Mechanism of action of antipruritic drugs.

Astemizole and terfenadine, two potent non-sedative H1 antihistamines, had no effect on itch measured objectively as nocturnal scratching and subjectively on a 10 cm line. Trimeprazine, however, a more sedative but less potent H1 antihistamine, was antipruritic, as was nitrazepam, a sedative benzodiazepine. We concluded (a) that antipruritic drugs act centrally by a property related to sedation; (b) H1 receptor antagonists have a peripheral antipruritic action only when itch is due to histamine release, as in the wealing disorders. Thus the new nonsedative H1 antihistamines have no place in the treatment of itch from other causes.

Assessment of a 35% fluorocarbon emulsion.

We have previously reported on the efficacy of a 20% fluorocarbon emulsion (Fluosol-DA, 20%) as an acellular O2 carrier at an FIO2 = 1.0. We are concerned, however, about the potential O2 toxicity that may result from extended exposure to FIO2 = 1.0. The O2 content of the fluorocarbon phase is linearly related to both the FIO2 and the fluorocarbon concentration (Fct). It should therefore be possible to maintain the same O2 content by raising the Fct using a higher fluorocarbon concentration and lowering the FIO2. The purpose of this report is to assess the ability of a 35% fluorocarbon emulsion (Fluosol-DA, 35%) at an FIO2 = 0.6 to support hemodynamics and O2 transport. Five adult baboons were paralyzed, anesthetized, intubated, and mechanically ventilated at FIO2 = 0.6. An isovolemic total exchange transfusion (E.T.) with Fluosol-DA, 35% was performed. Measurements were made at Hct's of 20, 10, 5, and less than 2%. All animals survived the exchange. Total arterial O2 content fell from 17.4 +/- 0.7 to 3.3 +/- 0.2 vol% (p less than 0.01), and O2 delivery decreased from 21.8 +/- 2.2 to 5.1 +/- 0.7 cc/min-kg (p less than 0.01) during the exchange. There was no significant change in MAP, H.R., C.O., or VO2 during the exchange transfusion. Fluosol-DA, 35% maintains normal hemodynamics and O2 transport despite a marked fall in arterial O2 content and total O2 delivery. Fluosol-DA, 35% is thus an effective O2 carrier at the safe FIO2 of 0.6.

Adipocyte cholesterol storage: effect of experimental hypercholesterolemia in the rat.

In order to more clearly define the influence of adipocyte size and plasma cholesterol on adipose tissue cholesterol storage, hypercholesterolemia was induced in male Fisher rats at a time when body weight and adipocyte size become fixed. In one experiment (experiment 1), increasing amounts (0, 0.05, 0.5 and 5.0%) of dietary cholesterol were added to a fat-free, purified diet (sucrose-casein-cellulose) fed to 1 year old virgin males. In experiment 2, retired breeder rats were fed 0 or 5% cholesterol, with and without corn oil supplementation of the purified diet. The plasma cholesterol responses (in experiment 1) to the various dietary cholesterol levels were in the order 0 = 0.5 greater than 0.05 greater than 5.0% = stock diet. In experiment 2, however, rats fed cholesterol had higher plasma cholesterol values than those fed the cholesterol-free diet. When total cholesterol concentration per 10(6) adipocytes was examined in four adipose depots, it was found that adipocyte cholesterol content tended to increase with increasing levels of circulating total cholesterol, regardless of the cholesterol content of the diet. The accretion of cholesterol in adipocytes was accompanied by enhanced storage in the esterified form, a response very similar to that of liver in these experiments but unlike skeletal muscle. It is concluded that adipose tissue participates physiologically in the response to hypercholesterolemia in the rat, but that the response may depend upon the nutritional or metabolic state of the animal prior to cholesterol feeding (young rats versus retired breeders) and is not similar in all adipose tissue depots.

Effectiveness of stress management training for nurses working in a burn treatment unit.

All fourteen nurses working in a burn unit were given three hours of individual training in cognitive behavioral stress management skills. Anxiety measures were obtained before, during, and after training by weekly administration of the State-Trait Anxiety Inventory, and by daily administration of a nurses' stress scale constructed for this study. Nurses were randomly assigned to two groups which were treated using a successive-groups time-series design. Results suggested that stress management training was effective in reducing work-related anxiety among inexperienced nurses but not among experienced nurses. The gains made by the inexperienced nurses may have been more associated with increases in their understanding of stress and their feelings of control over stress than with changes in their actual stress management behavior.

Adamantiades-Behçet's disease with inner ear involvement.

Adamantiades-Behçet's disease is a chronic recurrent inflammatory disorder involving the small and large vessels. Typical loci of manifestations are the mucous membranes, skin and eyes, as well as the joints and central nervous system. Other organs are not commonly involved. We present two patients, one with ocular and the other with mucocutaneous manifestation of Adamantiades-Behçet's disease. In addition, the first patient reported three episodes of sudden hearing loss while under immunosuppressive therapy for his eye involvement. The second, therapy-naive patient complained of tinnitus in his left ear. Careful examination revealed vestibular involvement in the first patient and retrocochlear involvement in the second. Inner ear involvement is an uncommon manifestation of Adamantiades-Behçet's disease. In case of relevant signs or history, such as hearing disturbance, tinnitus and/or vertigo, patients should be examined for inner ear involvement.
