Experience with epigastric free flap allograft in the rat.

We report our continuing experience with the rat epigastric free vascularized flap as a transplant model. Four groups of ten pure-bred rats underwent transplantation of epigastric vessel based skin flaps. The flaps were immediately revascularized in the recipient animal and the results recorded. No immunologic manipulation was performed. Rejection and acceptance of the flaps occurred according to known genetic predictions in these 40 procedures. This study will serve as a control for immunologic manipulation experiments.

Osteoid osteoma of the hand.

A review of all cases of osteoid osteoma of the hand seen by four hand surgeons over the last 10 years was performed. Seven cases were documented. Average follow-up was 28.3 months. Average age of the patients was 21.1 years. Five men and two women participated. Six lesions were in the right and one in the left upper extremity. Delay from presenting symptoms to definitive treatment averaged 13.5 months with a range of 7 to 30 months. Surgical excision was curative in all cases. We concluded that although a very unusual occurrence, osteoid osteoma of the hand should be considered in the differential diagnosis of pain in the hand.

Congenital carpal tunnel syndrome. A case report.

A boy with bilateral idiopathic congenital carpal tunnel syndrome was first seen at the age 4 years 6 months. He presented with triggering of the finger flexors at the wrist, abnormal posturing of the fingers and inability to pinch on one side. The triggering was relieved by transection of the transverse retinaculum. Pinch was restored by a tendon transfer. There have been no previous reports of idiopathic carpal tunnel syndrome present since birth. The literature and case are reviewed.

The natural history of scaphoid non-union. A review of fifty-five cases.

We reviewed the cases of fifty-six scaphoid non-unions in fifty-five patients, none of whom had received treatment of any kind before examination. In the thirty-two patients who had been injured five years or more earlier, arthritis developed in thirty-one (97 per cent). The one patient in whom osteoarthritis developed less than four years after injury also had avascular necrosis of the scaphoid. The incidence of osteoarthritis increased with time after injury. We concluded that patients with established scaphoid non-union should be advised that osteoarthritis will most likely develop.

Nocardia asteroides thenar space infection: a case report.

Nocardia commonly is thought of as producing a chronic pulmonary or granulomatous skin infection, but this report illustrates that it may begin as an acute inflammatory process. In view of the tenosynovitis which followed exploration of the tendon sheath in the patient presented and which required a second operation, it may be better to aspirate the sheath at a distant site to look for pus. Actinomycetes should be suspected and looked for with appropriate methods in any suppurative lesion, especially one not responding to conventional antibiotics.
