[Left ventricular diastolic function in patients with bronchial asthma].

The aim of the study was to evaluate the diastolic function of the left ventricle (LV) in patients with bronchial asthma (BA) in relation to the phase and severity of the disease. Seventy four patients with BA, including 16 with mild persistent BA, 39 with moderate BA, and 19 with severe BA were examined in different phases of the disease. Echocardiography and Doppler study of intracardiac blood flow were performed and external respiratory function was studied. Thirty six healthy persons matched by age, gender, body weight, and blood pressure values made up a control group. Most characteristics of LV diastolic relaxation were found to become much worse in BA, which is due to bronchial obstruction, hypoxemia, dilatation, hypertrophy, and altered diastolic filling of the right ventricle (RV). LV diastolic filling improved when an exacerbation of BA attenuated. Worsening BA severity was ascertained to lead to decreased early LV filling, which is associated with enhanced bronchial obstruction, progressive RV hypertrophy and dilation, concentrated LV remodeling. The most likely mechanism of impaired transmitral blood flow in progressive BA is the impaired LV and RV interaction and pulmonary hemodynamic rearrangement. The revealed LV diastolic dysfunction in patients with BA of various severity and in different phases of the disease were not found to depend on heart rate changes.

[Change in central hemodynamics in lymphoma patients].

AIM: To elucidate features of hemodynamics in patients with lymphoma and hemodynamic effects of polychemotherapy. MATERIAL AND METHODS: Parameters of central hemodynamics were studied echocardiographycally in 42 patients with lymphoma. RESULTS: In a definite rise in the levels of tumor progression markers, changes in the immunity, impaired hemopoiesis there was rearrangement of hemodynamics (development of tachycardia, trend to excentric remodeling of the left ventricle). The above changes in hemodynamics enhanced in conduction of polychemotherapy. CONCLUSION: To avoid cardiotoxicity of polychemotherapy, lymphoma patients should receive cardioprotectors.

[Altered left ventricular diastolic function in patients with lymphogranulomatosis].

Impaired elastic characteristics of the left heart ventricle and restructuring of its diastolic bloodflow were revealed in 28 patients with Hodgkin's disease by complex Doppler echocardiographic study of intracardiac hemodynamics. The most pronounced disorders of the diastolic function were detected in patients with pericardial involvement. More pronounced tumor intoxication in patients with mixed-cell variant of the tumor resulted in remodeling of the left ventricle, left atrial dysfunction, and restructuring of the diastolic bloodflow, which indicated combined impairment of the left ventricular diastolic relaxation and left atrial contractility. Chemotherapy augmented disorders in diastolic bloodflow, the increase in the number of polychemotherapy courses being associated with a trend to deterioration of left-ventricular relaxation. Hence, tumor intoxication and drug cardiotoxicity determine the formation of central hemodynamic disorders, one of whose principal components is diastolic dysfunction of the left heart ventricle.

[Efficacy of climate therapy in patients with chronic cardiac failure at a climate-balneological resort].

The aim of our study was to estimate clinical effects of spa therapy (ST) at Sochi-Macesta resort in patients with heart failure (HF). Spa therapy was given to 116 patients with HF functional class (FC) 0-II by NYHA classification. It is shown that ST leads to improvement of the patients' clinical status, reduction of HF FC (from 1.32 +/- 0.09 to 0.66 +/- 0.01, p < 0.001), to increased exercise tolerance (p < 0.05) and better 6-minute walk test results (from 460.3 +/- 11.4 m to 511.2 +/- 11.8 m, p < 0.01), higher myocardial coronary reserve, favourable shifts in blood pressure. ST also has a cardiotonic effect and corrects vegetative status, there was also antiatherogenic effect and an increase in blood coagulation potential. The received data allow us to conclude that ST can be an effective tool of medical rehabilitation in patients with initial stages of heart failure.
