Small dose anti-Rh therapy after first trimester abortion.

The standard prophylactic dose of anti-Rh immunoglobulin used in the United States is 300 microgram. For use after spontaneous or induced abortion and diagnostic amniocentesis, this dose represents immunological "overkill". A 50 microgram dose has been effectively used for these patients in other countries but the specific products used were not equivalent to the products available in the USA. No evaluation of this 50 microgram dose has been conducted in the USA. This is a pilot study of 315 patients given a 50 microgram dose or a 300 microgram dose of anti-Rh immunoglobulin after first trimester abortion. No patient in either treatment group developed atypical blood group antibodies within 6 months. This finding confirms that 50 microgram of the American product is protective after first trimester abortion.

Culdotomy for female sterilization.

Tubal occlusion through a culdotomy incision offers a method of surgical sterilization acceptable to women in some countries where abdominal sterilization is not acceptable. This technic requires skilled gynecologic surgeons, who are a limited resource in many developing countries. On the other hand, gynecologic surgeons practicing in outlying areas may find it more feasible to perform colpotomy sterilizations than laparoscopic sterilizations with the latter's requirements for specialized instrumentation and additional training. With adequate attention to operator training, patient selection, evaluation of patients for postoperative complications and appropriate care of those with such complications, culdotomy should be included among the various approaches appropriate for female sterilization.

Medical equipment costs: those you see and those you pay.

A review of life cycle and operation support costestimating capabilities in the health care industry shows few models that discuss total ownership costs for major items, let alone for some expensive minor ones. As yet, no data collection system exists wthat will provide total costs of acquisitions. There is a clear need for the health industry to focus greater attention on life cycle costs of medical equipment. This is particularly pertinent to those physicians responsible for medical equipment acquisition programs. The industry must evaluate the impact of life cycle costs and make such evaluations available to decision makers. The purpose of this paper is to improve the visibility and accuracy of cost estimating by examining the elements of life cycle costs.

Costs and Cost Analysis

The transfusion of Rh-positive blood into Rh-negative women.

The incidence of occurrence of transfusion of Rh-positive blood into Rh-negative individuals is unknown at the present time. A survey was designed to ascertain this information for the 12 month period ending March 31, 1974. Replies were obtained from 2,286 transfusion facilities in the United States. A total of 203 patients were reported to have received Rh-incompatible transfusions during the interval studied. Of these, 44 were women 49 years of age or under. Slightly more than half the women who received Rh-positive blood were treated with Rho (D) immune globulin. Those who received an adequate dose did not develop antibodies when appropriately tested. Twenty-five per cent of those who received an inadequate dose of Rh-immune globulin developed antibodies. Forty-two per cent of those receiving no Rh-immune globulin prophylaxis were noted to have antibodies.

Leukemia in twins: antenatal and postnatal factors.

Two factors emerged from a search for obstetric phenomena that might explain concordance of leukemia in both members of a twin pair within days or months of each other: antenatal exposure to ionizing radiation; and antenatal cojoined intrauterine circulation. In addition, antenatal tumor metastasis and chromosomal changes, antenatal or postnatal, may be contributory. Continued observation of reports should be carried out.

Adolescent

Cervical gonorrhea in women using different methods of contraception.

Cultures were made from the cervix, rectum, and oropharynx of 2,019 women to determine the prevalence of gonorrhea. For patients of similar race and age, the rates of cervical gonorrhea among users of oral contraceptives (10.6/100) or IUD users (9.5/100) were significantly greater than observed with patients using barrier methods, condom-diaphragm-foam (1.7/100). On the other hand, there were no significant differences in rates of rectal or oral infection by method of contraception. Postpartum patients were found to have similar infection rates at all three sites as a comparable group of nonpuerperas. Recommendations for utilization of barrier methods are made for suitable patients, including those in the immediate puerperium.

Attitudes of men toward contraception.

This report describes a pilot study conducted to determine the attitudes of men toward contraception. A questionnaire elicited responses from 436 males regarding: Personal data; contraceptive responsibility; contraceptive knowledge; use of contraception and preference for future methods. Results include attitudes toward contraceptive responsibility, assistance to pregnant partners and knowledge regarding eight common contraceptives. Usage of contraceptives in general and condoms and withdrawal specifically is reported. Preference for eight common contraceptive methods is also presented, along with data on delay of intercourse due to lack of contraceptives, partners' requests for male use of condoms/withdrawal and attitudes toward dissemination of contraceptive material to the public. Also reported are attitudes toward development of new contraceptives, use of male sterility shots or pills and the prices that men would pay for newly developed male contraceptives. The most preferred male contraceptive for future development is reported. It was concluded that the survey form and methodology were valid; that men desired to participate in family regulation; that the majority of men placed a low economic value on male contraceptives and finally that the overwhelming majority of men wanted contraceptive information to be widely disseminated in the public.

The odors of the human vagina.

Gas-chromatography analysis of 90 samples of vaginal secretions from 10 women revealed that at any given sample time the perceptible vaginal odor of each individual women was composed of numerous separate odorous effluents. Almost 2,1000 odoriferous effluents were observed. This corresponded to 16% of all recorded peaks on the 90 gas-chromatograms. The odorous effluents were assigned 34 characteristic odors in three hedonic categories (neutral, pleasant and unpleasant). This information supports the contention that the olfactory signature of an individual is complex, highly individual and composed of many "mini odors".
