Congenital intramedullary tumor with neonatal manifestations.

The authors report on a 45-day-old boy with a congenital intramedullary tumor with clinical manifestations since birth. Neurologic examination disclosed severe bilateral lower-limb hypotonia and diplegia, with exacerbated deep tendon reflexes and clonus associated with severe pain at manipulation. Further evaluation of this patient included screening for infections, computed tomographic scan, myelography, and somatosensory evoked potentials. The definite diagnosis was a solid holocord astrocytoma. This report discusses a rare disorder in the neonatal period and makes other medical practitioners aware of this diagnosis. Previously published literature is also reviewed.

[Cerebrovascular disorders: analysis of 190 autopsy cases].

A retrospective study of 190 postmortem examinations of cerebrovascular disease from the Department of Pathology was carried out. We found 94 cases (49.47%) of hemorrhage, 67 (35.26%) of infarction and 29 patients (15.26%) with hemorrhage and infarction; both were more frequent in males. Among the hemorrhagic cerebral vascular disease the intracerebral hemorrhage was more frequent. The cerebral infarction was more frequent in the territory of the middle cerebral artery. Arterial hypertension was the most common cause of cerebral hemorrhage and the cause in the great majority of the cerebral infarction cases was atherosclerosis.

Actinomycetoma caused by Rhodococcus spp.

We report a case of mycetoma caused by Rhodococcus in a 62-year-old man who presented with multiple draining sinuses of the left foot. Biopsy specimen showed granulomatous reaction and microabcesses contained granules. These granules were composed by rod and coccoid Gram-positive and partially acid-fast elements. Culture grew a 'Nocardia-like organism', confirmed at the Center for Disease Control as Rhodococcus spp.

[Neurocutaneous melanosis: autopsy study of a case].

The clinical, radiological and autopsy features of a 5-month-old child with neurocutaneous melanosis are described. The patient had multiple disseminated benign cutaneous nevi. The pneumoencephalogram showed non-obstructive hydrocephalus. The patient died at the third day of hospitalization and autopsy was performed. A mild hydrocephalus and melanic specks in cerebellum and in the brainstem were disclosed.

In vivo specific uptake of labeled insulin by turtle (Chrysemys dorbigni) thyroid gland.

Insulin labeled with 125I was injected into turtles to study its specific uptake by the thyroid gland. The gel filtration behavior of labeled material in blood and thyroid gland was determined in order to ascertain if the uptake is specific. Most animals were pretreated with KI to saturate the gland with iodide. Maximum specific uptake of radioactivity by the thyroid gland was only detected in animals pretreated with KI. A significant dose-related reduction (ED50 = 0.5 micrograms/kg) was observed when unlabeled insulin was administered simultaneously with 125I-insulin. Prolactin, glucagon and growth hormone (2 mg/kg) did not affect 125I-insulin uptake. Most of the radioactive material extracted from the turtle thyroid 15 min after 125I-insulin injection coeluted with 125I-insulin on Sephadex G-50. This peak decreased as a function of time after 125I-insulin administration. Similar elution patterns were found for thyroid extracts from turtles previously treated with KI. The labeled hormone in the gland was rapidly degraded or processed to both higher and lower molecular weight compounds. Prior administration of KI suppressed the former, whereas when unlabeled insulin was injected simultaneously with 125I-insulin the amount of degradation products was reduced. The demonstration of radioactive degradation products is consistent with the intracellular receptor-mediated degradation hypothesis. These findings indicate the presence of specific insulin-binding sites in the thyroid gland.

[Multiple brain histoplasmomas: autopsy study of a case].

Report of the case of a 28-years-old black woman who developed changes in behavior followed by neurological alterations. The CT scans of the brain revealed multiple calcified granulomas. The patient died and the autopsy showed granulomatous lesions with central caseation in the brain. Microscopical examination disclosed numerous Histoplasma capsulatum. This is the first case of histoplasmoma of the brain described in Brazil.

[Intracranial tumors in childhood. Study of 106 cases].

The authors analyze 106 intracranial tumors occurring in patients from 0 to 15 years of age. This study was carried on during the period from 1962 up to 1977. In 85 cases the diagnostic work up was made on clinical, radiological, and histological grounds; the remaining 21 cases had their diagnosis based solely on clinical and radiological evidence. Observations involving incidence according to age, sex, localization and histological types are made. The signs and symptoms and supplementary examination are discussed. The frequency of primary intracranial tumors in children under 1 year of age is stressed.

[Metastasis of breast carcinoma to the pituitary. Report of a case].

The case of a 65 old woman who had a rapidly progressive neurologic symtomatology is reported. The autopsy disclosed a carcinoma of the breast with metastases in brain, cerebellum, brainstem, pituitary, thyroid gland and the adrenals. The frequency of the pituitary metastases is discussed, especially in cases of breast cancer. The macro and microscopic aspects of the metastatic pituitary gland are studied.

[Bilateral cerebral mycotic aneurysm in a child. Report of a case and review of the literature].

The case of a 6 year-old boy, who was hospitalized with the diagnosis of purlent meningits is reported. The CSF examination disclosed pleocytosis and the antibioticotherapy was instituded. The bilateral carotid angiography demonstrated an arterial aneurysm of the frontal ascendent artery with hematoma in the left side and another in the posterior temporal artery, in the right side. The patient was operated on because of the hematoma, in the left side. The condition of the patient in the post-operatory period deteriorated and the death ocurred. The post-mortem examination disclosed a sub-arachnoidal hemorrhage, fronto-temporal hematoma in the left side and a para-capsular hematoma in the right side. The histological examination of the central part of the right side. The histological examination of the central part of the hematoma revealed a dilated arterial walls with a supurative inflammatory process.

[Metastatic carcinoma involving the cauda equina].

The case of a 60 year-old man who had an indifferenciated carcinoma in the lung is reported. He had received cobaltotherapy and had improved. After 3 months a lumbar pain had began and the patient was hospitalized. A myelography with lipiodol demonstrated and intra-dural mass. At operation a big intra-dural tumor including the cauda equina was found. The microscopic examination revealed an undifferenciated carcinoma, that inflitrated the epi and peri-nerual space.

[Meningo-encephalic tuberculosis in childhood. Anatomo pathological study of 10 cases].

The anatomo-pathological study of ten cases of tuberculosis of the central nervous system (CNS) in children under 10 years old is reported. The emphasis is given to the macroscopic and microscopic changes produced by the tuberculosis in the meninges, in the nervous parenchyma and in the vessels. The clinical and pathological findings are discussed and correlated with the literature. The authors concluded that: 1) the lesion in children with tuberculosis of the CNS is generally a proliferative meningoencephalitis; 2) the nervous parenchyma may be affected by the contiguous propagation of the tuberculous process or by the circulatory changes secondary to the arterial lesions; 3) because of the localization of the endocranial vessels in the sub-arachnoidal space they are generally injured by the inflammatory process, which can determine an obliterating endarteritis, which can be observed by a carotid angiography.
