Effects of baroceptor and cardiopulmonary reflexes on neural control of heart rate in normal man, as assessed by power spectral analysis.

This study was designed to establish the neurally-mediated effects of baroceptor and cardiopulmonary reflexes on heart rate in normal subjects. We therefore studied the effects of various interventions, able to modify the activity of arterial baroreceptors and cardiopulmonary receptors, on the power spectrum of the R-R interval in nine healthy men. To confirm the efficacy of these interventions left ventricular volume was monitored using a portable radionuclide probe. Isosorbide dinitrate (5 mgm sublingually) while sitting unloads both baroreceptors and the cardiopulmonary receptors; it decreased left ventricular end-diastolic volume (p < 0.01) and the mean R-R interval (p < 0.005), increased the power of the low-frequency (LF) component of the R-R interval (p < 0.01), decreased the power of the high-frequency (HF) component (p < 0.005) and increased the LF/HF ratio (p < 0.05). The application of cuffs around the thighs in the supine position, which unloads only cardiopulmonary receptors, decreased left ventricular end-diastolic volume (p < 0.05) but did not affect mean R-R interval, LF component, HF component and LF/HF ratio (p > or = 0.10). Leg raising, which loads only cardiopulmonary receptors, increased left ventricular end-diastolic volume (p < 0.05) and did not affect the mean R-R interval, LF component, HF component and LF/HF ratio (p > or = 0.10). In conclusion, our data suggest that, in normal man, baroreceptor unloading increased sympathetic and decreased parasympathetic neural control of heart rate; cardiopulmonary reflexes do not appear to have a direct neurally mediated effect on heart rate.

Case report: the air crescent sign in pulmonary leukaemic infiltrate.

Localized pulmonary infiltrates in leukaemic patients were until recently almost synonymous with superinfection. This case illustrates that true leukaemic pulmonary involvement should always be considered in these patients when a high peripheral blast count is present. We believe this to be the first description of cavitation in a proven leukaemic infiltrate.

Clinical usefulness of Tc-99m HMPAO labeled white blood cell imaging in prosthetic vascular graft infections.

The reported frequency of prosthetic vascular graft infections after reconstructive vascular surgery is low. However, it is a serious and potentially catastrophic complication of this treatment, with high morbidity and mortality rates. Effective therapy involves graft excision and replacement. Tc-99m HMPAO labeled leukocyte scintigraphy appears to be an accurate method of detecting prosthetic vascular graft infections. The value of leukocyte scanning is illustrated by two case reports.

Effect of ticlopidine on blood loss, platelet turnover and platelet deposition on prosthetic surfaces in patients undergoing aorto-femoral bypass grafting.

Sixty patients in whom a prosthetic aorto-femoral bypass graft was inserted were pretreated with ticlopidine (250 mg b.i.d.) or placebo in a double-blind study. Blood loss and transfusion need were higher in ticlopidine treated patients. Platelet survival was partially corrected and platelet deposition on prosthetic surfaces was inhibited with ticlopidine. The study suggests that ticlopidine does not increase excessively the bleeding risk during peripheral vascular surgery and that the antiplatelet drug may have beneficial effects in the early phase after insertion of prosthetic vascular graft.

Left ventricular function changes during pharmacological and physiological interventions and routine activities monitored in healthy volunteers by a portable radionuclide probe (VEST).

A miniaturized radionuclide cardiac probe incorporated in a semi-rigid plastic chest garment has made ambulatory left ventricular (LV) function evaluation possible, with gated nuclear data being stored on tape together with electrocardiographic data, for subsequent off-line processing. After red blood cell labelling with 555 MBq (15 mCi) 99mTc and standard gated blood pool imaging in 45 degrees LAO, we performed a continuous monitoring of LV function in 20 healthy male volunteers (age range: 22-25 years), in resting control conditions as well as during activities (standing, walking, climbing stairs) and after interventions (isosorbide dinitrate intake, Trendelenburg position, inflation of cuffs around the thighs). VEST-monitoring proved to be a reliable method that gave reproducible results: changes of ejection (EF) in basal conditions were lower than 5% in 95% of the patients. Changes in LV function caused by daily activities were easily demonstrated. While standing effected no significant EF changes, walking and climbing increased EF by 6.9% (p less than 0.05) and 21.2% (p less than 0.05) respectively. Changes in LV volumes caused by alterations in venous return were also demonstrated. Compared with baseline, Trendelenburg increased end-diastolic volume (EDV) by 2.9% while isosorbide dinitrate and inflation of cuffs decreased it by 5.7% and 2.2% respectively.

Clinical significance of squamous cell carcinoma antigen in cancer of the human uterine cervix. Comparison with CEA and CA-125.

Serum squamous cell carcinoma antigen (SCCa) concentrations were determined by a radioimmunoassay kit before and during the treatment of 50 patients with cervical carcinoma: 44 with squamous cell carcinoma (SCC) and 6 with adenocarcinoma. The positivity rate of SCCa was 50% (52% for SCC and 33% for adenocarcinoma). The sensitivity of SCCa for SCC was twice as high as that of CEA and CA-125. Low serum concentrations were observed in early-stage carcinoma, indicating that SCCa is not useful for diagnosis. In advanced cases, serum levels were directly and significantly correlated with the stage of the disease.

Combined Modality Therapy

Evaluation of the effect of thrombolytic treatment on infarct size and left ventricular function by enzymatic, scintigraphic, and angiographic methods. The European Cooperative Study Group for Recombinant Tissue Type Plasminogen Activator.

In a double-blind trial of the European Cooperative Study Group, 721 patients with acute myocardial infarction of less than 5 hours' duration were given either 100 mg recombinant tissue-type plasminogen activator (rt-PA) intravenously over 3 hours or an equivalent placebo infusion. In a subset of 312 patients, infarct size was assessed by the cumulative myocardial release of alpha-hydroxybutyrate dehydrogenase (HBDH) during the first 72 hours and by planar thallium scintigraphy (index of hypoperfusion) performed 10 to 22 days after the acute event. Left ventricular ejection fraction (LVEF) was determined by contrast and nuclear angiography. The median values of HBDH during the first 72 hours were 20% lower and the median values of thallium-201 28% smaller in the rt-PA group in comparison with controls. A significant but limited improvement of angiographic LVEF (2 absolute percentage points) was also shown in the patients treated with rt-PA. A moderate but statistically significant linear association between both measurements of infarct size and LVEF was found.

Tc-99m HM-PAO labelled leucocyte scanning for detection of infection in orthopedic surgery.

The use of labelled granulocyte scintigraphy is recognized as a reliable method for detecting osteomyelitis and other skeletal sepsis. Tc-99m hexamethyl propyleneamine oxime (Tc-99m HM-PAO), a lipophilic chelate offers the advantages of availability, lower radiation dose and higher image resolution, compared to other labelling agents. We have scanned 36 orthopedic patients with suspected infection, using autologous granulocytes labelled with Tc-99m HM-PAO. In all cases, scans were compared with clinical, microbiological and/or surgical findings. Sensitivity was 94% and specificity was 80%. We therefore believe that Tc-99m HM-PAO leucocyte scanning provides a useful method for detecting osteomyelitis and other skeletal infections.

Clinical experience with Tc-99m HM-PAO high resolution SPECT of the brain in patients with cerebrovascular accidents.

In order to evaluate the diagnostic contribution of brain SPECT imaging with 99mTc-HMPAO in cerebrovascular disease, we examined 92 stroke cases (144 lesions), 2 hematoma cases and 30 cases with transient neurologic symptoms. Abnormal tracer distribution is visible as zones of either hypoactivity or hyperactivity (border zone hyperemia or luxury perfusion). Remote vascularization changes could also be found (crossed cerebellar diaschisis or ipsilateral cortical perfusion reduction in thalamic or capsula interna lesions). Both X-ray CT and blood flow SPECT have comparable sensitivity in the exploration of cerebral infarction, with detection in, respectively, 89.5% and 87.5% of the lesions. False negative scintitomographic images are frequently recorded in small lacunar infarcts within the basal ganglia and white matter (capsula interna). Some early infarcts and asymmetry of brain perfusion in patients with transient neurologic symptoms are frequently not detected by CT. An additional advantage of blood flow SPECT is its ability to visualize remote blood flow changes and the changing pattern of vascularization of ischemic lesions and their surrounding areas including hyperemia.

Model-based quantification of myocardial perfusion images from SPECT.

A system for quantitative analysis of myocardial perfusion tomograms is proposed. The system starts with an automated delineation of the total left ventricle, including possible perfusion defects, to determine the mass and shape of the myocardium. Next, polar maps or bulls-eyes are computed from the delineation, which can then be compared to reference bulls-eyes to detect perfusion defects. The proposed system differs in three main aspects from currently available bulls-eye algorithms. First, radial slices are used rather than short-axis slices. In this way three-dimensional gradient information is retained, in particular near the base and the apex of the left ventricle. Moreover, the reproducibility of this method is expected to be superior, since the interactive selection of short axis slices through the left ventricle is eliminated. Second, the left ventricle is automatically delineated using a flexible computer model in order to obtain higher reproducibility. The resulting delineation contains both mass and shape information. Third, in addition to the classic count rate bulls-eye, a mass bulls-eye is computed, which contains the myocardial mass corresponding to each bulls-eye pixel. Analysis of the count rate bulls-eye reveals perfusion defects, the quantification of the defects is carried out with the mass bulls-eye.
