From food basket to food security. The food factor in nutritional surveillance.

One important indicator of nutritional surveillance is the one devoted to monitor food security. The experience toward the development of one of such indicators is presented. This includes the development of a food basket, defined as the group of foods that meet the characteristics such as is now consumed by important population segments of the community; it contributes a substantial portion of the calories and proteins purchased, and is responsible for an important proportion of the food budget. The concept implies a dynamic food basket, the quantities of which are calculated in a way that simulates the behavior of the consumer and the best nutrition knowledge. For this purpose we use linear program techniques. A measure of the risk of being unable to buy the foods needed for a family is presented, and is used as a proxy for food security risk. In the appendix, the mathematical expressions of the model used for a linear program is also presented.

Chondroblastoma of the rib. A case report and review of the literature.

Chondroblastoma, a rare bone tumor usually found in the epiphyseal region of long bones, appeared in the rib of a 49-year-old man. Review of the literature reveals that the most common locations for this tumor are the knee and proximal humerus. The most common age group is the second decade. Eighteen cases of chondroblastoma of the rib have been reported in the literature; the patients were older than typical patients with epiphyseal chondroblastoma and had an excellent prognosis after resection of the tumor.

Interconversion of estrone and estradiol-17 beta in lung slices of the adult human.

The metabolism of tritium-labeled estrone and estradiol-17 beta in slices of lung tissue obtained from an adult human was studied; estrone was identified as the only metabolite of estradiol-17 beta and estradiol-17 beta as the exclusive product of estrone metabolism. Product formation remained linear as a function of time of incubation up to 3 h and of wet lung tissue mass up to 300 mg/ml. At equimolar substrate concentrations, the rates of estrone formation were at least 2-fold greater than those of estradiol-17 beta. The apparent KM of 17 beta-hydroxysteroid oxidoreductase for estrone was 11 microM and that for estradiol-17 beta was 10 microM. These results are suggestive that the human lung enzyme binds estrone and estradiol-17 beta with similar affinities; however, the oxidative pathway is favored as indicated by the greater Vmax attained in the formation of estrone. It is possible that, in vivo, the human lung constitutes a site for estradiol-17 beta inactivation to estrone as well as a site for the conversion of estrone to estradiol-17 beta. This last process may become particularly important in instances in which the ovaries have ceased to function and secrete estradiol-17 beta, e.g. the postmenopausal women.

Influence of the salt moiety on the effectiveness of corticosteroid therapy in cardiogenic shock.

Using closed chest coronary artery microsphere embolization, myocardial infarction and subsequent shock were produced in healthy adult mongrel dogs. Following infarction animals were distributed among the following groups: (1) control; (2) methylprednisolone sodium succinate (MPSS); (3) methylprednisolone sodium phosphate (MPSP); (4) sodium phosphate (SP); and (5) sodium succinate (SS). Drug doses of equivalent anti-inflammatory activity were administered i.v. 15 min after infarction. Compared to control animals, only dogs treated with MPSS exhibited significant improvements in hemodynamic parameters and permanent survival. Survival in MPSS dogs was significantly (P = 0.02) better than that of either group treated with SP or SS and substantially (P = 0.02) better than that of either group treated with SP or SS and substantially (P = 0.065) better than the survival rate of dogs receiving MPSP. It appears that succinate is permissive, while phosphate is restrictive, with respect to efficacy of methylprednisolone in experimental cardiogenic shock. Possible explanations for these observations are discussed.

Massive impalement wound of the chest. A case report.

A 29-year-old man sustained an impalement wound of the right side of the chest with a 6 foot long, 4 by 4 inch wooden post during a high-speed automobile accident. The post entered the right side of the chest anteriorly and exited posteriorly. The great vessels of the chest were spared, but the right lung was lacerated and contused. Removal of the post, treatment of pulmonary parenchymal damage, and reconstruction of the two large chest wall defects presented problems in management. Rapid transport of the patient to the hospital, effective emergency treatment, surgical repair of the chest injury, and appropriate supportive measures contributed to his survival.
