problems. These couples had experienced repeated pregnancy losses that were diagnosed as immunological in nature. This prospective study documents the insurance reimbursement of couples receiving lymphocyte immunotherapy who subsequently became parents. The findings indicate that 80% of couples had insurance claims initially denied for reproductive immunotherapy-related services. The most common reason given for a denied claim was that the treatment was experimental. Couples took further action and almost all received cover. Cover for immunotherapy-related services averaged 65% of the cost. The percentage of expenses covered by the insurance was quite variable. The survey indicates that insurance cover is provided if patients are willing to take action.

Focal laser photophacoablation of normal and cataractous lenses in rabbits: preliminary report.

We evaluated the effect of focal laser photophacoablation on (1) a normal lens and (2) previously induced traumatic anterior cortical and posterior subcapsular cataracts in New Zealand albino rabbits. A Q-switched neodymium:YAG (Nd:YAG) laser was used to deliver 6 to 97 50 microns laser spots of 2.0 mJ to 8.3 mJ of energy/pulse to the normal lens. A neodymium:YLF (Nd:YLF) laser was used to deliver 60 microJ to 140 microJ of energy/pulse with a 0.3 mm to 0.6 mm cube at 1053 nm to ablate selective parts of the normal and cataractous lens. The Nd:YAG laser photoablation of the normal lens produced persistent circular opacities lasting up to one year. The Nd:YLF laser photoablation of the normal lens produced an empty space in the area of treatment (visualized as increased translucency), followed by a return to normal lens architecture at two days. The area remained translucent by slitlamp biomicroscopy for up to six months. Partial photoablation of the induced cataracts with an Nd:YLF laser produced partial clearing of the opacity without any evidence of surrounding lenticular damage. Focal Nd:YLF laser photoablation of lenticular opacities appears to be a relatively safe and noninvasive procedure that can be used for focal lens ablation without lens capsule disruption. The potential use of this treatment for selective removal of focal lens opacities requires further study.

Common modes of action of gamma-butyrolactones and pentylenetetrazol on the GABAA receptor-ionophore complex.

The effects of pentylenetetrazol and bicyclic gamma-butyrolactones of similar stereostructures were studied on the convulsant and benzodiazepine binding sites and chloride ionophore activity of the gamma-aminobutyric acid (GABAA) receptor-complex. Bicyclic gamma-butyrolactones displayed millimolar IC50 values and low stereoselectivities on [35S]t-butylbicyclophosphorothionate (TBPS) binding to the convulsant sites in synaptosomal membranes of rat forebrains. Ring saturation of bicyclic gamma-butyrolactones decreased their IC50 values by one order of magnitude. The IC50 values of saturated bicyclic gamma-butyrolactones and pentylenetetrazol were increased by GABA versus its antagonist R 5135 (3 alpha-hydroxy-16-imino-5 beta,17-aza-androstan-11-one). A bicyclic gamma-butyrolactone and pentylenetetrazol accelerated the dissociation of [35S]TBPS, displaced [3H]flumazenil binding in two phases and blocked the muscimol-elicited chloride currents in patch-clamped cortical neurones in culture in a similar manner. These similar effects on binding and ionophore function support their common modes of action on the GABAA receptor-ionophore complex.

Concanavalin A-induced posterior subcapsular cataract: a new model of cataractogenesis.

PURPOSE: To evaluate the effect of Concanavalin A (Con A) on cataract formation in New Zealand Albino rabbits. Uveitis is a chronic inflammatory condition of the eye involving the anterior and/or posterior segments. It may be acute or chronic and is associated with the development of posterior subscapular cataract over time. Con A is a nonspecific inflammatory agent and mitogen for T cells and some B cells. Used extensively in immunogenic studies Con A has been shown to induce uveitis after intravitreal injection in New Zealand Albino rabbits. METHODS: In two separate studies, Con A was injected intracamerally or intravitreally into one eye of 12 New Zealand Albino rabbits and an equal volume of balanced salt solution was injected into the opposite eye as a control. In a third study, the effect of topical steroids after intravitreal injection of Con A was evaluated. In all studies, anterior and posterior inflammation and the development of cataract was monitored by slit lamp biomicroscopy and photography. Cataract formation was also studied histopathologically. RESULTS: Initially, all eyes treated with Con A demonstrated moderate anterior chamber inflammation while eyes treated with balanced salt solution showed no inflammation. Three months after treatment, posterior subcapsular cataracts were present in all rabbit eyes treated with intravitreal Con A. In the third study, topical steroid treatment of Con A-induced inflammation significantly reduced anterior chamber inflammation but had no effect on vitreous humor and posterior subcapsular cataract formation. CONCLUSION: This experimental model was the first to demonstrate the development of posterior subcapsular cataracts after Con-A induced inflammation. The cataract was clinically and histologically similar to human posterior subscapular cataracts.

Lens regeneration in New Zealand albino rabbits after endocapsular cataract extraction.

PURPOSE: To evaluate the regenerative capacity of the adult rabbit lens after removal of a Concanavalin A-induced posterior subcapsular cataract. METHODS: Cataractogenesis was induced by intravitreal injection of Concanavalin A in adult New Zealand albino rabbits. At 7 mo postinjection, the cataracts were removed. Endocapsular lens extraction was performed by phacoemulsification and irrigation/aspiration with Balanced Salt Solution. RESULTS: Postoperatively, lens regeneration was first noted in the Balanced Salt Solution normal lens group at 3 weeks and the Concanavalin A cataract group at 6 weeks. By the 3-mo postoperative examination, lens regrowth, measured by digital image analysis, filled 74.5% of the capsule bag in the Balanced Salt Solution normal lens group and 46.6% in the Concanavalin A cataract group. In the latter group, less lens material was regenerated and at a slower rate than in eyes with extraction of a normal lens. CONCLUSION: This experimental model is the first to show that lens regeneration can occur after removal of cataracts secondary to inflammation.

Overexpression of the p53 protein and allele loss at 17p13 in ovarian carcinoma.

Mouse monoclonal antibodies PAb 240 and PAb 1801 which specifically immunoprecipitate p53 protein, were used to examine 27 fresh ovarian tumours (16 serous adenocarcinomas, six endometrioid carcinomas, one mucinous adenocarcinoma, one mucinous borderline tumour and three benign adenomas). Eleven out of 16 (69%) serous adenocarcinomas and one endometrioid tumour showed positive staining with one or both antibodies and none of the mucinous or benign tumours stained with either antibody. DNA from tumour and peripheral blood leukocytes was used to identify allelic deletions on chromosome 17p in tumours. 11/12 positively staining tumours showed less of heterozygosity (LOH) on 17p at the nearest informative locus to the p53 gene. In this series of ovarian tumours, LOH on 17p correlates closely with the aberrant expression of the p53 protein in a high proportion of advanced stage serous adenocarcinomas. This observation suggests that the p53 tumour suppressor gene is involved in the evolution of epithelial ovarian cancer (EOC) and may have prognostic significance.

Early loss of heterozygosity on 17q in ovarian cancer. The Abe Ovarian Cancer Genetics Group.

We have studied 146 ovarian tumours (94 carcinomas, 22 tumours of low malignant potential and 30 benign tumours) for evidence of allele loss on chromosome 17p and 17q sufficient to imply the proximity of a tumour-suppressor gene. We have examined two polymorphic loci (YNZ22.2 and BHP53) on 17p13 and one on chromosome 17q (17q23-qter). Loss of heterozygosity (LOH) was detected in 34/63 (54%) informative malignant tumours at YNZ22.2 and 22/47 (47%) at BHP53; on 17q, 45/64 (70%) had LOH. Allele loss was detected in a small number of benign and borderline tumours. There was a statistically significant difference between the patterns of allele loss in serous and endometrioid groups of tumours, and allele loss occurred with significantly greater frequency on 17q than on 17p. Comparison of all malignant tumours presenting with either localized (FIGO stage I/II) or widespread (FIGO stage III/IV) disease showed that, particularly on 17q, allele loss increases in the more advanced stages. The p53 tumour-suppressor gene is implicated in ovarian carcinogenesis, and our findings suggest that an important tumour-suppressor gene may be located in the region 17q23-qter. Loss of function in this gene may be responsible for the frequently observed rapid progression of serous-type adenocarcinomas to an advanced stage.

Practical approaches to obtaining hand radiographs and special techniques in hand radiology.

Radiography of the hands and joints involves a certain amount of decision making by the radiographer. Many disorders of the wrists are evaluated with radiography, and the positions and radiographic technique factors should be adjusted to reveal as much of the desired anatomy as possible. Immobilizing the patient can be of significant value in obtaining good quality radiographs. It is essential that the processed radiograph contains all pertinent anatomy and that all structures have been demonstrated anatomically as well as radiographically. A number of radiographic positions and procedures are useful in identifying specific structures. The knowledge of the radiographer is essential in obtaining these projections. Other special imaging methods, such as tomography, xeroradiography, and magnification radiography, permit the study of those structures too small to be noted on conventional radiographs. Fine-grain mammography film and rare earth screens in carbon fiber cassettes are essential tools in obtaining high-quality detailed images of the hand. A proper quality assurance program of scheduled and preventative maintenance will ensure an efficient radiology service with the least radiation dose to the patient.

Stereospecific synthesis and antiviral properties of different enantiomerically pure carbocyclic 2'-deoxyribonucleoside analogues derived from common chiral pools: (+)-(1R,5S)- and (-)-(1S,5R)-2-oxabicyclo[3.3.0]oct-6-en-3-one.

Enantiomerically pure (+)- and (-)-carbocyclic thymidine, (-)-carbocyclic 3'-epi-thymidine, (+)-carbocyclic 3'-deoxy-3'-azidothymidine, (+)-carbocyclic 2,3'-O-anhydrothymidine, (+)-carbocyclic 3'-O,6'-methylenethymidine, and (+)-(6'S)-carbocyclic 6'-methylthymidine were synthesized in a stereospecific manner from common chiral pools of (+)-(1R,5S)- and (-)-(1S,5R)-2-oxabicyclo[3.3.0]oct-6-en-3-one and evaluated for antiviral activity. (+)-Carbathymidine and, to a lesser extent, (+)-carbocyclic 2'-deoxyadenosine proved to be effective against HSV-1 [minimum inhibitory concentration (MIC): 0.2 and 2 micrograms/mL, respectively] and HSV-2 (MIC: 2 and 20 micrograms/mL, respectively), but virtually inactive against TK- HSV-1 (MIC: 40 and 100 micrograms/mL, respectively). (+)-Carbathymidine was also active against vaccinia virus (2 micrograms/mL). None of the compounds had a specific effect on the replication of HIV or other RNA viruses.

[Upper Austria model study to assess the prevalence and incidence of congenital abnormalities].

Among the 15.998 live births recorded in Upper Austria in the year 1985, a representative malformation rate of 1.79, respectively a representative incidence of 17.94 in 1000 live births is reported. The incidences of characteristic malformations and of single malformations combined in malformation groups are determined. An instrument of investigation, especially developed for and successfully used in this examination in form of an illustrated questionnaire is introduced.
