The vanadium isotopic constitution of petroleum asphaltenes: La Luna Formation (Venezuela).

High-resolution mass spectrometry indicates that the isotopic abundance of 50V of the Late Cretaceous La Luna petroleum asphaltenes of marine origin (highly enriched with V > 2000 ppm) is higher by about 3.5% than that of the inorganic source (VOSO4 x 5H2O, Merck). We propose that the difference in the 50V/51V values between the La Luna petroleum asphaltenes and the inorganic source can be best ascribed to the biological processing of seawater vanadium. The fact that the V isotopic compositions of petroleum asphaltenes vary over a very narrow range (2.46-2.52) suggests essentially the same (or similar) and fixed biological source of vanadium.

Astrovirus infection among children with gastroenteritis in the city of Zaragoza, Spain.

The incidence of astrovirus infection in children less than 10 years of age with gastroenteritis in the city of Zaragoza, Spain, was analysed during a 12-month period. A total of 718 stool samples obtained from 534 children were examined. In 401 samples no routinely searched for pathogenic organism was detected; these specimens were then tested for the presence of astrovirus antigens. Astrovirus was detected in the samples of 15 (5.5%) patients, a detection rate similar to that recognised for Yersinia enterocolitica and Cryptosporidium spp. All children with confirmed astrovirus infection had diarrhoea (median duration, 9 days), 11 experienced loss of appetite, 5 vomiting, and 4 fever. The incidence of astrovirus infection reported here indicates that Spanish children suffering from gastroenteritis should be routinely screened for the presence of astrovirus.

[Morphologic course of prostatic hyperplasia].

OBJECTIVE: To find a morphologic model of the development of prostate hyperplasia. METHODS: Study of the transitional zone in 60 patients (30 with infravesical obstruction, 30 with no obstructive symptoms) and quantification of the involved surface, number of pure stromal and glandular-stromal nodes, node area for each of them, and non-nodular area of the transitional zone, correlating each parameter to age based on clinical status. RESULTS: The greater transitional zone area is seen in patients with obstruction: 1376.83 +/- 408.17 mm2 vs 321.39 +/- 151.49 mm2 in asymptomatic patients, mainly due to a higher number of glandular-stromal nodes (17 vs 2.2) and their size, with a correlation to age (p = 0.03). Moderate increases of non-nodular areas are also found. CONCLUSIONS: These findings suggest that onset of prostate hyperplasia may be due to a consistent increase of the transitional area, and that in some patients, probably because of local factors, nodular development occurs as a result of both an increase in nodes number and size.

Increased plasma levels of corticosterone and prolactin and decreased T3 and T4 levels in short-term prehepatic portal hypertension in rats.

Corticosterone, T3, T4, and prolactin serum concentrations at 24 hr (N = 10), 15 days (N = 10), and 45 days (N = 10) of postoperative (postop) evolution were assayed to study the neuroendocrine response to portal hypertension. A triple stenosing ligature of the portal vein was used as the surgical technique of portal hypertension. This technique does not produce mortality and causes a decrease in the serum concentrations of T3 (0.043 +/- 0.009 vs 0.55 +/- 0.08 ng/ml) and T4 (3.93 +/- 0.55 vs 4.65 +/- 0.67 microg/ml) and an increase in those of prolactin (28.61 +/- 20.20 vs 12.84 +/- 3.96 ng/ml) and corticosterone (397.50 +/- 64.17 vs 311.53 +/- 57.41 ng/ml) at 45 days postop. The T3, T4, prolactin, and corticosterone alterations are associated with a persistent increase of TNF-alpha and NO, whose serum concentrations at 45 days postop are, respectively, 1838.33 +/- 247.07 vs 48.89 +/- 8.75 pg/ml and 0.43 +/- 0.13 vs 0.19 +/- 0.01 mmol/ml. TNF-alpha and NO could mediate these hormonal alterations in the evolution of short-term portal hypertension in the rat; thus they are involved in the systemic neuroendocrine response that is induced by this injury.

Isolation and characterization of a cysteine protease from the latex of Araujia hortorum fruits.

A new protease (araujiain h I) was purified to mass spectroscopy homogeneity from the latex of Araujia hortorum Fourn. (Asclepiadaceae) fruits by ultracentrifugation and ion exchange chromatography. The enzyme has a molecular mass of 24,031 (mass spectrometry) and an iso-electric point higher than 9.3. The optimum pH range for casein hydrolysis was 8.0-9.5. The enzyme showed remarkable caseinolytic activity at high temperatures, although its thermal stability decayed rapidly. The proteinase was activated by thiol compounds and inhibited by common thiol-blocking reagents, particularly E-64 and HgCl2, suggesting the enzyme belongs to the cysteine protease family. The concentration of active sites as determined by titration with E-64 was 3.3 microM. When assayed on N-alpha-CBZ-amino acid-p-nitrophenyl esters, the enzyme showed higher preference for the glutamine derivative, followed by those of alanine, asparagine, glycine, and leucine, in decreasing order. Partial homology (36-48%) with other plant cysteine proteinases was observed in an internal fragment obtained by Protease V8 treatment.

[Keratitis caused by Acanthamoeba spp.: presentation of 10 cases].

OBJECTIVE: To describe the clinical and microbiological characteristics of ten patients with Acanthamoeba spp. keratitis. METHODS: A retrospective review was made of the clinical charts of ten patients with both clinical keratitis and positive culture for Acanthamoeba spp. from 1992 (date of first diagnosed case in our hospital) on. Corneal tissue was cultured in Page media and also the contact lenses and maintenance or cleaning fluids used by each patient. RESULTS: The earliest clinical symptom observed in 60% of cases was foreign-body sensation. Before the parasitologic studies all the patients received different treatments for 4 days minimum to a maximum of 3 months. On ophthalmological examination the findings were: radial perineuritis in 4 cases, anular infiltrate in two, keratitis punctata in two and pseudodendritic ulcer in two more. Treatment employed was the following: propamidine and neomycin in 8 patients (four of them also received ketoconazole); biguanides in two. After a mean observation time of 7 months (minimum 4, maximum 10) the clinical status was favorable in 70% of cases. In three patients with delayed diagnosis of 2 to 3 months a keratoplasty was needed. COMMENTS: Acanthamoeba spp. keratitis is a process of difficult diagnosis and treatment. For early detection of this disease a narrow collaboration with the microbiologist is mandatory because the prognosis in this patients depends on the precocity of treatment.

Genetic effects observed in tetrads of Tradescantia induced by radon.

Inflorescences of Tradescantia clone 4430 were exposed to different concentrations of radon (Rn) gas (0.85, 12.10, 36.50 and 98.16 kBq/m3) from plants placed in an acrylic chamber that received radon from a container with pitchblende (containing uranium mineral). The exposure time was 24 h, afterwards the plants were left for 6 h in water and constantly aerated. Positive control plants were irradiated with gamma rays (0.8 Gy) and negative control plants received ambient air only (the background measurement had a mean of 0.38 kBq/m3). Micronuclei (MCN) in the tetrads induced by alpha particles emitted from Rn were tabulated and a linear concentration response was obtained. The potency of radon to induce MCN from the slope of the regression line was 0.13 MCN/kBq/m3 of alpha-radiation. Radon could reach the anthers by diffusion through the aerial spaces within the buds.

Difference between observed and predicted length of stay as an indicator of inpatient care inefficiency.

OBJECTIVES: To evaluate the performance of the difference between observed and predicted length of stay (OLOS-PLOS) as an inefficiency of care indicator for inpatients. SETTING: The Internal Medicine and the General Surgery departments of Hermanos Ameijeiras Hospital in Havana. DESIGN AND STUDY PARTICIPANTS: Two sets of clinical histories were needed for each department: one for deriving the predictive equation and another to validate it. The equation was a linear multiple regression model which included variables recognized as affecting length of stay. The validation group of histories was thoroughly examined and separated into two groups: (i) adequate efficiency or mild problems and (ii) inefficiencies considered to be moderate or severe. This classification was the gold standard to obtain a receiver operating characteristic (ROC) curve for the indicator. RESULTS: The function explained 41% of the total variation for Internal Medicine and 70% for General Surgery. The indicator's mean difference between the two validation groups of histories was around 10 days for both departments. The areas under the ROC curve were 0.80 for Internal Medicine and 0.88 for General Surgery. Sensitivity and specificity > 0.7 for detecting inefficiencies of care are achieved with a cut off point of 2 days for Internal Medicine and 1 day for General Surgery. CONCLUSIONS: The use of predictive equations might be quite useful for detecting efficiency problems in inpatient health care.

Visualization of hydatid elements: comparison of several techniques.

Some techniques available at our laboratory were tested for their ability to aid in the morphological diagnosis of hydatid elements (Echinococcus granulosus ["Taenia echinococcus"]) isolated from cysts in humans and sheep. Unstained, methanol-fixed hooklets were fluorescent, most starkly so under violet light (excitation filter wavelength, 405 nm; long-pass filter wavelength, 495 nm). Auramine-rhodamine and Gram procedures failed to stain hooklets. Ziehl-Neelsen stain yielded indifferent results when organisms were viewed under transmitted light but resulted in a surprisingly intense red fluorescence when organisms were viewed under green light (excitation, 546 nm; long pass, 590 nm). Wheatley trichrome stain gave better and more uniform results than fuchsin. Ryan trichrome blue stain was the best under transmitted light; hooklets stained uniformly and intensely and were easily distinguishable from the background. Very satisfactory results were also obtained with a much simpler procedure (modified Baxby technique: no fixation, steaming hot 1% safranin for 2 min, and malachite green for 30 s). Therefore, Ryan and modified Baxby stains are recommended for the examination of E. granulosus under transmitted light. For fluorescence microscopy, Ziehl-Neelsen stain under green excitation light, or violet light with no staining, is also very useful. Epifluorescence microscopy is especially convenient for examining samples concentrated by filtration, as it renders the filter pores inconspicuous.

Assessment of colour vision in epileptic patients exposed to single-drug therapy.

Diplopia, blurred vision and colour disturbances are well-known side effects associated with anti-epileptic drugs (AEDs). Farnsworth-Munsell 100-hue colour test (F-100) is an accepted and sensitive tool to detect changes in colour perception. To determine the impact of AEDs upon colour vision, we evaluated 37 consecutive patients with complex partial seizures exposed to monotherapy with phenytoin (PHT, carbamazepine (CBZ) or valproic acid (VPA). All had normal IQ and no congenital disturbances in colour vision or ocular diseases. Twenty normal controls were used for statistical analysis. Thirteen patients were exposed to PHT, 12 to CBZ and 12 to VPA. Visual colour perception was impaired in 30/37 (82%) of the study group. The most significant abnormality was detected in the blue-yellow axis in 10/13 patients exposed to PHT (p < 0.02) and in 8/12 treated with CBZ (p < 0.009). In 8/12 patients taking VPA, no significant abnormality was observed (p < 0.06). None of the studied patients complained of colour vision disturbances. Our findings strongly support the negative effect of AEDs upon colour vision discrimination, most likely due to changes at the retinal processing level. F-100 proved to be very useful to assess early toxicity due to AEDs.

Hepatic cytochrome oxidase in rats with microsurgical cholestasis or portocaval shunt.

AIMS: portocaval shunt and extrahepatic cholestasis are experimental models of chronic hepatic insufficiency of different etiology and histological characteristics, and which probably also differ in the mechanism of impairment of oxidative metabolism. To test this hypothesis we measured hepatic cytochrome oxidase. METHODS: cytochrome oxidase was assayed with a histochemical technique in three groups of Wistar rats: A (n = 8) control; B (n = 8) microsurgical extrahepatic cholestasis; and C (n = 8) end-to-side portocaval shunt. RESULTS: cytochrome oxidase activity was lowest in group B, both in the left middle (p = 0.00019) and in the inferior caudate (p = 0.00014) hepatic lobes, and was highest in group C in both hepatic lobes, especially in the left middle lobe (p = 0.0029). CONCLUSION: the decrease in cytochrome oxidase activity in the liver of rats with extrahepatic cholestasis and the increase in animals subjected to portal flow deprivation demonstrate the different nature of the impairment in hepatic oxidative metabolism in these two pathological conditions.

Use of a selective medium and a membrane filter method for isolation of Campylobacter species from Spanish paediatric patients.

A study was conducted to assess the value of a combination of two culture methods for isolation of Campylobacter spp. from Spanish children. Seven hundred twenty-nine diarrhoeal stool specimens from 599 patients were examined for Campylobacter spp. by culturing them on charcoal cefoperazone deoxycholate agar and on blood agar with a membrane filter. One hundred sixteen Campylobacter strains were isolated from a total of 108 specimens; 75 (64.6%) were Campylobacter jejuni, 32 (27.5%) were Campylobacter coli, 8 (6.8%) were non-typeable, and one (0.9%) was Campylobacter upsaliensis. Campylobacters were isolated from 99 positive samples using charcoal cefoperazone deoxycholate agar alone. The filtration technique alone yielded only 86 positive samples. Seven specimens yielded different Campylobacter spp. with different media. The only catalase-negative strain was recovered using the filter method. The combination of the selective medium with the filter method increased the isolation rate of Campylobacter strains by 14.1%. Isolation rates of campylobacters using the filter method were similar to those reported in European studies, in which a similar frequency of Campylobacter upsaliensis was observed. The addition of a filter method for routine laboratory isolation of campylobacters should be considered in selected age groups (in children < 10 years of age) or in areas where catalase-negative or weakly-positive Campylobacter strains may be of epidemiological significance.

Intraventricular ganglioglioma: case report.

We report a case of ganglioglioma located in the body and frontal horn of the right lateral ventricle. The lesion was found in a 71-year-old man who suffered from recurrent episodes of right hemicranial headache. CT and MRI showed a heterogeneous intraventricular tumour with multiple microcysts that suggested a diagnosis of subependymoma or central neurocytoma, but the pathological findings were consistent with ganglioglioma. Although the most frequent intracranial location of ganglioglioma is the temporal lobe, any location in the central nervous system may be seen. Nevertheless an exclusively intraventricular location is exceptional. To our knowledge this is the first complete radiological report of a purely intraventricular ganglioglioma.

[Quinolone resistance in Salmonella enterica].

Nalidixic acid resistance is a marker of quinolone sensibility in Salmonella spp. In our laboratory we observed an increase of nalidixic acid resistant strains from 0.44% in 1990 to 13.7% in 1996. The purpose of this work was to study the evolution of nalidixic acid resistance in Salmonella spp., as well as the association with certain serotypes and the cross-resistance to others agents. A total of 1,782 isolates of Salmonella enterica were studied from fecal and blood sources over a 7-year period; 45 strains (2.52%) were resistant to nalidixic acid (MIC > 16 microgram/ml). Two isolates required 2 microgram/ml of ciprofloxacin for inhibition, while the remaining 43 strains required 1 microgram/ml. The most frequent serotypes in nalidixic acid resistant strains were the following: enteritidis (33.3%), hadar (31.11%) and virchow (15.5%). In 1996, enteritidis was the predominant serotype (13/31), and hadar serotype was found to have the greatest nalidixic acid resistance at 80% of strains. No differences of sensibility to other agents were detected between the nalidixic acid resistant group and nalidixic acid sensitive strains. This type of resistance appears regularly in uncommon serotypes such as hadar. The increasing incidence of resistant isolates in enteritidis in 1996 is of even greater concern.

Changes of cytochrome oxidase activity in rat suprachiasmatic nucleus.

This paper evaluates the changes of cytochrome oxidase (CO) activity that take place in the suprachiasmatic nucleus (SCN) during the light-dark cycle. CO is a mitochondrial energy-generating enzyme used as a marker of neural oxidative metabolism. We measured CO activity using quantitative histochemistry calibrated with brain tissue standards and a computerized analysis image system. The results indicate that the CO enzyme activity changes on the basis of a circadian pattern, with the higher levels during the light phase (P < 0.0001). These changes are detected over a period of hours, in accordance with other studies on the possible short-term regulation of CO activity in the nervous system. It is, therefore, possible to apply this methodology to the study of the SCN and other brain areas which show functional rhythmicity.
