[Fundus changes due to arteriosclerosis and chronic-arterial hypertension (author's transl)].

The authors report on difficulties in ophthalmoscopac differential diagnosis of fundus changes due to arteriosclerosis and hypertension. An attempt is made to present a classification of hypertensive alterations which -- in close association with the scheme published by Keith, Wagner and Neubauer -- uses the same descriptive terms but restricts itself to distinguish between changes of the vascular system on the one hand and changes of the parenchym as a consequence of the vascular changes on the other hand.

The judgement of disablement is of special economic and social-medical importance. While there are nearly no differences between physicians concerning the judgement of disablement in cases of grave ophthalmological clinical pictures, there exist greater differences in the judgement of so-called "bagatelle"-injuries and -diseases, which demand a temporary unilateral eye-dressing. The problem of functional unilateral vision up to now was considered under the aspect of a lasting disease, but not for a short period of time. A unique decision concerning the disablement should be aimed at by all medical practitioners. In addition to the judgement of disablement the private insurances demand a detailed report for the temporary impairment of disablement, which by no means can be demanded from the ophthalmologists.

Adolescent

[The judgement of the degree of disability in cases of aphakia and pseudoaphakia (author's transl)].

Since more than 40 years the judgement of the degree of disability in cases of unilateral aphakia is discussed. This means the effort toward an advisory objectivity on the one hand and human understanding for the patient's damage on the other hand. It is supposed to repeal the 4th emergency ordinance of 1932 and to pay pensions below 20% of the degree of disability, too. As long as new legislatory measurements do not exist, the maintenance of a pragmatic authoritative activity is plead for, which assigns for a pension-duty compensation in cases of unilateral aphakia and/or pseudoaphakia independent on the optic result.--The situation in cases of bilateral aphakia is also discussed.

Aphakia

[Professional discretion in cases of unfitness to drive due to impaired vision (author's transl)].

The question of professional discretion is discussed in relation to cases where the physician believes that the patient's visual impairment renders him unfit to drive. It is often difficult for the ophthalmologist to make a valid judgment. Since physicians are not bound to report such cases to the traffic authorities or to third persons, any assumption of responsibility in making a statement, either without the patient's knowledge or against his will, is an infringement of professional secrecy. Article 34 of the (West German) Penal Code makes provision for "justified" disclosures. There is also a relevant verdict of the German Federal Supreme Court dated Oct. 8, 1968, case ref. VIZR 168/67, which deals with basic considerations. However, it is advisable in each case to consult the legal adviser of the responsible Medical Council prior to making any disclosure in order to establish whether it is justified.

[Conjunctival covering (Harms) (author's transl)].

The authors report on 7 cases of conjunctival flap cover in the technique of Harms and Mackensen (1963) in cases of leackage of the bleb after filtering operations. The technique consists in excision of the thin conjunctiva of the bleb, undermining of the conjunctiva until the fold, arched incision of the cornea in front of the bleb, incarceration of the conjunctival lip in the corneal incision with fixation by continuous suture with monophile nylon-string (10X0). In 6 of 7 cases the intraocular pressure was spontaneously regulated after the operation with morphological functioning bleb in 4 cases.

Conjunctiva

[Obligation to tolerance and reasonability of cataract extraction (author's transl)].

The reasonability of cataract extractions is mentioned with exposition of the interests of the insuring bodies and of the patient. We discuss individually the terms "medical treatment", "obligation to tolerance" and "reasonability". The surgical method and the general physical assessment of the patient earn special consideration for a "reasonable" cataract extraction. We also discuss the special situation of the surgeon.

[Clinical tests on the use of ethoxzolamide in glaucoma therapy (author's transl)].

Ethoxzolamide is a potent carboxyanhydrase inhibitor, and its use in chronic glaucoma and in the acute glaucoma attack is described in the present investigation. Ethoxzolamide shows itself to be extremely well tolerated. The frequency and intensity of the side-effects is low. The electrolyte balance remains also after prolonged administration compensated. The important advantage of this drug is its low dosability. Ethoxzolamide, which was already 1958 clinically evaluated (Gordon-Posner), represents a valuable medication for conservative glaucoma therapy.
