Genetic screening of donors for artificial insemination.

A routine has been established for genetic screening of donors in an artificial insemination and frozen sperm bank program. This report is a summary and analysis of the information obtained on the first 168 donor applicants and 89 recipients who were genetically screened. The specific forms for screening, family information obtained, characteristics of the donor and recipient groups, and guidelines for acceptance or rejection of donors are discussed. The donor and recipient often failed to perceive that the disorders in the family were genetic. The simple question of whether or not there were genetic or hereditary problems in the family was ineffective, even when the donor or recipient had formal medical training.

Structural and functional regression of polycystic ovaries by danazol.

The effects of danazol on ovarian steroidogenesis and follicular size was studied in androgen-sterilized rats bearing polycystic ovaries. Daily treatment with 4 or 20 mg of the drug caused a reduction in ovarian weight and regression of cystic follicles. Serum levels of luteinizing hormone (LH), testosterone, and estrogen were significantly decreased. The persistent estrus smears were changed to diestrus, indicating inhibition of estrogenic activity.

A clinical clerkship in a multicenter setting: an appraisal.

This paper describes the organization and evaluation of a third-year clerkship in obstetrics and gynecology conducted in a multiple-hospital setting. No difference was detected in objective test performance of students based on the site of the clerkship or in the subjective evaluation of students by faculty in the community hospitals. Students do spend their time in the various hospitals differently, but this seems more attributable to the individuality of specific hospitals than to the university-community dichotomy. PG 1 performance of graduates of the program compares favorably with that of graduates of other schools. It was concluded that community hospitals can carry out their primary responsibility of patient care while making an effective contribution to a university teaching program.

Effect of clomiphene citrate on corticosteroid-binding globulin and serum progesterone levels during ovulation induction.

Mid-luteal phase serum progesterone, cortisol, and corticosteroid-binding globulin concentrations were measured in 46 conception cycles and in 19 ovulatory, non-conception cycles in patients with spontaneous ovulation and following ovulation induction with clomiphene citrate (Clomid) or human menopausal gonadotropin. Significant elevations (P less than 0.05) of serum corticosteroid-binding globulin were noted in patients under therapy with Clomid. This effect was not secondary to elevated serum estrogen levels. This alteration in serum binding protein levels appears to be secondary to a direct effect of Clomid on hepatic synthesis and may have significance in relation to progesterone binding and metabolism during ovulation induction with Clomid.

The effect of a low-dose oral contraceptive on serum testosterone levels in polycystic ovary disease.

A study was designed to determine the effect of a low-estrogen-dosage oral contraceptive on the serum concentration of testosterone-binding globulin (TBG) and total and unbound testosterone in patients with polycystic ovary disease (PCOD). Treatment with norethindrone 0.5 mg and ethinyl estradiol 0.035 mg (Modicon, Ortho) increased the TBG concentration and reduced the unbound serum testosterone concentration.

A systematic comparison of teaching hospital and remote-site clinical education.

This paper present a methodology for examining activities of medical students on multisite clinical clerkships to determine whether differences exist in the educational experience offered at various sites. Five criterion variables are explored: distribution of student activities, type or class of clinical conditions encountered by students, degree of "esoterism" of those conditions, type of student role, and flexibility of student role. A format for data collection, employing a specially designed activity recording pad, was developed as part of the study. Application of this method to a clerkship in obstetrics and gynecology documents the existence of systematic differences in the educational experiences offered at different sites, particularly with regard to type of activities undertaken and conditions encountered. Most notably, the results suggest that it is fallacious to dichotomize clerkship sites as "academic" or "community" based. It is found that community hospitals themselves can differ markedly and offer an experience paralleling that of the academic referral center.

Maternal and fetal serum levels of total cortisol and cortisone, unbound cortisol, and corticosteroid-binding globulin in vaginal delivery and cesarean section.

The levels of corticosteroid-binding globulin (CBG, transcortin), total cortisol, and unbound cortisol were measured in maternal and fetal serum at the time of cesarean section or vaginal delivery. While the total cortisol levels in maternal serum do not differ significantly in vaginal delivery as compared to cesarean section, cord serum cortisol levels are significantly higher after vaginal delivery. There are no differences associated with the route of delivery between the mean levels of CBG in cord and maternal blood. However, after the vaginal delivery the unbound cortisol level in maternal serum is more than twice as high as that at cesarean section and in cord serum it is seven times as high. These changes reflect changes in total cortisol and may represent a prelabor surge of fetal cortisol production, or a stress response associated with labor, or a combination of both.

Adrenal cortical response to circumcision in the neonate.

A study was designed to document the responsiveness of the hypothalamic-pituitary-adrenal axis to the stress of circumcision during the first 6 hours of life. There was a rise in both serum cortisol and cortisone levels in all infants studied. The rise in cortisol was greater than the rise in cortisone in all infants.
