The current study evaluated long-term effects of a training model in human sexuality on public school teachers' knowledge and attitudes. The data indicated that, although significant changes were found from the pretest to posttest assessment of knowledge and attitudes on two standardized measures, attitudinal changes for teachers could not be maintained over time. In addition, knowledge decreased with most drastic changes occurring in the educator who, while trained, did not teach the material in a classroom setting. The results of this model staff development program suggest factors that may alter sex education training effects for professionals who work in public school settings.
Examination of the job satisfaction of paraprofessional and professional mental health employees has revealed that black employees were significantly less satisfied with their jobs then were white counterparts. A cultural explanation for these observed differences would hold that values and beliefs contributing to job satisfaction differ across race. This hypothesis was tested by separately factor analyzing 79 black and 222 white employee responses for black and white employees were compared using the Pearson Product Moment Correlation Coefficient and revealed six partially convergent job satisfaction themes. However, differences in the complexity and importance of analogous black and white factors supported the hypothesis that differing dimensions underlie job satisfaction for black and white employees.
The authors describe a 2 1/2-day training program designed to teach mental health professionals how to use a new DSM-III classification scheme. The 251 participants had previous clinical training but no prior exposure to DSM-III. They were asked to diagnose three cases from a series of five videotapes case vignettes; the authors then compared the participants' diagnoses with the expert opinion of clinical faculty. The degree of agreement among the participants with the expert opinion varied across the five cases, but the overall level of agreement was high (74.2%). These results indicate that with adequate training the new DSM-III classification scheme can be accurately applied to diagnostic situations.

Abnormalities of cell-mediated immunity-anergy to delayed hypersensitivity skin testing, diminished T-lymphocyte concentrations and mitogen responsiveness, and the presence of serum inhibitors--have been shown to be present in patients with portal venous obstruction and normal hepatic function. In contrast, tests of humoral immunity--antibody titiers to viral and bacterial antigens and immunoglobulin concentrations--were normal. These abnormalities in cell-mediated immunity are probably the result of the development of portal hypertension, portosystemic shunting, and congestive splenomegaly. These studies raise the possibility that qualitatively similar defects in patients with chronic liver disease and portal hypertension may, in part, be attributable to the same mechanisms.
The purpose of this study was to investigate the relationship between sex and mental illness. A sample of male and female adult psychiatric outpatients (n = 44) completed the Denver Community Mental Health Questionnaire (DCMHQ), the Taylor Manifest Anxiety Scale, an abbreviated version of the Minnesota Multiphasic Personality Inventory (the Mini-Mult), and the Social Readjustment Rating Scale. Differences in life event experiences, as well as differences on measures of anxiety, depression, psychological distress, and social functioning, were explored and identified by means of Pearson product-moment correlations. The strongest correlations were found between sex and four of the subscales of the DCMHQ: women tended to be less productive and evidenced more psychological distress than men; men had more legal difficulties and were more likely to use soft drugs. A modest correlation was found on the Taylor Manifest Anxiety Scale, with women showing higher scores.
Extra-hepatic obstruction of the portal vein is a well known cause of hypertension in childhood and 55 out of the 97 patients (57 per cent) seen with this condition presented before they were 15 years old. However, nearly half our cases (43 per cent) presented in adult life. Haematemesis was the commonest mode of presentation in both the adult and childhood group although splenomegaly was common, especially in the children. The severity and frequency of haematemesis increased during puberty in the children. Twelve children and three adults have now not bled for between five and 30 years, 10 of them ceasing spontaneously and five following surgery. Intra-abdominal infection or septicaemia precipitated the portal venous obstruction in 38 patients (39 per cent), though the exact cause of such obstruction in patients who had no cirrhosis was obscure in about half the cases. Ascites was present in 18/51 (35 per cent) of the children and 24/35 (69 per cent) of the adults. Its presence was associated with an increased mortality (p less than 0.01). Nineteen out of 42 (45 per cent) patients with ascites also had portal-systemic encephalopathy. Twenty-four patients died, nine having presented during childhood, variceal haemorrhage was responsible for death in 19 and infection in five. Sixty-four patients underwent 114 operations for variceal haemorrhage. Mortality was greater in the surgical group compared with those managed conservatively. Surgery is therefore indicated only in the rare case where bleeding cannot be controlled by medical means.

The state of the extrahepatic portal venous system was correctly assessed by grey-scale ultrasonography in twenty-one patients with extrahepatic portal-vein obstruction and the results agreed with those obtained by portal venography. In twenty-two age-matched controls a patent portal vein was displayed. The diameter of the portal vein on the ultrasound scan was significantly less in the twenty-two controls than in eighteen patients with chronic liver disease. Grey-scale ultrasonography is a reliable, inexpensive, and non-invasive method for diagnosing extrahepatic portal-vein obstruction.
