[Neuropsychological investigations of thought processes in aphasics (author's transl)].

Fifteen aphasics and 10 patients with cortical lesions of the right (nondominant) hemisphere were examined using 10 nonverbal psychological tests with 23 variants. The number of correct solutions and the duration of fulfillment of each variant were recorded with statistical significance and standard deviations. Aphasics as well as patients with right-sided cortical damage have lower scores than normal individuals for both correctness and duration of performance. The two groups of patients differed in that the number of correct solutions was higher in aphasics than it was in right-sided cortical injury cases. Aphasics required a longer time to perform the tests used by the authors than normals did, and those with right-sided cortical lesions required an even longer time. The results were also dependent on the kind of test applied. For example, the differences shown by tests 4, 5, 6, and 10 were comparatively less among the three groups (normals, aphasics, and right-sided damage of the cortex) and nonsignificant. The results are discussed in relation to thought processes, language, and aphasia.

[The interrelations of genital lesions-urinary tract infections in girls].

This investigation was aimed at emphasizing the importance of early detection and elimination of the favoring factors which associated with an adequate treatment of urinary tract infection (UTI) represent the only way to prevent the recurrences that with time might cause severe kidney dysfunctions. It is a retrospective study of 220 girls aged 1 month to 16 years referred in the interval 1993-1997 to Iaşi Polyclinic No. 1 for pediatric gynecological examination and diagnosed with urinary tract infection. The clinical and laboratory evaluations were directed at determining the site of UTI, the factors that favored the recurrences, and the effects on renal function. The following favoring factors were found: disorders of the genital tract (22.2%), constipation (11.6%), oxyuriasis (4.5%), urinary tract obstructions (5.9%). The genital tract disorders were represented by inflammation (70%) (vaginitis, vulvovaginitis) and malformations (30%) (labia minora fusion, imperforate hymen, etc). The specific inflammation (48.5%) was mycotic (41.1%), bacterial (29.3%), the same pathogen being detected both in urine and vaginal discharge, trichomonal (17.6%), and gonococcal (11.6%). The high rate of recurrence in some UTI cases has proved once again the important role played by the favoring factors (genital tract disorders, urinary tract obstruction, constipation) and suggested that the correction of these conditions should be the only reasonable way to prevent recurrences.

Melodic intonation in the rehabilitation of Romanian aphasics with bucco-lingual apraxia.

The main objective of the present study was to assess the efficiency of the melodic intonation therapy (MIT) in the rehabilitation of Romanian aphasics. Eighty predominantly Broca aphasics used the melodic intonation therapy when no other therapy methods were very efficient. The speech therapist intonated the respective word, then together with the patient and finally let him continue alone. The control group counted 80 aphasics and it applied other therapy methods. Each patient, regardless of the group, was tested twice, before and after therapy. Since most of the patients displayed severe language disorders, and other therapy methods failed in rehabilitating them, MIT was considered an efficient method in the early stages of Broca aphasia with bucco-lingual apraxia.

Vocabulary grammatical structure in aphasic patients.

In 10 aphasics (7 Broca's and 3 Wernicke's) of vascular etiology and 10 normal controls, a corpus of 2,500 words was selected in each subject by means of a standard interview. The 10 grammatical classes of the Romanian language were studied. The vocabulary reduction observed in aphasics was produced by the loss of nouns, adjectives and verbs. In exchange, proportionally to their vocabulary, they used more adverbs, pronouns and numerals than the normals. Articles, prepositions and conjunctions showed a similar use with that of the normals. The aphasic patients preserved basic words for each part of speech, the loss of vocabulary operated among the non-basic words. It was revealed by the Token-Type ratio that the verbal creativity was lower in aphasics than in controls. However, in normals as in aphasics verbal creativity was higher in the non-basic words than in the basic ones. The results are discussed by the action of different factors as: the semantic and syntactical constraints, semantic characteristics and syntactical functions proper to each class as well as the influence of the different amounts of words comprised in each grammatical class on the selection of the necessary word.

Use of word- or short-syntagma-stimulation for utterance production in aphasics.

In 50 aphasics and 30 normal controls, a therapeutic stimulating technique using different classes of words and short syntagmas was performed. The different classes of words or short syntagmas elicited different types of answers. Unlike controls, the aphasics showed a tendency to less often include the stimulus in the answer and to substitute it inadequately. The stimulus was not too often modified when included in the answer, the initial position being more frequent, as a support for sentence production. Grammatical agreement was deficient. Poor structures of the answers were predominant. After speech therapy using various types of stimuli, the performance in aphasics improved. Since stimuli of different morphological classes produce different performances, it seems necessary to use all types of stimuli in the speech therapy.
