5 years' experience of 5-S-cysteinyldopa in melanoma diagnosis.

Determinations of the urinary excretion of 5-S-cysteinyldopa were performed in 571 patients previously treated by surgery for melanoma or melanoma metastasis. 90% of the 161 patients with metastases showed values exceeding 0.15 mg/24 h, and 9% of the 410 patients without metastases had such values. The increase in 5-S-cysteinyldopa excretion was generally more pronounced in men with metastases than in women, 98% of the men and 77% of the women with metastases showing values exceeding 0.15 mg/24 h. High levels of 5-S-cysteinyldopa are of grave prognostic significan4% died within one month, and only 3% survived for more than a year. In Sweden, determination of 5-S-cysteinyldopa in patients operated on for melanoma gives maximum information in the winter (October--March), when sun exposure does not influence the excretion levels.

Measurements of total and regional tumor blood flow and organ blood flow using 99Tcm labelled microspheres. An experimental study in rats.

Cardiac output, regional tissue and arterial tumor blood flow were studied in rats, with 99Tcm labelled microspheres using a reference sample simultaneously drawn as the spheres were injected. Both cardiac output and regional blood flow can be estimated in absolute values (ml X min-1 X g-1). Cardiac output and normal tissue blood flow were not affected by tumor growth in the liver or subcutaneously. In small tumors total blood flow was inversely proportional to tumor size, a finding similar in all 3 tumors studied. A relation found in liver as well as in subcutaneously implanted tumors. Blood flow in subcutaneous tumors was greater than muscle or skin blood flow. BP sarcoma had higher blood flow than the hepatoma and the adenocarcinoma. In big tumors the peripheral blood flow was larger than that in the centre of the tumor.

Methodological aspects of an agar plate technique for determination of biologically active 5-fluorouracil in blood, urine and bile.

A microbiological agar plate technique for estimation of 5-fluorouracil concentrations in blood, urine and bile from man, dog and pig was evaluated. Different bacterial test strains, media modifications and techniques for inoculation were studied. The strain Streptococcus faecalis ATCC 8043, recommended previously by Clarkson et al., was found to be the most suitable. The influence of prediffusion, dilution, antibiotics and chemotherapeutic agents and their antagonists, as well as the effect of storage of samples containing 5-fluorouracil were examined. A detailed methodological description is presented. The method seems to be sufficiently sensitive and practical for routine determination of cytotoxic compounds from 5-fluorouracil in serum, plasma and urine.

Liver metastases found by follow-up of patients operated on for colorectal cancer.

One hundred and twenty-six patients earlier operated on for colorectal cancer were followed-up once yearly with serum screening tests. The activities of alkaline phosphatase (AP) and gammaglutamyltranspeptidase (GT) were recorded. 58 patients had positive tests. The majority of the patients with liver metastases (20/21) was possible to encircle with these simple serum tests. 38 of the 58 "screening positive patients" were further investigated with celiac angiography and/or liver scintigraphy and liver metastases were very suspect in 29 of these patients. 18 of them were laparotomized and the suspicion was verified in 8.7 of these patients could be subjected to surgery against their liver tumours and 2 of them have then survived more than 2 years. The authors suggest a follow-up system with shorter interval between the examinations.

Selective celiac angiography and surgical exploration for suspected liver cancer. A comparative study of 196 patients.

In 196 patients subjected to celiac angiography and exploratory laparotomy for suspected liver tumor, 81% of the tumors were diagnosed by angiography, 26% of patients had false-negative angiograms, and 16% had false-positive angiograms. Diangosis on laparotomy proved correct except in one patient. Of 22 patients with liver tumors not diagnosed on angiography, 10 could be specifically treated and 3 of them have survived more than one year. This gain is discussed in relation to the rather high number of unnecessary laparotomies.

Results of regional portal infusion of 5-fluorouracil in patients with primary and secondary liver cancer.

The survival of 47 patients with liver malignancies treated with continuous portal infusion of fluorouracil (5-FU) has been studied. 18 of them had been treated initially by hepatic artery ligation. Total mean survival was 9.4 months. Patients treated with hepatic artery ligation + 5-FU lived longer (mean 10.8 months) than those treated with 5-FU alone (7.4 months). The survival was longer than could be expected for patients treated for primary liver cancer or for liver metastases from colo-rectal cancer, when compared with the "untreated" group. It is concluded that continuous portal infusion of 5-FU appears to prolong survival in some groups of patients with malignant liver tumours. However, the influence of "unspecific, general" therapy is difficult to evaluate.

Results of liver dearterialization combined with regional infusion of 5-fluorouracil for liver cancer.

40 patients with primary or metastatic liver malignancies have been treated with hepatic dearterialization, 19 of them in combination with regional infusion with 5-fluoroacil. The survival was longer than expected from untreated materials. This was especially shown to be valid for patients with primary liver cancer and liver metastases from cancer coli-recti, in which the prolongation of survival was associated with an acceptable quality of life. The long term results were correlated with the "aggressiveness" in therapeutic approach. Only studies of prospective randomized series can settle whether the results depend on the specific measures against the liver tumours or on the general treatment.

Liver resection for cancer.

Liver resection was performed for primary and secondary cancer in 46 patients. Left lobe resection was performed in 11 patients, right lobe resection in 22 and extended right lobe resection in 13. There was no post-operative mortality after left lobe resection, 9 after right lobe and 5 after extended right lobe resection. Ten patients have survived two years after the liver resection. This surgical procedure seems to be of benefit especially for patients with primary liver cancer and with metastases from cancer coli-recti.
