Diagnostic applications of wrist arthrography.

Arthrography of the wrist joint as a dynamic examination aids in confirming lesions of interosseous ligaments of the triangular fibrocartilage complex and soft tissue. The results of arthrography in 185 wrists, classified according to age of patients and pathological disorders with or without trauma demonstrate mostly post-traumatic discus lesions in younger patients. Older patients have an increase in pathological findings with a similar distribution between ligaments and discus. Soft-tissue injuries or discus lesions can be confirmed or excluded before osteoplastic surgery for distal radius fractures or reconstruction of ligaments in cases of carpal instability. Pathological changes of biomechanics of the hand and wrist are noticed more often and consequently are treated surgically. Arthrography of the wrist joint is a valuable means of diagnostic imaging.

[Indications for computerized tomography of the wrist joint and carpal bones].

Twenty-nine patients with thirty-one fractures of the distal radius and persistent pain or disability had computed tomography of the radio-carpal and distal radioulnar joint. We found incongruity of the distal radio-ulnar joint in six patients. Ten patients had a rotational malalignment in supination. Seven patients suffered from rotational malalignment in pronation. The ability to quantify rotational malalignment and to have images of the degenerative arthritis following these disorders without superimposition aids in selecting surgical treatment.

Surgical treatment of the rheumatoid wrist and aspects of the natural course when untreated.

Based on 20 years of experience in surgery of the rheumatoid hand, the author has presented diagnostic and surgical problems of tendons, ligaments and bones round the rheumatoid wrist. The results of long-term follow-up studies after wrist synovectomy have been given and also aspects of the natural course of the untreated rheumatoid wrist. The author's technique of wrist arthrodesis has been described in detail.

[Osteosyntheses in rheumatoid arthritis].

The wrist joint is fused by an original method consisting of a Rush pin from the third metacarpal bone high up in radius and secured against rotation by two staples between carpal bones and radius. The MP-joint of the thumb is arthrodesed by aid of a staple with compression - a special instrumentarium is used. PIP-joints were fused according to BOYES technique and the DIP-joints with compression screws.

The carpal tunnel syndrome. A retrospective study of 400 operated patients.

Carpal tunnel syndrome is a common cause of pain, often combined with hypaesthesia in the median distribution of the hand and atrophy of the thenar. We describe pre-, per-, and post-operative findings in 400 patients. Venous stasis is a good diagnostic method in establishing the syndrome. Operative treatment generally gives freedom from pain and normalization of sensibility.

Silastic arthroplasty of the metacarpophalangeal joints in rheumatoid arthritis.

Fifty rheumatodi patients (fifty hands) had Silastic joint implants in 144 metacarpophalangeal joints. Average follow-up time was 2.5 years (maximum 3.5 and minimum 1.5). At clinical examination, gross power, range of motion, lessening of ulnar deviation, relief of pain, and appearance were determined. The results were correlated with preoperative indications. Postoperatively, there was no significant gain in gross power. The range of motion was somewhat increased and the lessening of ulnar deviation was marked. Relief of pain was achieved. Complications included fracture of four of the 144 implants, one infection, and six cases of tendon adhesions.
