Asperger's syndrome and ligamentous laxity.

Two autistic girls and one autistic man in whom Asperger's syndrome coexists with lifelong ligamentous laxity and muscular incoordination are described. Two had cranial circumferences at or above the 90th percentile as children, two have complex partial epilepsy, one has a colloid cyst of the third ventricle, and one had evidence of Sotos syndrome. Echocardiography was performed in the two girls and both had evidence of increased aortic compliance. It is suggested that all three suffer from a Marfan-like disorder of connective tissue and, more speculatively, that this has led to anomalous development of midline brain structures with consequent social handicaps characteristic of Asperger's syndrome.

The Seventh Jack Tizard Memorial Lecture. Aspects of adoption.

Adoption has a long history and there has been a change in traditional assumptions and practice. It is now seen as a form of child care, one of a spectrum of resources for children in need. Adopted children generally show a good outcome but are over-represented in clinical populations. This trend is likely to continue as older children with histories of severe deprivation, neglect, abuse and institutionalization are placed. New light has been thrown on "telling" in adoption by developmental studies. Adoption disruption increases with age and other factors at placement, requiring further research into its origins and management.

Child psychiatry in Britain--the last 30 years.

The development of child and adolescent psychiatry over the last thirty years is described with emphasis on the ideas and concepts which have become a characteristic feature of clinical practice and research. Aspects of training, innovations in practice and the relationship with paediatrics are discussed and the growing participation in international bodies concerned with mental health is outlined. The specialty is now well established and is attracting physicians of high calibre to its ranks. Multidisciplinary collaboration is still a strong feature but has taken a different form in the light of new developments in clinical and developmental child psychology and psychiatric social work.

Management of children with non-organic (hysterical) disorders of motor function.

Five children aged between 10 and 13 years are discribed whose disorders of motor function were found to have a non-organic (hysterical) cause. In most cases extensive investigations had been made to establish an organic cause before the hysterical basis was diagnosed. The three essentials of management of such cases are to limit further investigations to the essential minimum, to institute quickly a programme of graduated physical rehabilitation, and to establish and treat the underlying psychological problems.
