Managing care for mental illness: paradox and pitfalls.

Historically mental health has been separated from physical health care, and within mental health there have been largely separate public and private systems. A managed care approach now offers the potential of integrating and rationalizing mental health care. Counihan et al. and Stelovich represent the historical forces of mental health and health care, playing out as the two key models of managed care: the carve-out and the integrated model. However, the advent of managed care in today's health care system also presents significant dangers, particularly for vulnerable populations such as adults and children with severe and longterm mental disorders.

Reflections on the mental health community's experience in the health care reform debate.

The creation of an effective coalition of mental health organizations was integral to the mental health community's early success in furthering coverage for mental health services in national health care reform. A unified coalition was able to establish political credibility and clarify substantive choices while the Task Force on National Health Care Reform was formulating the Clinton Administration's proposal, leading to initial indications that parity for mental health benefits and flexible coverage of a wide range of services would be included. As concerns about the cost of the benefit package increased, the Administration stepped back from including comprehensive coverage, and some organizations in the coalition became divided over specific provisions. However, as of June 1994, the mental health community's consistent consensus about the need for comprehensive coverage and parity enabled the field to gain inclusion for mental health services and, in fact, wider coverage in some of the proposals emerging from Congressional subcommittees.

Deinstitutionalization and mental health.

Better understanding of the problems related to deinstitutionalization is essential for all those involved with community care. Finding ways to address the issues raised by deinstitutionalization is the key to using all community resources-including home care-in building a better system of mental health care.

Mental health law and public policy.

Litigation has been a successful strategy in securing rights for the mentally ill. After early victories, however, limits of litigation began to emerge; implementation of court orders raised many policy questions. The translation of broad rights into complex public policy has led directly to the legislative and administrative policy processes, as can be seen with the proposed Mental Health Systems Act. The author uses the evolution of Title 3, a rights and advocacy amendment of the proposed act, as a case study in the formation of public policy.
