Acid phosphatase patterns in microfilariae of Onchocerca volvulus s.l. from the Upper Orinoco Basin, Venezuela.

The patterns of acid phosphatase in strains of Onchocerca volvulus s.l. which parasitize an Amerindian population (Yanomami) in Venezuela's Upper Orinoco Basin were examined by using the naphthol AS-TR phosphate method. The study sample consisted of 40 Yanomami inhabiting a savannah area at 950 m above sea level and 21 Yanomami residents of a tropical rainforest area at an altitude of 250 m. Stained intrauterine microfilariae, still within the egg case, exhibited a diffuse distribution of the enzyme in the early stages of embryonic development and a negative reaction at a more developed stage. Four of the five enzyme staining patterns described by Omar (1978) were found in the 3157 microfilariae examined from skin snips. Their distribution was: Type I--17.2%, Type III--0.5%, Type IV--75.6% and Type V--6.6%. No examples of Type II were observed. The results indicate that acid phosphatase patterns of the Upper Orinoco Onchocerca strain most resemble those of strains from Guatemala and Yemen, and are different from the African strains found in Upper Volta and Liberia. The relative frequency of acid phosphatase patterns was modified by cryopreservation of microfilariae.

Permanent wear of soft contact lenses in aphakic eyes.

In a five-year study, permanent wear of soft contact lenses was achieved by selecting patients and lenses properly, and offered a viable and probably safer alternative to intraocular lenses. Twenty-five of these 49 patients wore their initial lens or a similar lens continuously for over three years. A visual acuity of 6/12 (20/40) or better was obtained in 45 (91.8%) patients. Four (8%) had ocular problems not related to contact lenses that precluded the same improvement in central vision. The most common complications were reversible epithelial edema, superficial vascularization, and conjunctivitis. There was a strong correlation between edema and superficial vascularization in eyes fitted with lenses having a poorly made inside bevel. Calcium deposits were the most frequently found spot on permanent-wear soft contact lenses (nine eyes, 18%).

Simplified soft contact lens treatment in corneal diseases.

The advantages and disadvantages of the newly introduced Plano-T soft contact lens for the treatment of corneal diseases were evaluated. The physical characteristics, base curve, diameter, and thickness account for the fitting advantages which make it a versatile lens. The assumption that one lens fits almost any eye and that it may be used in most instances where a bandage lens is indicated proved correct, except for corneas of extreme measurements as in keratoconus. The results obtained compare very favorably with the results of the more conventional lathe cut lenses for the treatment of corneal diseases. Visual improvement does not seem as dramatic as in the latter, but some of the most common side effects are almost eliminated. Its physical characteristics and actions are reviewed. The corneal diseases for which it has been used are mentioned and the results discussed.

Simultaneous (combined) operation for glaucoma with cataract.

Henceforth we mention the observations of one of the authors (E.G.) which were made in 1949 and published in 1956. The author recommends iridectomy of one pillar, intracapsular extraction of the cataract and iridenclesis of the other pillar for the treatment of cases with glaucoma together with cataract. Between the years 1968 and 1976 in the Department of Ophthalmology of the Hospital Universitario de Caracas, 966 cataract extractions were performed; within these, 84 were performed in glaucomatous eyes. In 17 cases the procedure that is used today was performed: iridectomy of one pillar, sphincterotomy, intracapsular extraction and iridencleisis of the other pillar. In other cases, other procedures were used. From our observations we consider that this technique is the most recommended in cases of glaucoma together with cataract.

[The effect of Actihaemyl on epithelial regeneration (author's transl)].

After well-defined chemical and mechanical removal of the corneal epithelium of 40 rabbit eyes 20 eyes were treated with Actihaemyl-Augengel 4 times daily. The epithelial regeneration was compared with the untreated contralateral eye. Under Actihaemyl treatment the healing rate was not accelerated. The well-known healing effect of Actihaemyl on different corneal diseases does not seem to come from improving the epithelial regeneration but is probably the result of a general improvement of corneal metabolism.
