Knee arthrodesis with the Wichita fusion nail.

We reviewed 32 patients who all had knee arthrodesis performed after failed knee replacement. The minimum clinical follow-up was 1 year. The arthrodesis was performed by means of the Wichita fusion nail in 11, by external fixation in 15 cases, by plating in three and by intramedullary nailing in three. The mean patient age was 68.6 years. When the Wichita nail was used, fusion was achieved in ten out of 11 cases after a mean period of 4.5 (3-7) months. Of the remaining 21 patients, fusion was only achieved in 11 cases after a mean period of 6.5 (4.5-10) months.

A comparative study of trochanteric fractures treated with the Gamma nail or the proximal femoral nail.

The aim of the present paper is to compare the results obtained using two osteosynthesis systems developed for the surgical treatment of fractures of the trochanteric region of the femur, based on the principles of closed intramedullary nailing: the Gamma nail and the proximal femoral nail (PFN). A prospective study is presented of 125 trochanteric fractures of the femur treated with PFN and 125 treated with the Gamma nail in our service between the years 1997 and 2000, with the prerequisite of a minimum follow-up of 1 year. Fractures were classified according to the AO system, the most commonly recorded subtype encountered being the A2. Clinical and radiographic controls were performed upon admission and in the post-operative follow-up at 1, 3, 6, and 12 months, the corresponding assessment protocol being completed on each occasion. Of the 250 patients, 179 were women and the rest men. The average age was 78.9 years. Both systems enabled early mobilisation and walking in most of our patients. There were no significant differences in the use of either nail in terms of the recovery of previous functional capacity, nor in terms of the time required for fracture healing (12 weeks on average). With regard to the more significant technical complications recorded, shaft fractures and the cutting-out phenomenon were more common with the use of the Gamma nail, while secondary varus occurred at a greater rate when using the PFN.

Trochanteric fractures treated with a proximal femoral nail.

We followed prospectively 295 patients with trochanteric fractures treated with the proximal femoral nail. The average age of the patients was 80 years and three out of four were female. The most frequent fracture type was A2 (59%). Clinical and radiographic controls were performed at 1, 3 and 6 months. There were technical complications during the operation in 12% of the cases, complications in the immediate postoperative period in 27% and late complications were detected in 4% of patients. Previous walking ability was recovered by 71%. The surgical technique is not complex, the number of complications recorded was acceptable and the overall results obtained are comparable with other fracture systems.

Living arrangements of the Filipino elderly.

The authors describe the living arrangements of the elderly in the Philippines. They find that "only a small fraction of the Filipino elderly are isolated from close kin; however, it is not certain that their living arrangements are favourable for their overall well-being. In many cases of co-residence, the flow of support is from the elderly parents to their children. The article raises several questions concerning social change in the Philippines that need to be answered in future research."

Current fertility: a first glimpse from the 1983 National Demographic Survey.

This paper highlights initial findings from the 1983 National Demographic Surveys (NDS) based on a 25% sub-sample of the survey respondents. The discussion focuses on measures of current fertility as well as on selected fertility "predictors" for the 3 major island groups -- Luzon, Visay, and Mindanao. Owing to the restrictive nature of the data set, no statistical tests are performed to measure the influence on fertility of these explanatory variables. Instead, inferences are drawn from a descriptive analysis of the patterns exhibited. Findings point to a continuation of fertility decline as evidenced by the estimated crude birthrate of 33.6 births/1000.
