Long term radiologic outcome of Pseudomonas osteomyelitis of the foot.

The medical records of 27 patients with puncture wound-induced Pseudomonas aeruginosa osteomyelitis/septic arthritis of the foot hospitalized from 1980 through 1988 were reviewed. Twenty patients received anti-Pseudomonas therapy for greater than 21 days and 7 patients received anti-Pseudomonas therapy for less than or equal to 21 days. There was no difference in age, sex, race, duration of wound or symptoms before admission or in admission physical findings including temperature, lymphadenopathy, puncture site appearance or admission laboratory data including white blood cell count or erythrocyte sedimentation rate between the two groups. All children but two in the long term therapy group had surgical as well as parenteral anti-Pseudomonas therapy. Fifteen patients returned for follow-up radiographs 1 to 8 1/2 years after hospitalization, 10 of 20 who had received therapy for greater than 21 days and 5 of 7 who had received therapy for less than or equal to 21 days. Poor radiologic outcome, ranging from bony deformity to joint space abnormality, was noted in 4 patients, of whom 3 had had joint involvement. Clinical abnormality was noted in only one adolescent male, and he had the most severe radiologic sequelae. The longer term functional significance of these radiologic anomalies awaits further delineation.

Efficacy of sponging vs acetaminophen for reduction of fever. Sponging Study Group.

Seventy-three children with acute febrile illnesses were enrolled in a study to compare the efficacy of sponging, sponging plus acetaminophen, and acetaminophen alone as methods of lowering body temperature. The greatest temperature reduction was seen in the combined acetaminophen plus sponging group. The smallest temperature reduction was noted in children who received sponging alone. We urge reconsideration of routine sponging of febrile young patients.

Ferrochelatase activity in Azospirillum brasilense with reference to the influence of metal cations.

Ferrochelatase in membrane preparations from Azospirillum brasilense displayed an activity of 2.17 mumol protoheme formed.h-1.mg protein-1 which is 10-fold greater than previous reports for other bacteria. This ferrochelatase showed an apparent Km of 20.9 microM for Fe2+, a pH optimum of 6.0-6.5, and stimulation by oleic or stearic acids. Co2+, Cu2+ and Zn2+ inhibited the incorporation of Fe2+ into protoporphyrin IX while Ni2+ and Mg2+ had no effect on protoheme synthesis. Activity with Fe2+ and mesoporphyrin IX was less than with protoporphyrin IX but deuteroporphyrin IX produced the highest rate of protoheme synthesis. The membrane fraction containing ferrochelatase activity was found to insert Cu2+, Ni2+, Zn2+ and Co2+ enzymatically into protoporphyrin IX to produce metalloporphyrins. Cu2+ incorporation into protoporphyrin IX proceeded at a rate greater than with Fe2+ and the Km for Cu2+ was 21.9 microM.

Impetigo contagiosa III. Comparative efficacy of oral erythromycin and topical mupirocin.

Ninety-seven patients with impetigo were prospectively enrolled in a study to determine the comparative efficacy of systemic and topical antibiotic therapy. After obtaining a bacterial culture from a representative lesion, the children were randomized to receive seven days of either oral erythromycin or topical mupirocin administered three times daily. Staphylococcus aureus alone was isolated from 51% and in association with group A beta-hemolytic streptococci (GABS) from 29%; GABS alone was isolated from 4% of patients. Of 48 children who received erythromycin, 43 (90%) were clinically improved or cured, and 11 of 17 were bacteriologically cured. Of 49 children who received mupirocin, 47 (96%) were clinically improved or cured, and 10 of 14 were bacteriologically cured. At three-week follow-up, clinical cure rates and number of secondary household cases of impetigo were equivalent in both treatment groups. Mupirocin appears to be a well-tolerated, albeit expensive, alternative to erythromycin for the treatment of impetigo.

Alkaline Phosphatase and Other Hydrolyases Produced by Cenococcum graniforme, an Ectomycorrhizal Fungus.

Cell extracts of Cenococcum graniforme have been found to contain the following hydrolytic enzymes: protease, esterase, alpha-d-galactopyranosidase, beta-d-galactopyranosidase, alpha-d-mannopyranosidase, beta-d-xylopyranosidase, alpha-d-glucopyranosidase, beta-d-glucopyranosidase, and alkaline phosphatase. Sulfatase, inorganic pyrophosphatase, and beta-d-mannopyranosidase were not detected in the extracts. beta-d-Xylopyranosidase and alpha-d-mannopyranosidase were most active in the neutral pH range, protease and phosphatase were most active in the alkaline pH range, and other enzymes were most active in the acidic pH range. These enzymes showed a high association with cell wall material, and the release of enzymes from the cells into the culture fluid appeared to occur only when the cells were undergoing autolysis. Alkaline phosphatase in C. graniforme is a constitutive enzyme, and examination of the alkaline phosphatase following a purification of 265-fold produced the following characteristics: pH optimum of 9.5, M(r) of 60,000, K(m) of 2.1 x 10 M for p-nitrophenylphosphate, and activation energy for hydrolysis of the substrate at 9.9 kcal (1 cal = 4.184 J)/mol.
