[Clinico-morphological changes in a burn wound after electric stimulation with pulsatile current].

The healing of sluggishly postburn wounds after local electrostimulation (ES) with the use of current-conducting carbon tissue as the electrode was studied in 12 patients. Analysis of the clinical data which were corroborated by the results of morphological and biochemical studies confirmed the marked stimulating effect of local ES which facilitates successful taking of dermal autografts and healing of burn wounds. The obtained data are evidence that the method of ES through a current-conducting carbon tissue is promising in the complex of measures for local treatment of burn wounds.

[Pathomorphologic changes of the solar plexus ganglia in burn disease].

Solar ganglia taken from 64 subjects who died in different periods of burn disease: (shock, toxemia septico toxemia, and burn exhaustion) were pathomorphologically examined. The specific time course of changes in solar ganglia was established in various periods of the disease. Dystrophic changes, necrosis of individual neuron groups, and varices of myelinated fibers were detected during burn shock, hypertrophic neurons with thick processes interwoven in between as "felt" predominanted in toxemia whereas in septicotoxemia and burn exhaustion, necrotic changes in neurons and degeneration of nerve fibrous bundle.

[Effect of controlled bacteria-free medium on the course of the wound process in patients with chronic venous insufficiency of the legs complicated by trophic ulcer].

Treatment in a controlled abacterial medium was applied to 102 patients with venous ulcer of the lower limbs. The course of the wound process was studied on the basis of objective examination methods (microbiological, cytological, morphological, study of the activity of tissue hemostasis factors). The results showed that a controlled abacterial medium causes favourable changes in the course of the wound process and allows the trophic ulcer to be prepared for operation in a short period of time (9-12 days).

[Vacuum treatment in the surgical management of suppurative wounds].

Vacuum treatment was used in 116 patients with purulent wounds. It was established that the vacuum treatment of purulent wounds was effective but after surgical treatment. It considerably decreased the amount of microbes in wound tissues, had no harmful effects and improved clinical results of healing the wounds after putting early sutures.

[Morphology of healed burns treated by different methods].

The dynamics of burn wound healing was studied on biopsies from the wound area in 30 patients treated by open methods (treatment under conditions of abacterial environment and in a cabinet with laminar air). Twenty patients were treated by applications on the wound of ointment bandages. Under the bandages, the crust formed on the burn surface was infiltrated with polymorphonuclear leukocytes. When patients with burns over 10-25% of the body surface were treated in abacterial environment, a thin dry crust developed on the burn wound surface with small accumulations of microflora in its external layers. The granulation tissue filling the wound after early necrectomy contains numerous vessels and cell elements. In extensive burns (35-50% of the body surface) the granulation tissue fills the wound cavity incompletely, as separate islets.

[Effect of water-soluble antioxidants on the permeability of lysosomal membranes and on the structure of the liver in rats with thermal burns].

Experiments on rats were made to study the effect of water-soluble antioxidants on the permeability of lysosomal membranes of liver cells and liver structure under burn. Antioxidants were injected intraperitoneally shortly after burn, whereas examination was performed after one day. It has been discovered that one day after burn there takes place an appreciable destabilization of lysosomal membranes with the release of a lysosomal matrix enzyme, cathepsin D to the cytoplasm. Liver structure had undergone substantial changes by that time. After administration of water-soluble antioxidants lysosomal membranes got stabilized while liver structure manifested but insignificant disorders.

[Morphological and cytological characteristics of the burn healing process by various treatment methods].

Morphological and cytological methods were used to study the healing of a burn wound in local isolation wards (an open treatment method) and under ointment bandages (a closed treatment method). Biopsy specimens and imprints from the burn wound surface were taken from 40 patients. During treatment in an abacterial medium, burn wound was rapidly decontaminated, which was a consequence of the appearance in wound exudate of segmented neutrophils with marked neutrophilic granularity. Granulation tissue was formed in the wound cavity at the 5th-7th day after the treatment commencement. During treatment under bandages wound decontamination was extremely slow. For a long time wound exudate shows the predominance of dystrophically changed neutrophils, while granulation tissue in the wound cavity if formed only at the 21st-24th day after the treatment commencement.

[Dynamics of the morphological changes in a burn wound during treatment in a controlled abacterial environment].

Histological and histochemical research methods were used to examine the healing of burn wounds treated under bandage and in monitored abacterial environment (open type treatment). Biopsies of burn wounds were investigated in 40 patients. It was shown that upon the treatment in abacterial environment the wound heal 10-12 days more rapidly as compared to those treated under bandage. Upon open type treatment there was an early appearance of fine granulations with marked macrophagal and fibroblast reactions, whereas under bandage there formed edematic granulations with high content of lymphoid and plasma cells.

[Action of a high-molecular toxin from burned skin on the hepatocyte lysosomal apparatus].

Experiments on rats were made to study the action of toxic glycoprotein isolated from burn skin on the lysosomal apparatus of hepatocytes and morphological structure of the liver. It was shown that 2-2.5 h after intraperitoneal injection to normal rats of toxic preparations of varying purity, the animals developed morphological alterations similar to those seen in the liver of the burn animals. At the same time there was an appreciable increase in the percentage of unsedimentable activity of catepsine D of the total one, thereby indicating the toxin-induced injury to the lysosomal apparatus of hepatocytes.

[Morphological characteristics of healing of extensive purulent wounds during treatment in the controlled bacteria-free environment].

Histological, histochemical and cytological techniques were used to examine the healing of extensive purulent wounds treated in a controlled abacterial medium (an open-type method of treatment). Biopsies were taken from 25 patients and explored throughout the entire treatment course (from 1 to 26 days). It was shown that the treatment brought about acceleration of wound purification and reparation. Wound tissues manifested a drastic lowering of the content of microflora and polymorphonuclear neutrophils. On days 3-5 of the treatment in the wound cavity there appeared close-grained granulations containing mature macrophages with high phagocytic activity and proliferating fibroblasts. On days 7-10 there emerged mature granulation tissue well fit for autodermoplasty.

[Morphology of the suppurative wound in the treatment process in a regulated environment].

Morphological and histochemical studies of biopsies from the areas of purulent wounds in the process of their treatment in a chamber with regulated environment were carried out. After 2-week course of treatment the purulent wound heals like a pure surgical wound. The granulation tissue contains a small number of leukocytes, the vessel is filled with polymorphonuclear leukocytes. A typical feature is the presence in the wound of lymphoid cell infiltrates in the maturing granulation tissue which indicates enhanced immune processes in the host. The results of treatment show that treatment of purulent wounds in a regulated environment considerably improves the condition of the wound contributing to its purification and stimulating healing processes in it.

[Histochemical study of experimental burn wound].

A composite histochemical and histoenzymechemical study of an experimental burn wound was carried out. The results indicated that the process of healing of the burn wound differed considerably from that of wounds of different etiologies. The burn wound was characterized by secondary obligate deepening, inhibition of leucocyte and macrophage reactions with a reduced functional activity of the cells, simultaneous existence of alterative-exudative and macrophage reactions, inhibition of granulation development, microbial dissemination in the wound and development of vasculites in the wound.

[Inhibitors of trypsin-like proteolytic enzymes as means of preventing the development of secondary necrosis in burn wounds].

The effect of proteolytic inhibitor contrical on the experimental burn wound healing was studied in rats using biochemical, histological and histochemical methods. In control untreated animals with flame burn of 20% of body surface wound healing was associated with development of secondary necrosis, marked inflammatory reaction, augmented activity of proteases and peptidases. The use of contrical prevented secondary necrosis; this effect was apparently related to reduction of tissue proteolytic enzymes activity.

[Effect of the protease inhibitor--contrical--on granulation tissue metabolism].

Studies of the influence of contrycal on metabolism of rat granulation tissue showed that this protease inhibitor stimulated C14-glycine incorporation into collagen proteins and caused reorganization of the isoenzyme LDH spectrum; at the same time there was a delay in the maturation of the granulation tissue with its marked growth.

[Current data on the functional morphology of granulation tissue cells in skin wounds].

The article summarizes the latest literature data on the morphology of the granulation tissue cells: neutrophils, monocytes, macrophages, mast cells and fibroblasts. In the majority of cases these cells were studied with the help of such methods as electron microscopy, histochemical analysis autoradiography. Sources of origin of these cells and dynamics of their development in the period of formation and maturation of the granulation tissue are considered. Findings of the current studies into the structure and functions of fibroblasts and their role in collagen formation, as well as the role of hormonal factors in these processes are analysed in detail.
