[Short- and long-term results of balloon dilatation of aorta coarctation].

Balloon dilatation is an alternative method of aorta coarctation (AC) treatment, and it may be effective in 80 - 90% cases with appropriate indications. Arterial hypertension is one of the symptoms of AC and frequent complication of short- and long-term postoperative period. Surgical correction does not guarantee regress of pathological symptoms in all the cases, and method of surgery does not influence the rate of postoperative arterial hypertension. The study reviews CA balloon dilatation experience and analyses cases of late arterial hypertension rate in long-term period after balloon dilatation procedure.

[PRKAR1A gene mutations in two patients with myxoma syndrome (Carney complex)].

Carney complex is an autosomic dominant disorder initially described as the association of cardiac myxomas, spotty skin pigmentation and endocrine overactivity and considered as a multiple neoplasia and lentiginosis syndrome. Mutations in the tumor suppressor gene PRKAR1A, coding for the type 1-alpha regulatory subunit of cAMP-depended protein kinase A have been previously identified in about half of the Carney complex kindreds. In this paper we report identification of the molecular defect in PRKARIA gene in two Carney complex patients. A new mutation (403delAC) located in a 3rd exon of PRKARIA gene has been observed in one case, and a previously described mutation in exon 7 (847delTC) in the second case.

[Regulation of the functional status of the lower esophageal sphincter with gastrointestinal hormones in cardiospasm and reflux esophagitis].

A role of gastrointestinal hormones in the regulation of the lower esophageal sphincter was studied in 22 patients with cardiospasm and 21 with reflux esophagitis. The levels of gastrin, vasoactive intestinal polypeptide (VIP), glucagon, insulin, and c peptide were determined by radioactive assay before and after surgical treatment. In opposite abnormalities (cardiospasm and reflux esophagitis), there is a different degree of VIP secretion both at the beginning and after functional exercises. Before and after functional exercises, the level of VIP was higher than in those with cardiospasm. The value of VIP on fasting and after functional exercises may be an additional information to establish the diagnoses of cardiospasm and reflux esophagitis and to evaluate the efficiency of the treatment performed.

[Clinical, diagnostic and therapeutic features of tricuspid infectious endocarditis].

From 1987 to 1998, examination and treatment were conducted of 12 patients with infectious endocarditis of the tricuspid valve (TIE). 11 of them were operated. TIE was clinically characterized by lesser circulation thromboembolism and marked right ventricular failure. TIE was successfully diagnosed by echo-CG (diagnostic sensitivity 83.3%). Serious disturbances of cellular immunity demanded immunocorrection. Indications for surgical treatment are listed.

[The surgical risk factors in patients with nonmalignant space-occupying lesions of the liver and the importance of immunological studies in assessing their extent].

Results of surgical treatment of benign new growths of the liver (BVML) depend not only on timely diagnosis of local growth but on precise analysis of surgical risk factors (SR) as well as accurate definition of its significance. The proposed classification helps to evaluate factors of SR in these patients. Results of the investigation showed that in majority of patients (94.7%) with BVML without complications immune system was were not suppressed. The fact may be considered as a favourable sign, demonstrating potential immunologic safety of patients in definite period of BVML development. Patients with lowered parameters of immunologic status made up a group of a high risk to develop inflammatory and septic complications in postoperative period. Definite correlation between the extent of immunodeficiency and frequency of postoperative complications has established the necessity of action to prevent the provocation of immunological status disturbances.

[Iron metabolism in patients with portal hypertension].

Ninety-four patients with portal hypertension (48 with cirrhosis of the liver and 46 with pathological changes of the portal vein trunk) were examined. The main parameters of iron metabolism were determined by the radioimmunoassay and the iron deports in the liver and spleen biopsy material were revealed. Tissue iron deficiency was found in all patients. The results of the tests were marked by essential peculiarities. In sharp reduction of the level of serum iron and ferritin the parameters of total and latent iron-binding capacity were decreased.

[Myxoma syndrome].

The paper provides the follow-up findings of patients with cardiac myxomas according to the presence or absence of the myxomal syndrome. It is shown that it is a multiorgan abnormality wherein the cardiac myxomas that are prone to primarily multiple growth, extraseptal and/or multifocal fixation are associated with the changes in the hypothalamus-pituitary-adrenal and renin-angiotensin-aldosterone systems, skin spotty pigmentation, neoplasms in the viscera, skin, and fat. These specific features emphase the predictive value of the myxomal syndrome, which predetermines that its early diagnosis should be made in routine clinical practice.

[B2 microglobulin in the diagnosis and prognosis of postoperative infectious complications in the pulmonary surgery].

The radioimmunoassay was used to study blood serum B2-M levels in 56 patients with pulmonary diseases (lung carcinoma in 20, chronic suppurative lung disease in 20, and benign lung disease in 16 patients) before the operation and in the early postoperative period. The control group was formed of 21 practically healthy individuals. A direct correlation between the character of the course of the postoperative period in patients with benign and chronic pulmonary diseases and the dynamics of changes of immunoreactivity (IR) was found. A favorable course of the postoperative period was marked by an increase of IR, in postoperative infectious complications IR did not change. Dynamics of IR was not revealed in patients with lung carcinoma, irrespective of the course of the postoperative period.

[Use of tumor markers in the differential diagnosis of voluminous formations of the liver].

The radioimmunological method was used to determine the content of tumor markers APP, REA, CA 19-9, and CA 72-4 in 53 patients with nonmalignant three-dimensional structures in the liver (hemangioma, nonparasitic cysts, alveococcosis, hydatid cysts), in 9 patients with primary carcinoma of the liver, and in 24 healthy individuals. The significance of APP determination and complex study of tumor markers in the recognition and differential diagnosis of benign and malignant structures in the liver is shown. It allowed the percentage of disparity between the laboratory and the verified diagnosis to be reduced to 4.8%.

[The current pathogenetic aspects of diarrhea in ulcerative colitis].

The authors consider different mechanisms participating in progression of diarrhea in ulcerative colitis. The involvement of osmotic component was suggested in view of incomplete hydrolysis of carbohydrates by small intestinal glucoamylase under mono- and bisubstrate digestion, whereas diarrhea secretory component develops in participation of cyclic nucleotides. Motor mechanism is also present in diarrhea as evidenced by high concentrations of blood motilin and changes in intestinal evacuation and motility. Polycomponent dysbioses detected in ulcerative colitis patients may also be essential for pathogenesis of diarrhea. Determination of the leading component in each diarrhea case is thought helpful in valid choice of individual therapeutic policy.

[Immunological indices of "drainage" blood used for compensation of blood loss].

The authors discuss experience in compensation for blood loss in patients with ischemic heart disease after aortocoronary shunting using drainage blood without or with donor blood (80 persons) and donor blood alone (60 persons). Mild changes of immunity cell factors were revealed and no differences in the concentration of class A, M, and G immunoglobulins and phagocytosis as compared to the indices recorded before the operation and 24 hours after it. Analysis of postoperative infectious-inflammatory complications showed their incidence to be less in patients who received drainage blood without or donor blood (23%) than in those given transfusion of only donor blood (55%). The results of the study allow drainage blood to be recommended for reinfusion.

[Myoglobin as an indicator of stress damage].

The effects of single 1-hour immobilization stress exposure and of long-term (40 days) adaptation to intermittent high altitude hypoxia (in hypobaric chamber at "altitude" 4000 m, 4 hr, a day) on serum myoglobin content were studied in Wistar rats. It has been shown that such stress exposure induced the 2.5 fold increase in the serum myoglobin content. The adaptation did not change the myoglobin content, but effectively limited mentioned above stress-induced increase in the serum myoglobin level. These data suggest that serum myoglobin content can be used as a criterion for stress damage and efficiency of factors protecting against stress damage.

[Endocrine response to extracorporeal circulation].

The authors analyze the results of measuring the content of vasopressin, renin, angiotensin II and aldosterone coupled with the results of studying hemodynamics, acid-base state, and lactate in the arterial blood during operations on the heart in 32 patients with different acquired heart diseases. The main attention was concentrated on studies into the nature of humoral changes during extracorporeal circulation in three types of anesthesia. In group I, anesthesia was maintained by fentanyl given in a dose of 5-6 micrograms/kg/h, diazepam (0.1 mg/kg/h), and arduan (0.02 mg/kg/h). In the second observation group, at the beginning of perfusion the patients were administered trimetotan (artonad) (0.2 mg/kg), and in group III, the dose of fentanyl was raised during perfusion to 10-12 micrograms/kg/h. It is concluded that during extracorporeal circulation, it is desirable that the dose of fentanyl be increased to attain more adequate anesthesia in that period of heart surgery. The magnitude of humoral changes occurring in the body during extracorporeal circulation served as a criterion for anesthesia adequacy.

[The effect of cyclooxygenase inhibition on the indices of the thrombocyte-vascular link in hemostasis and on the free-radical processes of lipid oxidation in experimental Salmonella intoxication].

Injection of Salmonella typhimurium endotoxin to the laboratory animals (rabbits) in dose of 1 mg/ml (LD84) induces the particular changes in the thrombocyte vessels system of hemostasis: decrease of aggregatory ability of thrombocytes, increase of thromboxane A2 and prostacyclin activation of lipid peroxidation process. Use of indomethacin--the cyclooxygenase inhibitor--leads to less progressive alterations of the studied parameters of the thrombocyte vessels hemostasis and lipid peroxidation processes.
