The serum concentrations of fazadinium and its metabolites were measured in 14 surgical patients with end-stage renal failure and in 11 patients free from kidney disease undergoing abdominal surgery. A two-compartment open model was used in the pharmacokinetic analysis of the data. The elimination half-life (T 1/2 beta) was prolonged by 60% in patients with renal failure (from 85 to 140 min) and corresponded to a 30% decrease of the plasma clearance. The plasma concentration of the metabolites never exceeded 7% of the unchanged fazadinium. These results suggest that the duration of action will be less prolonged for fazadinium than for other non-depolarizing neuromuscular blocking drugs in patients with renal failure. A supplementary biliary pathway appears to be a possible explanation for the rapid elimination of fazadinium, even in patients with renal failure.
An attempt of sedative procedures associated with a locoregional anesthesia was performed on fifty old patients before cataract operation. The blood gases, the blood pressure and the heartrate were measured and the behaviour of the patient just before he awoke was examined to determinate the sedative value. A parallel study revealed a decrease of the intra ocular pressure of 2,5 mm d'Hg due to a slight decrease of the blood pressure and to the effect of the neurolep mixture: the Innovar. This method seems to be very satisfactory in Ophthalmology.

In Rabbit carotid sinus, the presence of sympathetic nerve endings capable of releasing noradrenaline has been demonstrated. The release of NA in response to sympathetic nerve stimulation was decreased by PgE2 and a precursor of Pg (arachidonic acid) but was strongly increased by an inhibitor of Pg biosynthesis (indomethacin). The experiments carried out demonstrated that freshly synthesized Pg acts in the same way as exogenous Pg and suggested that Pg could have a regulating effect on adrenergic neurotransmission in carotid sinus. The role of this regulating mechanism in the physiology of carotid sinus has been discussed.
A study of the effects of dopamine, dobutamine and of isoprenaline was carried out in 20 patients with chronic respiratory insufficiency, 16 of whom had pulmonary arterial hypertension (PAP: 22 mmHg). The following findings emerged: --absence of any significant effect of dobutamine on heart rate,--increase in systolic and mean PAP, after infusion of dobutamine,--increase in systolic and mean PAP, after infusion of dopamine,--decrease in BP after administration of infusion of dobutamine,--decrease in pulmonary arterial resistances under the influence of isoprenaline,--decrease in total systemic resistances after the administration of dobutamine,--increase in ventricular, systolic work after the infusion of dopamine.

The authors present six cases of difficult intubation and review the literature on this subject; The circumstances during which difficult intubation may be feared are recalled; some are due to the morphology of the subject, the others to his previous pathology which may be rheumatological or dermatological. The authors then discuss the method of anaesthesia for this unusual intubation and the various technical means proposed to carry out safely the intubation itself.

This report considers interference between medications for, on the one hand, drugs given during pregnancy, and, on the other hand, drugs given to the pregnant women and anaesthetic agents. Consideration is given to progestational agents, oxytocics, beta-mimetics, corticosteroids, insulin, hypotensive agents, diuretics and psychiatric drugs. For each case of interference, an attempt is made to provide practical data, with particular respect to those combinations frequently prescribed by obstetricians, e.g. the combination of the beta-mimetics and corticosteroids with the aim of preventing impending premature onset of labour and to ensure foetal pulmonary maturation; the combination of insulin and corticosteroids given to diabetics with the aim of preventing hyaline membrane disease; the combination of corticosteroids with anti-histamines in the treatment of rhesus disease; the association of oxytocics with large quantities of intravenous fluid in the case of post-partum haemorrhage and hypontensive medication combinations and the problems which may result with emergency anaesthesis.

Results of our study of 91 cases show that many indications of ventilation at home are debatable:--Tracheostomized patients submitted to continuous ventilation with short disconnected periods;--Patients using the respiratory apparatus more than 4--6 hours per day, through a mouth-piece. The existence of a reserve closely related to the intensive care department and mobile emergency unit (S.A.M.U.) facilitates supervision and eventual technical or medical assistance. In addition, the running cost remains small because of the adaptability of the system. However, it seems of interest to point out a group of 11 patients whose treatment was interrupted either because of their improvement (physical therapy, suppression of tobacco mainly), inadequate indications, or poor cooperation.
