Catalytic role of the amino-terminal proline in 4-oxalocrotonate tautomerase: affinity labeling and heteronuclear NMR studies.

4-Oxalocrotonate tautomerase (EC 5.3.2-; 4-OT), a hexamer consisting of 62 residues per subunit, catalyzes the isomerization of unsaturated alpha-keto acids, converting unconjugated ketones to the conjugated isomers via a dienolic intermediate. The recently solved crystal structure of an isozyme of 4-OT suggests that the amino-terminal proline is the catalytic base [Subramanya, H. S., Roper, D. I., Dauter, Z., Dodson, E. J., Davies, G. J., Wilson, K. S., & Wigley, D. B. (1996) Biochemistry 35, 792-802]. In support of this proposed role, we have found that the active-site-directed irreversible inhibitor 3-bromopyruvate (3-BP) blocks the amino terminus of 4-OT to Edman degradation and results in the disappearance of the 15N resonance of Pro-1 (delta = 49.2 ppm at pH 6.40 and 42 degrees C) in the 15N NMR spectrum of uniformly 15N-labeled 4-OT. Furthermore, covalent bonding between a 15N resonance of 4-OT and the methylene carbon of the reduced, 3-(13)C-labeled lactyl adduct derived from [3-(13)C]-bromopyruvate was then directly demonstrated using two heteronuclear NMR methods, an 1H-(13)C HSQC experiment and a novel inverse correlation experiment which we call H(C)N. The chemical shift of the modified 15N resonance (delta = 86.5 ppm) is consistent with that of an alkylated and cationic, amino-terminal proline. Affinity labeling with 2-(14)C-labeled bromopyruvate indicates that the ultimate stoichiometry of modification is I equiv of 3-BP per 4-OT monomer. However, an analysis of the residual enzyme activity after differing extents of fractional modification with 3-BP indicates that modification of three active sites per hexamer abolishes essentially all activity of the hexamer. Thus, 4-OT exhibits half-of-the-sites stoichiometry with 3-BP. Finally, the pH dependence of kinact/KI for affinity labeling by 3-BP yields a pKa value of 6.7 +/- 0.3, in reasonable agreement with the pKa values found for kcat/KM for the non-sticky substrate 2-hydroxy-2,4-pentadienoate and by direct NMR titration of Pro-1 [Stivers, J. T., Abeygunawardana, C., Mildvan, A. S., Hajipour, G., & Whitman, C. P. (1996) Biochemistry 35, 814-823]. These results strongly implicate the amino-terminal proline as the general-base catalyst on 4-OT.

Liver endogenous antioxidant defenses in mice fed AIN-76A diet and infected with murine AIDS.

The effects of murine AIDS infection on endogenous antioxidant defenses in mice fed the AIN-76A liquid diet were investigated. C57BL/6 female mice were divided into 2 groups: one group was injected interperitoneally with LP-BM5 murine retrovirus (MAIDS) stock, and the other group served as the non-infected control. Two weeks after the infection, the mice were killed and livers were excised for biochemical analysis of the antioxidant defenses. Liver reduced glutathione (GSH) levels and activities of both cytosolic superoxide dismutase (SOD) and mitochondrial SOD were significantly depressed by MAIDS infection. Activities of glutathione reductase (GR) selenium (Se)-dependent glutathione peroxidase (GPx), catalase and glutathione-S-transferase (GST) toward 1-chloro-2,4-dinitrobenzene (CDNB) were not affected by MAIDS infection. A previous study by this laboratory using the Lieber-DeCarli (L-D) all purpose liquid diet caused a decline in total SOD activity and GPx activity, but not GSH levels. The results suggest that MAIDS infection depresses liver antioxidant defenses; however, MAIDS infection of mice fed the AID-76A liquid diet depresses different liver antioxidant defense parameters when compared to those of the mice fed the L-D all purpose liquid diet.

[Study on the collecting method of human umbilical cord blood].

The methods of cord blood collection was investigated in this study. The purpose of this study was to evaluate human umbilical cord blood as a alternative to bone marrow in the provision of transplantable progenitor cells for hematopoietic reconstitution. The results showed that using modified technique with a sterile and closed system, the volume of cord and placental blood would all exceed 120 ml. The average amount of blood collected was 132.2 +/- 12.13 ml and the highest was 158 ml. Cord blood culture for bacteria and fungi were negative. Blood clots were not present. And in the examination of each single cord blood, the contents of nucleated cells (NS) and mono-nucleated (MNS) were, respectively, 18.9 X 10 +/- 1.7 X 10, 8.2 X 10 +/- 0.8 X 10. The result suggested that the collection of placental blood with the modified technique in a sterile and closed system is a simple, safe and efficient procedure. The average amount of blood collected and hematopoietic cells would satisfy the needs of cord blood transplantation. Immediate cut of the umbilical cord followed by a prompt puncturing is critical for the volume and quality of the sample.

Reduction-fixation spinal system in spondylolisthesis.

Posterior transpedicular instrumentation with short segmental fixation represents a current trend in surgery for unstable spinal problems, such as spondylolisthesis. A reduction-fixation spinal system with 5.75- to 6.25-mm diameter pedicle screws of variable angles and an 8-mm diameter rod can achieve a rather rigid, short-segment fixation and satisfactory reduction of spondylolisthetic deformities. In clinical applications, 84% of 85 patients with spinal instability had good-to-excellent results for back pain, 88% for sciatica, and 86% for claudication; 91% achieved solid posterolateral fusion from the roentgenographic studies. This reduction-fixation system achieved a reduction of 71% +/- 5.3% of slipping width in this population. Posterior lumbar interbody fusion is mandatory for effective and lasting reduction. Complications from reduction fixation including pedicle-screw loosening (12%), pedicle screw breakage (7%), adjacent instability (7%), and nerve root injury (2%).

[Metastatic spine disease: surgical treatment by anterior corpectomy and Ziekel's VDS fixation].

The spine is the common site of skeletal metastases. The goal of surgical treatment of metastatic spine disease is to improve the quality of remaining life, by relief of pain, the preservation or restoration of neurologic function, and reconstruction of the spinal stability. From 1984 through June 1993, 40 consecutive patients underwent surgical treatment by anterior corpectomy, and bone cement with VDS fixation for corporal metastatic diseases. There were 28 men and 12 women. The average age was 53.2 years, range from 21 to 76 years. Twenty-four patients had metastasis to the thoracic spine, 12 to the lumbar spine, and 4 had both thoracic and lumbar metastases. The primary malignancies were lung cancers in 10 cases, colorectal carcinomata in 6, hepatomas in 5, thyroid cancers in 4, cancer of stomach, renal, breast, nasopharynx, long bone, skin, cervical in 1 case respectively. A primary carcinoma was never identified in 8 patients. In the present series, 4 patients died within one month, 36 patients were allowed for follow-up study. Thirty-two patients (89%) attained pain relief. Thirty patients presented with neurologic compromise. After operation, neurologic improvement was noted in 21 (70%). No patient's neurologic function deteriorated secondary to surgical intervention. We conclude that anterior corpectomy to decompress neural encroachment with VDS and cement fixation to stabilize the collapsed spine is an adjunctive treatment to reduce pain and restore the neurologic function in these highly selected patients.

Direct sequence data from heterogeneous creatine kinase (43 kDa) by high-resolution tandem mass spectrometry.

Isoelectric focusing separation of recombinant rabbit muscle creatine kinase (CK) and its 282Cys-->282Ser mutant shows the presence of three and two isoforms, respectively, that exhibit equivalent enzymatic activity. Electrospray ionization coupled with Fourier-transform mass spectrometry (10(5) resolving power) of both CKs indicates that their major components are within +/- 2 Da of the M(r) value predicted from the cDNA sequences of these mixtures. Dissociation of (M + nH)n+ gives no evidence that the components of either CK are isomers; the masses of the 51 fragment ions correlate completely (+/- 1 Da) with the values predicted from the cDNA sequence and confirm the identities of 21 of the 380 amino acids and the 282Cys-->282Ser replacement in the mutant. The results are consistent with one or two steps of post-translational amidation/deamidation (NH2-->OH, 16 Da-->17 Da), each of which would produce only a 1 Da difference in M(r), with the fragment masses indicating that at least one modification occurs between residues 212 and 282.

Sequence verification of human creatine kinase (43 kDa) isozymes by high-resolution tandem mass spectrometry.

Amino acid sequencing by recombinant DNA technology, although dramatically useful, is subject to base reading errors, is indirect, and is insensitive to posttranslational processing. Mass spectrometry techniques can provide molecular weight data from even relatively large proteins for such cDNA sequences and can serve as a check of an enzyme's purity and sequence integrity. Multiply-charged ions from electrospray ionization can be dissociated to yield structural information by tandem mass spectrometry, providing a second method for gaining additional confidence in primary sequence confirmation. Here, accurate (+/- 1 Da) molecular weight and molecular ion dissociation information for human muscle and brain creatine kinases has been obtained by electrospray ionization coupled with Fourier-transform mass spectrometry to help distinguish which of several published amino acid sequences for both enzymes are correct. The results herein are consistent with one published sequence for each isozyme, and the heterogeneity indicated by isoelectric focusing due to 1-Da deamidation changes. This approach appears generally useful for detailed sequence verification of recombinant proteins.

Case-control study of passive smoking and the risk of small-for-gestational-age at term.

There is concern about the effects of passive smoking during pregnancy on fetal growth. The authors conducted a case-control study of the association of maternal exposure to passive smoking during pregnancy and the risk of term small-for-gestational-age (SGA) infants in a population of white women who did not smoke during pregnancy and had only a small percentage of users of illegal drugs and alcohol. A total of 111 cases, defined as singleton term (> or = 37 weeks) infants with birth weights < or = 10th percentile for gestational age were compared with 124 term, non-SGA controls. All were identified from Contra Costa, California birth certificates for January 1-September 30, 1991. Subjects were interviewed face-to-face to collect information on exposure to passive smoking during pregnancy. The estimated relative risk for term SGA in association with passive smoke exposure during pregnancy was not increased (> or = 30 hours: odds ratio (OR) = 0.41, 95% confidence interval (Cl) 0.12-1.29) and the risk of term SGA did not increase with increasing hours of exposure to passive smoking. Controlling for parity, weight gain, prepregnancy weight, maternal age, prenatal care, education, income, alcohol consumption, and work during pregnancy in multivariate analysis did not change the findings (> or = 30 hours: OR = 0.47, 95% Cl 0.13-1.69). In this study, maternal exposure to passive smoking during pregnancy was not associated with an increased risk of term SGA. Although difficulties in precisely assessing exposure limit these findings, they should provide some reassurance to women who cannot avoid passive exposure to cigarette smoking during pregnancy.

Combination chemotherapy with carmustine and cisplatin before, during, and after radiotherapy for adult malignant gliomas.

Twenty-two patients, aged 16 to 67, who had malignant gliomas after surgical resection were treated with carmustine and cisplatin intravenous infusion before, during, and after radiotherapy. All patients had subtotal or total resection, or biopsy as the initial procedure. Twenty-one patients who had at least 2 cycles of chemotherapy and finished the whole course of radiotherapy were considered to be evaluable for responses. Among them, 5 had glioblastoma multiforme, 16 had anaplastic astrocytoma. The median time to tumor progression was 35 weeks (range 12-130 weeks) and median survival time was 66 weeks (range 10-156 weeks). Early progression occurred more frequently in patients with biopsy only and subtotal resection, and in patients with glioblastoma than in those with anaplastic astrocytoma. This combined modality treatment program was associated with reversible hematologic toxicity which was severe in 2 patients, and with ototoxicity in 1 patient, nephrotoxicity in 2 patients. Combination of carmustine and cisplatin with cranial irradiation for malignant gliomas is moderately toxic and appears to offer no obvious survival advantage compared with radiation therapy plus BCNU alone.

Evaluation and prediction of the facial appearance after surgical correction of mandibular hyperplasia.

The response of the entire facial soft tissue to correction of mandibular hyperplasia was evaluated in 25 patients by moiré topography. Postoperative changes were quantitatively analyzed by a computer-aided superimposition technique. Posterior displacements occurred in the lower third of the face. A marked retraction was observed around the chin, but the changes gradually decreased in the coronal and posterior directions with distance from the chin. The upper lip was slightly reduced; however, no direct effect was found in the cheeks. The results of this study constitute a database that, if used in conjunction with an interactive imaging system, would allow surgeons to predict accurately the postoperative appearance of the patient.

Potent antiplatelet, anti-inflammatory and antiallergic isoflavanquinones from the roots of Abrus precatorius.

Five isoflavanquinones have been isolated from the roots of Abrus precatorius L. (Leguminosae). Three of them are new and designated as abruquinones D, E, and F. The pharmacological activities of the isoflavanquinones have been evaluated. The results indicated that abruquinones A, B, and D exhibited remarkable inhibitory effects on the platelet aggregation. The IC50 of abruquinones A and B for the inhibition of the platelet aggregation induced by arachidonic acid (AA) and collagen were less than 5 micrograms/ml, and of abruquinone D, was less than 10 micrograms/ml for that induced by AA. On the other hand, abruquinones A, B, D, and F showed strong anti-inflammatory and antiallergic effects. The IC50 of abruquinones A, B, D, and F for the inhibition of superoxide formation were less than 0.3 micrograms/ml, for the inhibition of the release of both beta-glucuronidase and lysozyme from rat neutrophils and the release of both beta-glucuronidase and histamine from mast cells were less than 1 microgram/ml.

Uterine perforation during elective first trimester abortions: a 13-year review.

OBJECTIVE: The purpose of this study was to determine the incidence, presentation, management and outcome of uterine perforation during elective first trimester abortions. METHODS: We conducted a retrospective study of 40 patients, including 2 transferred patients, who sustained uterine perforation during elective abortions from January 1980 to December 1992. RESULTS: The incidence of uterine perforation was 0.8 per 1,000 procedures (0.08%). There were 8 (20%) nulliparae and 3 (7.5%) grand multigravidae. 82.5% of the cases occurred when the abortion was performed by medical officers or junior registrars under training. The commonest perforating instrument was the suction cannula (25%) followed by the uterine sound (22.5%) and the dilator (20%). Three (7.5%) cases were treated conservatively, 33 (82.5%) cases underwent emergency operation, 2 (5%) cases were discovered during subsequent sterilisation, and 2 (5%) cases suffered undiagnosed perforation and were re-admitted for emergency surgery. Morbidity included post operative fever (12.5%), bowel injury (7.5%), retained conceptus (5%) and wound breakdown (2.5%). There was no mortality. CONCLUSION: A careful assessment of the uterine size and position, vigilance in the use of uterine sound and dilators, greater care in the use of suction cannula, and experience in vacuum aspiration will decrease the incidence of uterine perforation during elective abortions. A high degree of suspicion, early diagnosis and treatment will prevent the potential complications that may arise from uterine perforation.

Back pain after thoracolumbar fracture treated with long instrumentation and short fusion.

The purposes of this retrospective study are to evaluate the degeneration of facets in unfused segments immobilized by Harrington rods and to see how often back pain developed after treatment with this method. Sixty patients with thoracolumbar fracture were treated with the Harrington rod long-fuse short technique. Harrington rods are removed an average of 14 months after initial surgery. Every case showed solid fusion during implant removal exploration. Thereafter, all cases were followed for a minimum of 5 years. Roentgenographic studies and a bone scan were performed to evaluate the degeneration of facet joints. Back pain complaint was found in 11 patients (18.3%). The degeneration of unfused facets was visualized in six patients by roentgenograms. Increased activity in the instrumented spine by bone scan examination also was noted in six patients: three cases were at unfused area, one at a fused area, and two at fused and unfused areas. In conclusion, degeneration in the immobilized and unfused segments after rod long and fused short was rarely seen clinically after long-term follow-up. Back pain was experienced in only 18.3% of the patients and seldom required medication.

A ten-year review of uterine rupture in modern obstetric practice.

The aim of this paper is to determine the antecedent factors, clinical presentation, complications and management of uterine rupture in the context of modern obstetric practice in Singapore. We conducted a retrospective study of 26 proven cases of uterine rupture in Kandang Kerbau Hospital, Singapore between January 1983 to December 1992. These cases were analysed with regards to their past history, clinical presentation, complications, management and outcome. The incidence of uterine rupture was 1 in 6331 deliveries. The ratio of cases with scarred uteri against those with unscarred uteri was 3:1. The commonest antecedent factor was previous lower segment caesarean section for the scarred group and cephalo-pelvic disproportion in the unscarred group. Overall, 46.2% of the patients had augmentation with oxytocin. The major clinical presentations were abnormal cardiotocogram (25%) and blood-stained amniotic fluid (20%) in the scarred group, and postpartum haemorrhage (50%) and shock (33%) in the unscarred group. Repair of the uterus with or without tubal ligation was performed in 95% of the patients with scarred uteri, whereas 67% of the patients with unscarred uteri underwent total abdominal hysterectomy with or without salpingo-oophorectomy. There was 1 (3.8%) maternal death. Maternal morbidity included bladder injuries, broad ligament haematoma, disseminated intravascular coagulation and gastrointestinal bleeding. The overall incidence of fetal loss was 7.4%. When compared to a previous study on uterine rupture in the same hospital, there was an improvement in obstetric performance.

The ompA 5' untranslated region impedes a major pathway for mRNA degradation in Escherichia coli.

The unusual longevity of the Escherichia coli ompA transcript is determined by its 5' untranslated region (UTR), which functions in vivo as an mRNA stabilizer. Here we show that this 5' UTR can prolong the lifetime in E. coli of a variety of heterologous mRNAs to which it is joined, either as a gene fusion or as an operon fusion. Statistical extrapolation suggests that it is quite likely that most E. coli mRNAs could be stabilized in this manner. We conclude that the ompA 5' UTR impedes a major pathway for mRNA degradation in E. coli and that stabilization by fusion to this UTR does not require translational readthrough of the heterologous mRNA segment by ribosomes that initiate translation at the ompA ribosome-binding site. Additional experiments indicate that the E. coli ribonuclease whose action is slowed by the ompA 5' UTR is not RNase III.

Extramedullary plasmacytoma of the head and neck.

Extramedullary plasmacytoma (EMP) is an uncommon malignant plasma cell neoplasm which usually presents as soft tissue mass in the head and neck. We retrospectively reviewed 7 cases with EMP of the head and neck between 1978 and 1992. All patients were male. Their ages ranged from 27 to 66 years. M-protein was identified in 5 patients (IgG-kappa in 3, IgG-lambda in 1 and Lambda light chain in 1). All but one were treated with local irradiation. Three patients received chemotherapy. All patients have been alive for 37+ to 116+ months since the diagnosis of EMP. One patient had a cervical node recurrence 9 years after local irradiation and the other one who refused local irradiation but received chemotherapy developed multiple myeloma 5 years later. Based on our experience and the review of the literature, it is recommended that irradiation to the primary sites and the involved cervical nodes is the treatment of choice. Additional chemotherapy may be considered in patients with disease progression, recurrence, or dissemination. The M-protein is a useful parameter to assess tumor control or disease progression.
