[Synchronous bilateral breast cancers].

In application of mammography and aiming biopsy, synchronous carcinoma of contralateral breast may be detected in 2.4% of cases, on the average, 3 years earlier than by clinical examination only. Increased proliferation of contralateral breast tissue (a factor contributing to tumor development) is not a pathognomonic symptom since proliferation rate in ductal and lobular tissues decreases with age and in menopause. At least half metachronous carcinomas of the second breast should be considered synchronous ones overlooked in the course of primary treatment.

[Differential diagnosis of primary and metastatic ovarian tumors in patients treated for breast cancer].

The report deals with some problems of differential diagnosis between primary and metastatic ovarian tumors in 78 patients treated for breast cancer (without local recurrence). Clinical symptoms, results of palpation, evidence supplied by surgical intervention, data of cytological examination of exudates and even the findings of express histological analysis of bioptates fail to provide a sufficient improvement on diagnosis. Final diagnosis can be obtained by means of an all-round histological examination only. The extent of surgical procedure should match the operation of choice for primary ovarian tumor. Since ovarian involvement is rather frequent in breast cancer patients, they should be followed up by the gynecologist in the course of the whole survival time.

[Use of adriamycin in the treatment of metastatic breast cancer].

Adriamycin (alone and as a component of FAC scheme) should be administered to patients with disseminated breast cancer resistant to cytostatic drugs. This treatment is followed by remission and arrests the process for an average of 9 months (in some cases up to 30 months). Mean doses of 30-40 mg should be given to outpatients on days 1 and 8 of each course, with intervals between courses being 21-28 days. No apparent side-effects are observed. The total dose should be as high as 450-550 mg. Adjuvant antiestrogen treatment (tamoxifen) should be given to pre- and menopausal patients.

[Pulsed laser radiation in the treatment of recurrences of breast cancer].

Radiation from a pulsed laser installation Pulsar - 1000 (wavelength - 1.060 nm. pulse duration - 1 ms) was used in the treatment of 87 recurrences of breast cancer in 31 patients. Fluence ranged 300-450 J/cm2 depending on the depth of recurrence. No recurrence of the first relapse was registered within six months - three years after exposure to laser radiation. Pulsed laser radiation was shown to be an effective means of treatment of localized relapse of breast cancer which does not involve musculature or thoracic wall. Laser therapy should be considered a method of choice in patients who have previously undergone radiation treatment as well as in cases of clusters of advancing relapse. Laser treatment of recurrent breast cancer may be conducted in conjunction with chemo - or hormone therapy.

[Use of "split" courses of combination chemotherapy in breast cancer].

The results of treatment of 110 out-patients with breast metastases, using various repeated schemes of chemotherapy, are discussed. The results and recommendations may be integrated with the procedures of selection of schemes of breast metastasis treatment. Good tolerance, virtual absence of side--effects and complications (e. g. persistent leukopenia) were observed. Integration of the new scheme with antibiotic treatment (carminomycin and adriamycin) is suggested.

[Changes in the primary focus and regional metastases of breast cancer under the influence of concentrated preoperative irradiation].

Using of concentrated preoperative radiotherapy in 84 breast cancer patients failed to result in any quantitative and qualitative enhancement of the damaging effect on tumor parenchyma and lymphnode metastases as compared with the routine fractionation technic of irradiation. Shortening of the time of irradiation and the interval between termination of gamma-therapy and the operation should be regarded the principal advantage of the concentrated irradiation technic. Morphological assays of the specimens obtained rise the question on the necessity of increasing a total dosage for zones of regional metastases up to 1500 rad.

[Treatment of phylloid tumors of the breast].

Phylloid fibroadenoma of the mammary gland is a comparatively rare lesion--0.3% of all breast affections, its clinical course is generally known to be characterized by both the possibility of recurrences development after non-radical treatment and its transition into mammary gland sarcoma. The authors report the data on 56 cases of phylloid fibroadenoma of the mammary gland treated at the N.N. Petrov Research Institute of Oncology of the USSR Ministry of Health during the period of 1926 to 1973. Fifty one of these patients showed benign tumors, 5--malignant tumors. The clinical diagnosis of the tumor is rather complicated (the correct preoperative diagnosis was established only in 27.5% of cases). The use of non-contrast mammography and trepanbiopsy failed to appreciably improve the diagnosis. The differential diagnosis between the benign and malignant forms seems to be mostly difficult. Only surgery is advocated. The extent of surgical intervention depends on morphological features of the tumor, its size, rate of growth and the breast size. Among benign forms recurrences were noted in three patients (6.3%). Of 5 patients with malignant phylloid fibroadenoma two patients died due to metastases during two postoperative years.
