[Determination of chlorphenamine and propantheline bromide in pukeling tablets by differential--linear combination derivative spectrophotometry].

This paper provides the basic principle and experimental technique of differential--linear combination equal value derivative spectrophotometry. Quantitative analysis of three or four component--mixture can be done without separation. This method was investigated to assay the contents of chlorphenamine and propantheline bromide in Pukeling tablets by using UV-210A spectrophotometer. The average recoveries of chlorphenamine and propantheline bromide were 100.2 +/- 1.49% (CV) and 99.89 +/- 1.03% (CV) respectively. The results showed that it not only can effectively remove disturbances from each other, but also give higher sensitivity and accuracy.

[Determination of trimethoprim and sulfamethoxazole in pharmaceutical preparations by non-relative component reference-multiplier derivative spectrophotometry].

This paper provides the basic principle and experimental technique of non-relative component reference-multiplier derivative spectrophotometry. The procedure overcomes the problem of overlapping in derivative spectrophotometry and quantitative analysis of lower content component in mixture can be done without separation. This method was investigated to assay the contents of trimethoprim (TMP) and used to assay the zero crossing derivative spectrophotometry for the assay of sulfamethoxazole (SMZ) in pharmaceutical preparations. The average recoveries of TMP and SMZ were 102.5 +/- 1.63% (CV) and 100.3 +/- 0.99% (CV) respectively. The results show that it not only can effectively remove the interference from each other, but also give a higher sensitivity and accuracy.

Case-control study on pathogenesis of pneumonia in children aged 0-2 years.

In order to know the incidence of risk factors predisposing children to pneumonia, case control study was carried out in six MCH Model Counties in 1986. Single factor analysis showed 29 factors were responsible for the increasing incidence of pneumonia. 13 of 29 factors were major pneumonia risk factors by the standard of means greater than 15 and OR greater than 3. They were malnutrition, anemia, riskets, pneumonia history, repeated colds, chronic diarrhoea, congenital malformation, asphyxia neonatorum, amniotic fluid aspiration, artificial feeding, too much clothing, family member with acute respiratory illness (ARI) and contact with ARI patients. Among them 7 factors were related to individual health condition. Therefore, it is important to improve general health of children so as to reduce the incidence of pneumonia.

Budd-Chiari syndrome. Surgical treatment of 45 cases.

Fourty-five patients with Budd-Chiari syndrome were treated surgically from December 1985 to May 1988. The procedures consisted of cavoatrial shunt, mesoatrial shunt, transcardiac membranotomy, splenopneumopexy, portal vein-systemic vein shunt and re-opened umbilical vein-right auricle shunt, etc. Two of the patients died postoperatively. Others showed improvement after follow-up for 3-24 months. Recurrent hemorrhage from varices did not occur after operation in all patients. The diagnosis, surgical indications, and various methods of surgery for Budd-Chiari syndrome were discussed.

[Immunohistochemical detection and ultrastructural features of solitary plasmacytoma of maxillary bone--case presentation].

Plasma cell neoplasm have been classified as multiple myeloma, solitary plasmacytoma and extramedullary plasmacytoma. The solitary plasmacytoma of maxilla is a rare condition. It is a single focus of myelomatous tissue with no dissemination to other parts of the skeleton. This paper presents a case of solitary plasmacytoma in the maxillary bone. Roentgenographic examination revealed a cystic-like osteolytic lesion over the left posterior portion of maxillary bone, invading maxillary sinus. The CT scan showed the tumor mass occupied the left maxillary sinus and lateral wall of the nasal cavity, protruding to the buccal side of the left face. The tumor cells were composed of densely packed round and polygonal cell structures which were scattered in relatively sparse stoma. The neoplastic cells have large, single eccentric nucleus, resembling typical plasma cells. The ultrastructural studies of tumor cells revealed numerous endoplasmic reticulum. There were arranged in a lamellar pattern, large numbers of mitochondria in perinuclear distribution and prominent Golgi complex. Nuclei showed patch condensation of chromatin and large nucleoli. The monoclonal staining (anti-Kappa and anti-IgG) of tumor cells by the peroxidase anti-peroxidase immunohistochemical technique, proved that the plasma cell lesion is neoplastic in nature. The tumor mass was eradicated by total resection, followed by X-ray radiation (4200 RADS). The patient is in good physical condition. There has been no clinical evidence of recurrence three years after surgery.

[Dark epithelial cells in oral premalignant lesions].

Dark basal epithelial cells (D cells) are characterized by a strong affinity for basic dyes and by the high electron density of the cytoplasm and nucleus. These cells have been considered to be putative dedifferentiated precursors of a neoplastic keratinocyte. They are significantly increased in preneoplastic lesions of the human airway and rat trachea induced with chemical carcinogens and/or tumor promoters. Twenty-four cases of oral premalignant lesions were classified as slight, moderate and severe dysplasia, and carcinoma in situ according to the histologic criteria proposed by MacDonald, katz and Pindborg. The percentage of appearance in D-cells of the dysplastic basal layer was calculated and shown as follows: in slight dysplasia 10.6 +/- 0.6%, in moderate dysplasia 22.9 +/- 0.8%, in severe dysplasia 23.9 +/- 0.4%, and in carcinoma in situ 31.4 +/- 0.9%. This result revealed that the increase in the number of D-cells in the basal epithelial layer was in proportion to the severity of the epithelial dysplasia. Thus, the D-cells probably can be used as an indicator in premalignant lesions of the oral cavity.

Aneurysmal bone cyst of the mandible--a case report.

Aneurysmal bone cysts originating in the craniofacial region have seldom been reported in Taiwan. A case of aneurysmal bone cyst arising in the right mandible of a 15-year-old Chinese boy was presented with both the roentgenographic and microscopic findings. In view of the high recurrent rate and the large destructive mandibular lesion, segmental resection with immediate bone grafting for reconstruction and a conscientious follow up regimen was employed. The etiology and pathogenesis of this interesting lesion remained unclear but several theories were reviewed and the differential diagnosis of this lesion was also discussed. Furthermore, immunoperoxidase staining for Factor VIII related antigen was performed to help differentiate between the aneurysmal bone cyst and a central hemangioma and then to elucidate the pathogenesis of the former.

Calcifying odontogenic cyst presenting as a lateral periodontal cyst--a case report.

We report a case of the calcifying odontogenic cyst occurring on the left anterior mandible of a 41-year-old male with clinical and radiographic features which made it initially to be a lateral periodontal cyst. This unusual presentation of a calcifying odontogenic cyst is discussed and its significance for the clinician is reviewed. A calcifying odontogenic cyst may present some problems for the differential diagnosis both clinically and histopathologically. The differential diagnosis of a lesion occurring in a lateral periodontal location should include the calcifying odontogenic cyst.

[Reattachment or new attachment in apical-marginal periodontal bone loss. A clinical-histological discussion].

Loss of marginal alveolar bone of the tooth can be caused by periodontal, pulpal-periapical, or a combined pulpal and periodontal disease. The regeneration is supported by the formation of reattachment or new attachment. Reattachment is probably the usual healing process in periapical surgery of the tooth with apicomarginal bone loss caused by pulpal-periapical disease. In animal experiments (rats, dogs) ist was well documented, that the cells which repopulate the periodontal wound adjacent to the root surface determine the nature of the attachment formed.

Cytokeratins in hamster cheek pouch epithelium during DMBA-induced carcinogenesis.

The pattern of keratin expression in hamster cheek pouch epithelium during 15-wk of DMBA-induced carcinogenesis was studied. The sequential changes in cytokeratins of premalignant and malignant tissues and comparative investigation of normal epithelial tissues were examined during a weekly sequential DMBA-induced chemical carcinogenesis. Keratin polypeptides of normal pouch epithelium appear in a molecular weight range of 43-67 kd and 5-6 proteins can be identified. The disappearance of high molecular weight keratin (61-67 kd) was observed from the 6-wk DMBA-treated premalignant group to the 15-wk DMBA-treated malignant group. An additional keratin polypeptide was noted initially on the 11th-wk-DMBA-treated group and remained to the 15th-wk-DMBA treated group.

Lectin binding patterns in squamous epithelium in experimentally induced hamster buccal pouch carcinoma.

The distribution pattern of certain monosaccharides in the epithelial cells of the hamster buccal pouch was studied during carcinoma development induced by 9,10-dimethyl-1,2-benzathrancene (DMBA). An avidin-biotin-peroxidase complex (ABC) immunohistochemical technique with high affinity biotinylated lectins was employed to identify monosaccharides. Lectins used in this experiment included Concanavalin A (Con A), for identifying mannose or glucose, Ricinus communis agglutinin I(RCA-I), for identifying galactose, and Ulex europaeus agglutinin I(UEA-I), for identifying fucose. The results show that in normal buccal pouch epithelial cells, fuctose or galactose were concentrated predominantly on the cellular membrane, while mannose and glucose were distributed in the cytoplasm. In the epithelial cells undergoing neoplastic transformation induced by DMBA, most cells showed decreased staining of the above-mentioned monosaccharides, while in other areas the cells were heavily stained. However, the most striking change which occurred was that galactose and fucose shifted from the cellular membrane to the intracytoplasmic area during the malignant transformation. Thus, the changes of anatomic location and intensity of staining of monosaccharides in the buccal pouch epithelium may be used as a criteria for early histochemical diagnosis of malignant transformation.

Antigenic detection and prognosis of patients with transitional cell carcinoma of the urinary bladder.

We used the specific red cell adherence (SRCA) technique to detect the A, B, O cell surface antigen on 61 transitional cell carcinomas of the urinary bladder seen from 1979 to 1982 at the Urology Department of Kaohsiung Medical College Hospital. It was suggested in this study that the loss of surface antigen had good correlation with both the pathological grade and clinical stage of tumors. On the other hand, the patients deprived of the surface antigen were poorer in prognosis than those with positive surface antigens. Among the four groups of A, B, O and AB patients, patients with blood group O had higher mortality rates, and more susceptibility to loss of the surface antigen. This study also demonstrated that the activation of surface antigen will reduce as the age increased. Of 11 patients with recurrent stage A tumors in this study, 8 had positive surface antigen and 3 had negative surface antigen. In 8 cases of positive surface antigen, 1 (13%) subsequently developed an invasive tumor. In contrast, 2 (67%) among the 3 negative surface antigens developed invasive tumors and the statistics showed significant differences (p less than 0.05). Consequently, the SRCA technique may be valuable for predicting malignant potential in early stage cancer of the bladder.

Vancomycin pharmacokinetics in infants: relationship to postconceptional age and serum creatinine.

Multidose pharmacokinetics of vancomycin were studied in 15 infants with gestational age less than 36 weeks and suspected or confirmed Staphylococcus epidermidis infections. Postconceptional age (PCA) at the time of the study ranged from 26 to 44 weeks. Vancomycin individual doses ranged from 6.7 to 10.6 mg/kg and were infused over 60 min. Five postinfusion samples were obtained in 13 infants, while 4 samples were obtained in 2 patients. Vancomycin pharmacokinetic parameters were determined by fitting the data to a two-compartment model using a weighted least-squares nonlinear regression method. Mean vancomycin body clearance (CL), volume of distribution (Vdss) and terminal elimination half-life were 1.37 ml/min, 0.58 liters and 5.6 h, respectively. When standardized for patient weight, the CL and Vdss values were 1.07 ml/min/kg and 0.48 liters/kg, respectively. The CL (ml/min/kg) was strongly inversely correlated with the serum creatinine (r = -0.82), while a weaker but significant association was noted with PCA (r = 0.41). These data suggest that in sick infants, in addition to the PCA, serum creatinine should be considered when determining the initial vancomycin dosing regimen.

"Otodental" dysplasia.

The case of a 3 11/12-year-old Chinese boy with the dental abnormalities of "otodental" dysplasia is reported. Hearing was normal. Dental anomalies consisted of delayed eruption of globe-shaped molars, bulbous deciduous canines, and double pulp chambers in the molars. Radiographs taken 4 years later showed taurodontic molars, supernumerary microdontic teeth, retarded formation of premolars, and probable aplasia of the mandibular second premolars.
