Pelli-Robson contrast sensitivity test in Zaire.

The purpose of this study was to establish in this first report the age standards of the Pelli-Robson contrast sensitivity in Zaïre. Contrast sensitivity using the Pelli-Robson chart was performed in 100 normal Zairian black subjects aged from 10 to 59 years and 36 patients (22 patients with open-angle glaucoma and 14 with optic nerve disease). Scores of normal subjects were age related (p < 0.05). The results of Zairian young subjects were similar to those found previously in young white subjects; scores for older subjects were lower when compared to those of whites. Scores of patients were lower than those of normals (p < 0.001). Contrast sensitivity using the Pelli-Robson chart can be useful in developing countries.

[Ocular complications in diabetes mellitus in Zaire].

OBJECTIVE: to determine the incidence of ocular complications of diabetes in Zaïre. METHODS: the study was based on an analysis of data from 382 diabetic patients seen from 1978 through 1989, and from Jan. 1990 through April 1990. Ocular complications were determined by routine ophthalmic examination including an inspection of adnexa, biomicroscopy, ophthalmoscopy. RESULTS: the average age of patients was 46 years the male to female ratio being 2:1. Ocular complications were found in 200 (52%) of 382 patients and included diabetic retinopathy (32%), cataract (18%) and other manifestations (vitreous hemorrhage, diabetic papillopathy, neovascular glaucoma, rubeosis iridis, keratitis, hordeolum) were rare (2%). Blindness caused by diabetes mellitus was found in 4% of patients and was due to diabetic retinopathy and cataract. CONCLUSION: Ocular complications seen in this study were comparable to those reported in Africa and the World.

Prospective HLA-DR matching in penetrating keratoplasty.

This prospective study examines whether HLA-DR matching has a beneficial effect on corneal graft survival of high risk patients. Until now, 196 donors have been typed in order to provide 20 patients with a matching graft. The results, though preliminary, are very encouraging.

Light-induced maculopathy.

In this retrospective study of fluorescein angiograms performed between 1991 and 1995, we retrieved 17 eyes (15 patients) with light-induced maculopathy after a cataract extraction and lens implantation. Sixteen eyes underwent non-complicated procedures; 12 extracapsular cataract extractions and 4 phaco-emulsifications. One phaco-emulsification was complicated by a posterior capsular rupture and required an anterior vitrectomy. In 11 eyes the maculopathy was an incidential finding and the lesions were paracentral. In 6 eyes the phototoxic scar was central and the cause of a central scotoma with metamorphopsia.

Non-arteritic anterior ischemic optic neuropathy and refraction.

A small cup-disk ratio has been reported as a risk factor in non-arteritic anterior ischemic optic neuropathy. Hyperopia is possibly related with a small cup-disk ratio. The purpose of the present study is to compare the refraction of 37 eyes with non-arteritic anterior ischemic optic neuropathy with a sex and age matched control group of 74 eyes. In our study, mild hyperopia was more frequently observed in the anterior ischemic optic neuropathy group than in the control group.

[Myelin fibers in Zairian patients].

PURPOSE: to determinate the characteristics of myelinated nerve fibres in Zairian blacks patients. METHODS: a review of records of patients with myelinated nerve fibers seen from 1962 to 1994. RESULTS: the relative incidence of myelinated nerve fibers was 0.02%. There were 13 (76%) male patients and 4 (24%) female patients. The average age at the moment of diagnosis was 32 years (range, 7 to 61 years). Myelinated nerve fibers were bilateral in 8 (47%) patients (16 of 25 eyes, 64%) and unilateral nerve fibers, 21 (84%) eyes contained a single lesion and 4 (16%) eyes contained multiple separate and distinct patches of myelinated fibers. Continuous lesions were present in 21 (84%) eyes and discontinuous lesions in 4 (16%) eyes. The 25 affected eyes varied in refractive error from a spherical equivalent of + 1.5 to - 16.00. The distribution of refraction was: emmetropia in 2 (12%) patients (2 eyes, 8%), simple myopia in 4 (24%) patients (5 eyes, 20%), myopia over 6 D with amblyopia in 6 (35%) patients (10 eyes, 40%), hypermetropia in 5 (29%) patients (8 eyes, 32%). CONCLUSION: this study showed a low relative incidence of myelinated nerve fibers and the bilaterality of lesions was frequent.

[Exophthalmias in Zaire].

PURPOSE: To determinate the frequencies of causes of exophthalmos in Zaïre. METHODS: In a retrospective study, the records of 129 patients with exophthalmos seen from 1986 to 1994 were reviewed. All patients were examined ophthalmologically and most of them medically. X-Rays, orbital CT Scan, were performed most frequently. RESULTS: The sex distribution was: 79 (61%) male patients and 50 (39%) female patients. The sex ratio being 1.50/1. The ages of patients ranged from one to 68 years. Unilateral exophthalmos was frequent (76% of cases). Tumors were present in 42% patients, followed by inflammatory diseases in 26% of patients, thyroid eye disease in 9% and traumatic aetiology in 1.6%; the etiology was not determined in 14%. Retinoblastoma (8%), lymphoma (7%) and rhabdomyosarcoma (5%) were the most frequent tumors seen. Orbital cellulitis (26%) was frequently associated with adjacent sinus infection. CONCLUSION: The etiologic factors found in this study could be compared to those published in the World with the exception of orbital cellulitis and retinoblastoma that showed a high incidence in this study.

Efficacy and safety of indomethacin 0.1% versus flurbiprofen 0.03% eyedrops in inflammation after argon laser trabeculoplasty. The Belgian Study Group on Glaucoma.

After Argon Laser Trabeculoplasty (ALT), non steroidal anti-inflammatory drugs (Indomethacin 1% suspension and Flurbiprofen 0.03% solution) were shown to be efficient in the management of post laser inflammation. To evaluate the efficacy and safety of Indomethacin 0.1% solution versus Flurbiprofen 0.03% eyedrops in that purpose. 64 patients suffering from glaucoma were recruited in a multicenter randomized double masked study. The results demonstrate that Indomethacin 0.1% eyedrops is as efficient as Flurbiprofen 0.03% in terms of control of post ALT inflammation, with a good tolerance.

Optic atrophy as the presenting sign in Hallervorden-Spatz syndrome.

A girl of eight years of age is described with the Hallervorden-Spatz syndrome. Optic atrophy with progressive visual loss was the only presenting symptom for three years. Behavioural and motor disturbances emerged after that time. Optic atrophy can be the first and for some time only symptom of the Hallervorden-Spatz syndrome.

[Congenital malformations of the eyeball and its appendices in Zaire].

OBJECTIVE: to determine the epidemiology of congenital eye malformations. METHODS: the records of 1740 patients (1913 eyes) with congenital eye malformations seen from 1962 through 1992 were reviewed. RESULTS: the prevalence rate of congenital eye malformations was 2.2%. Congenital cataract (38%), atresia of the naso-lacrimal duct (10%), congenital glaucoma (9%), congenital ptosis (8%), microphthalmos (8%), albinism of iris (8%), microcornea (7%) were the most frequent eye malformations. The common types of associated malformations included albinism, microcephaly, cardiopathy and anomalies of ears. CONCLUSION: the findings of this study were similar to those published in Europe and United States.

Method for detection of conjunctival chlamydial infection and allergic conjunctivitis.

In this study 104 adult patients with chronic conjunctival irritation without obvious diagnosis were investigated by conjunctival scrapings. The smears were stained with May-Grünwald-Giemsa or fluorescein-conjugated monoclonal antibodies prepared against Chlamydiae. In 45 cases it was possible to make a correct diagnosis of Chlamydia infection (29%) or allergy (20%).

Ocular findings in prematurely born children at the age of 12.

A group of 79 prematurely born children was assessed ophthalmologically at the age of 12 and compared with a group of 100 12 years old children born at term. Of the 79 children, results of neonatal eye examinations were available in 26 prematures (52 eyes). In 28 eyes no signs of retinopathy of prematurity (ROP) were detected (non-ROP group). In 24 eyes a stage 1 or 2 retinopathy of prematurity was present (ROB group). 75% of the children with ROP had an ophthalmological problem. Myopia (29%), hypermetropia (21%), astigmatism (37%) and strabismus (33%) were significantly higher. Considering the whole group of prematurely born children, there was more astigmatism (11%) and strabismus (13%). Preterm children, with or without ROP, are more prone to develop ocular problems.

[The Farnsworth-Munsell 100 Hue test in the Bantu population. Preliminary results].

The purpose of this work was to establish in this first report the age standards of the Farnsworth-Munsell 100 hue in Zaire. The Farnsworth-Munsell 100 Hue test was performed in 132 normal subjects. All subjects were Zairian blacks aged from 20 to 49 years. The test was administered binocularly, once, to each subject. Subjects were divided in three age groups. The first age group had 35 subjects aged between 20 and 29 years, the second age group had 48 subjects aged between 30 and 39 years and the third 49 subjects aged between 40-49 years. The total error scores were 7.06 +/- 2.10 for the first age group, 9.03 +/- 1.80 for the second age group and 9.80 +/- 2 for the third age group. These results were aged related (r = 0.42, p inferior to 0.05). Scores of Zairian subjects in this study were higher when compared to those reported in Europe and U.S.A. Interpretation of results of the Farnsworth-Munsell 100 hue test must take in account this difference in ethnic origin.

Silastic intubation for Canaliculus communis stenosis.

Silastic intubation was performed for Canaliculus communis obstruction in 32 cases. Two eyes showed additional impatent nasolacrimal duct and were not treated. The silastic intubation was performed by aid of a long, hollow blunt injection needle which allows us to check the patency of the lacrimal system once the stenosis of common canaliculus is perforated. The success rate for isolated silastic intubation was 70%, which is similar to the results of other often more invasive surgical techniques for Canaliculus communis stenosis.

A subgroup of bipolar cells in human retina is GABA-immunoreactive.

The distribution of GABA in the perifoveal and the near and far peripheral region of human retina was studied with peroxidase anti-peroxidase immunocytochemistry applied on semithin epoxy resin sections. Among the labeled amacrine cells in these regions, four types can be identified: putative diffuse A2 amacrines, stratified semilunar amacrines, interstitial amacrines and small displaced amacrines. GABA-immunoreactive interplexiform cells and ganglion cells also occur. Contrary to previous post-embedding studies, our preparations show that some bipolar cells in the near and far peripheral region are GABA-immunopositive. This indicates that a number of bipolar cells in human retina does have an enhanced GABA content.

Eye Enucleation

New aspects of dopaminergic interplexiform cell organization in the goldfish retina.

Dopaminergic interplexiform cells (DA-IPCs) in the goldfish retina have been reexamined by light and electron microscopic immunocytochemistry with antisera against dopamine (DA) or tyrosine hydroxylase (TH). Successful immunostaining with a specific anti-DA antiserum offers further direct support for DA-IPCs. Anti-DA immunocytochemistry in combination with [3H]-DA autoradiography shows 92% colocalization of the two markers, indicating that [3H]-DA autoradiography is a reliable technique for identification of DA-IPCs. Incubations with anti-TH antiserum show that immunoreactive DA-IPCs have a homogeneous distribution, with an average frequency of 71 +/- 8 cells/mm2 in retinas of 14-15 cm long goldfish. Their arrangement is distinctly nonrandom. Electron microscopy of TH-immunoreactive cell processes confirms that horizontal cell axons synapse onto DA-IPCs and adds the following junctional arrangements to the circuit diagram of the DA-IPC: 1) adjacent serial synapses between DA-IPCs, external horizontal cells, and putative glycinergic interplexiform cells, 2) junctional appositions between DA-IPCs and photoreceptor cells, 3) junctional appositions between neighbouring DA-IPCs, and 4) the "gap junctional complex," typically consisting of a DA-IPC process juxtaposed with a gap junction between horizontal cell axons. The gap junction is flanked by clusters of small, round vesicles and groups of electron-dense structures resembling intermediate filaments. These morphological results support the functional involvement of DA-IPCs in adaptive retinomotor movements and in horizontal cell gap junction modulation and/or dynamics. They also suggest particular interaction between the dopaminergic and the glycinergic IPC system in the outer plexiform layer of goldfish retina.
