[Myocardial revascularization surgery via a left minithoracotomy].

PURPOSE: There is, today, a global tendency towards a surgical approach privileging very small incisions, the so-called minimally invasive intervention, which results in a less aggressive action. The introduction of this new technique makes it possible to dissect the left internal thoracic artery (LITA) and to perform in the anastomosis with the anterior interventricular artery (AIA) through a left minithoracotomy. METHODS: From May of 1996 to october of 1997, 11 patients with ischemic heart disease and a single proximal lesion of the AIA were submitted to a myocardial revascularization (MR). The surgical approach consisted of a left anterolateral thoracotomy through the 4th left intercostal space, of approximately 10 cm, and in the last 6 cases, resection of part of the cartilage of the 4th and 5th ribs, dissection of the LITA, as well as opening and repair of the pericardium adjacent to the AIA was done. All patients received a single bypass to the AIA with a graft of the LITA, without extracorporeal circulation. RESULTS: The patients' age varied between 46 and 76 years (mean = 58.55). Ten patients (90.90%) were males and 1 (9.09%) was a female. Hospital stay ranged from 4 to 8 days, with the average of 5.2 days. None of the patients presented any electrocardiographic change in the immediate post-operatory period. During the control period one patient developed a clot in the distal LITA, with important compromise of the flow. In another patient the stenosis was at the level of the anastomosis. Both were successfully submitted to angioplasty. There were no deaths in the groups studied. CONCLUSION: The absence of deaths suggests to us that MR surgery carried out with this minimally invasive technique, in selected groups and is an excellent alternative to the revascularization of the AIA.

[Body, strength, and labor: reflexions in the light of gender relations].

The present paper intends to contribute on a reflection and discussion of some issues related to female's body and the strength present in laborwork, understood as the moment in which through the support of another person, a woman "gives birth". It also discusses the perspective of male dominance having as reference the symbolic violence from Bordieu, proposing that the humanizing of labor demands a thorough discussion on gender dominance.

Multidisciplinary experience in the selection of patients for tubal sterilization.

Results of the use of a special protocol for evaluation of patients requiring tubal ligation is presented after applied by a multidisciplinary group. The authors conclude that the use of defined parameters of age, parity, marital union duration, number of children alive and the presence of maternal clinical pathology are useful to identify patients with smaller chances of regret after surgery.

Angyomatous vocal polypus--a complete spontaneous regression.

The authors describe a male patient who had malignant lymphoma seven years ago which remitted with chemotherapy. Two years ago he developed dysphonia. An unilateral, pediculate smooth red lesion on the right vocal fold was later discovered. Even without benefit of medicamentosus treatment, the patient refused surgery. In a reevaluation using rigid telescopy of the larynx two years later, the lesion had disappeared, completely and spontaneously. As there are no existing publications on this topic, this case report is an alert that surgery should be recommended with extrema caution in this type of vocal disease.

Nasal septal pediculate carcinoma in situ: differential diagnosis.

Pediculated lesions of the nasal cavities are relatively common in daily practice, and include inflammatory polyps, benign tumors (papillomas being the most common), malignant tumors, and specific processes, such as polypoid rhinosporidiosis. The authors describe a female patient with a warty, pediculated, and asymptomatic lesion in the nasal septal mucosa. The anatomo-pathological exam showed this to be a "carcinoma in situ". The few bibliographic citations report only an association between the tumor and contact with wood dust, such as oak, ebony and beech. The patient was not exposed to these elements. It is important to emphasize the routine performance of a complete otolaryngological exam for patients seeking out specialists, in order to detect potentially malignant lesions whose early removal would permit a complete cure.
