[Testicular varicocele. Reason for surgery and recurrence frequency].

In the period 1979-1989, 63 patients were operated on for testicular varicocele. The study was a retrospective follow-up from case records and from a questionnaire. Thirty-eight had local complaints, 19 were operated on because of infertility and six had an asymptomatic varicocele. 10% had minor, reversible complications. 92% answered the questionnaire and 38% had a personal contact. 21% had recurrence of the varicocele although to a minor degree. In the infertility group, 42% succeeded in conception after operation.

Adolescent

Treatment of hydrocele testis with aspiration and injection of polidocanol.

Hydroceles of the testis were treated by aspiration and injection of the sclerosant solution polidocanol in 20 patients. Three patients had recurrences but otherwise the complication rate was nil. Injection of polidocanol is a promising treatment that should be investigated further for primary therapy of hydrocele of the testis. In contrast to some other agents this drug does not cause pain during or after the injection.

Surgical treatment of acute malignant large bowel obstruction.

OBJECTIVE: To evaluate the morbidity and mortality in all patients operated on urgently for acute large bowel obstruction caused by carcinoma of the colon or rectum during a 10 year period. DESIGN: Retrospective study. SETTING: Aalborg Hospital, Denmark. SUBJECTS: 156 consecutive patients operated on for obstructing primary colorectal cancers. MAIN OUTCOME MEASURES: Operations done, morbidity and mortality. RESULTS: 95 patients (61%) had advanced disease (Dukes' stage C or "D") and their median age was 73 years (range 38-93). 97 had the obstructing lesion resected with a 30 day mortality of 5%. 43 patients underwent primary resection and 4 died (9%), and 54 underwent staged resection with one death (2%). Complications were common, particularly after staged resections, median hospital stay being 19 days after primary, compared with 30 days after staged resection. 59 patients (38%) had palliative operations with 29 deaths (49%); in 39 the tumor was completely unresectable. CONCLUSION: Patients with obstructing primary colorectal cancers are a high risk group who are characterised by advanced disease and old age. Only prospective trials comparing different operations can assess whether it is possible to achieve a reduction in mortality.

[The role of oat bran in the treatment of hypercholesterolemia].

Review of published studies suggests that high dietary content of soluble fibre can reduce serum cholesterol by up to 17%. The reduction may be proportional to the degree of hypercholesterolemia before the change in diet. The mechanism is not fully elucidated. It may be analogous to the effect of anion-binding-resins on the metabolism of bile acids and cholesterol but it may also be due to partial substitution of soluble fibre for dietary saturated fat which causes elevation in serum cholesterol.

Plasma levels of carbamazepine and carbamazepine-10,11-epoxide during treatment of epilepsy.

Carbamazepine and its epoxide in plasma were measured by liquid chromatography in 25 patients treated with a mean dose of carbamazepine of 12.5 +/- 3.3 mg/kg body weight. The mean concentrations of parent drug and metabolite were 5.4 +/- 2.5 mug/ml and 1.10 +/- 0.42 mug/ml, respectively. A singificant correlation was found between the plasma concentrations of the two compounds (r = 0.64; p less than 0.001), but marked interindividual variation existed in the ratio of carbamazepine to carbamazepine to epoxide. Based on simultaneous measurements in plasma and cerebrospinal fluid, the unbound fraction of carbamazepine in plasma was of the order of 20% as compared to 45% for the epoxide. Thirteen ambulant patients suffering from partial epilepsy with complex symptomatology, who were already being treated with phenytoin in optimal doses (plasma level 14-20 mug/ml) were also given carbamazepine. At plasma levels of the latter of about 5 mug/ml there was no further reduction in the frequency of partial or generalized epileptic seizures. In five patients the dose was increased to produce plasma concentrations of 7 - 8 mug/ml. There was still no improvement and side-effects were seen in three patients.

Carbamazepine
