Minimum energy for cardiac pacing.

Conditions for minimum energy pacing of the heart have been investigated. A total of 99 patients undergoing permanent cardiac pacing with one of three different leads were studied. Measurements of threshold voltage and current flow were made at 0.25, 0.5, 0.75, and 1.0 ms pulse durations. All measurements were made directly from the lead at implantation. There were no significant differences in threshold energy between the three leads used, and therefore all 99 leads were pooled for further analysis. There were significant differences (P less than 0.05) between the pacing energies required at the four pulse durations; the mean threshold energy at 0.25 ms was 156 nJ, at 0.5 ms 115 nJ, at 0.75 ms 110 nJ, and at 1.0 ms 102 nJ. Significant energy savings may be achieved when programming pacemakers to the optimum output for each patient by reducing pacemaker voltage - current output in preference to reducing pacing pulse duration.

Preliminary X-ray data for a D-amino acid amino-transferase from a novel thermophilic Bacillus.

Crystals of the D-amino acid aminotransferase (D-ATA) from a novel thermophilic Bacillus species (Escherichia coli pICT113 cloned gene product) have been examined by X-ray analysis. The crystals grow as hexagonal prisms, with the symmetry of space group P61 or P65 (indistinguishable crystallographically). The cell dimensions are a = b = 135 A, c = 53 A, alpha = beta = 90 degrees, and gamma = 120 degrees. The unit cell has a volume of 850,000 A3 with six asymmetric units per unit cell. There is one dimer of molecular weight 62,000 per asymmetric unit, and the crystals diffract to 2.7 A.

Study of neuromotor reaction times under the influence of thyrotropin-releasing hormone.

Neuromotor reaction times (simple, choice, and decision) were measured when Thyrotropin-releasing hormone (TRH) was administered intravenously to nine healthy men in a double-blind study. Measurements were made of simple reaction time, choice reaction time, and decision time for each subject at various intervals over a 54-hr. period. Given the observed inherent interaction of the drug with the long time used (54 hr.), most analyses were conducted across separate time epochs. Injected subjects showed inhibition in the normal improvement of simple reaction time (which occurs with practice), and they reduced the time required to make a decision. Choice reaction time, however, remained unchanged across the drug-nondrug experimental conditions.

"Are the differences between 'high performance' pacing leads clinically significant? A comparison of sintered platinum and activated carbon".

A retrospective assessment was performed of the acute implantation characteristics of two pacing leads, one with a sintered platinum (S 80-T) electrode and the other with an activated vitreous carbon (412-S) electrode in 124 patients. The S80-T lead had a significantly lower implantation threshold. The differences between the two leads in sensing properties, in current flow at threshold voltage and at the output of the generator were not significant at implantation. The 412-S lead had a higher impedance measurement than the S-80-T at implantation but this was mainly due to the resistance differences of the conductors rather than to electrode-patient interactions. A subgroup of 20 patients had chronic threshold measurements. The S 80-T lead had a significantly lower chronic voltage threshold. However because the acute and chronic differences are small we conclude that, although these differences are statistically significant, they are not clinically significant. Both can justifiably be termed 'high-performance' leads.

Effect of body inversion on hemodynamics determined by two-dimensional echocardiography.

The purpose of this study was to determine if head-down inversion of normal men from a supine position would increase blood return to the heart. Eight healthy, normovolemic men were placed into 10 degrees, 30 degrees, 60 degrees, and 90 degrees head-down tilt positions. BP and two-dimensional echocardiograms were recorded at the supine baseline and immediately after inversion. There was no significant change in left ventricular end-diastolic volume, stroke volume, cardiac output, or BP with inversion, except for a significant increase in diastolic pressure at the 60 degree position. The efficacy of placing hypotensive shock patients in the head-down tilt position is discussed.

Variation in the molecular size of the DNA from closely related strains of type I herpes simplex virus.

We have investigated the comparative genome size of five strains of HSV type 1 (HSV-1). Two of these strains have a common origin and differ in their plaque morphology on HeLa cells, one strain is a wild type isolate, and two others are established laboratory strains. All strains show high sequence homology based on the melting behavior of heteroduplexes. The greatest sequence divergence between any two strains was found to be no more than 10%. There are real differences in the size of the genomes of these strains of HSV-1 as measured by electron microscopy. The shortest and largest genomes measured differ in size by 10%, however, the size of the genomes of all strains are within 5% of a median valve of 87-88X106.

A simple, inexpensive, high pressure liquid chromatographic method for separating cytokinins in plant extracts.

Separation of a mixture of the main cytokinins occurring naturally in plant tissues was achieved by high pressure liquid chromatography using insoluble polyvinylpyrrolidone as the solid support. The separation of each cytokinin was first assessed over a range of salt and l-butanol concentrations and pH using a mixture of borate buffer and l-butanol as the mobile phase to determine the conditions necessary for optimum resolution. A discrete separation of zeatin, N-6-(Delta-2-isopentenyl)adenine, their related ribonucleosides, and kinetin was achieved using a simple isocratic elution with 0.025 m borate buffer at pH 6.8 and 4% (v/v) l-butanol. A number of cytokinin-active compounds were detected in cabbage extracts by the Amaranthus betacyanin bioassay using this separation technique.

Diagnostic concordance of telecytology and conventional cytology for evaluating breast aspirates.

OBJECTIVE: To evaluate the diagnostic concordance between static electronic images versus glass slides for breast fine needle aspiration (FNA) cytology. STUDY DESIGN: Nine malignant and 12 benign breast FNA cases were imaged using the Roche Image Manager, at a resolution of 1,024 x 768 pixels. A cytotechnologist or pathology resident selected five to nine representative images per case. Case histories and images were assembled into hypertext documents. Two pathologist blindly made diagnoses for each case based on the images and the clinical history and independently made diagnoses for the glass slides, also evaluating each set of images and glass slides for a list of cellular features. RESULTS: The two pathologist had concordance rates of 90.5% and 66.7% between their image and glass diagnoses. For each pathologist's discordant cases, 100.0% and 85.7%, respectively, were due chiefly to suspicious rather than definitive diagnoses. CONCLUSION: Evaluation of the electronic images indicated that almost all the cellular features were comparable to those viewed though a microscope.
