[Arterial hypertension, disorders of carbohydrate and lipid metabolism in obese perimenopausal females].

AIM: To examine prevalence of arterial hypertension (AH), disorders of carbohydrate and lipid metabolism in females with obesity in perimenopause. MATERIAL AND METHODS: One-stage trial enrolled 358 obese females in menopause seeking advice in the city center for health and family planning in Ekaterinburg (mean age 50.5 +/- 5.4 years). RESULTS: 111 (31%) females suffered from obesity, 154 (43%) had overweight, 93--normal body mass. A relative risk to develop AH in overweight was 2.61, in obesity--3.52. Diabetes mellitus was in 15 females with obesity, disturbed glucose tolerance was in 38 females with overweight. CONCLUSION: Body weight loss is an important component of perimenopausal women rehabilitation.

[The role of eosinophils in bronchial asthma].

A total of 65 patients with infection-dependent bronchial asthma (BA) in the phase of exacerbation were examined for eosinophil number in the peripheral blood, forced expiration volume within the first second (FEV1), systolic pressure in the pulmonary artery (SPPA), the content of endogenous hydrocortisone and IgE, and for the titre of circulating immune complexes (CIC). The group of patients with eosinophilia over 600/mm3 showed a more grave course of BA. As the number of eosinophils in the blood increased, the SPPA and the CIC titre significantly rose. A marked tendency was discovered towards enhancement of bronchial obstruction as well as towards the increase of the content of endogenous hydrocortisone. A good agreement was demonstrated between the number of eosinophils in the blood and hydrocortisone content in the serum as was a negative correlation of the mean strength between the eosinophilia level and FEV1. On the patients' distribution into groups in terms of the gravity of obstructive abnormalities a significant rise of eosinophilia was noticed in patients with marked respiratory disorders. It is concluded that eosinophilia in BA patients is an unfavourable symptom evidencing that the patients are prone to a graver disease course, whereas the level of initial eosinophilia may be regarded as one of objective criteria for glucocorticosteroid administration.

[Effect of cerebral hypoxia and hyperventilation hypocapnia on the epileptiform activity of the cerebral cortex of the cat].

Influence of cerebral hypoxia and hyperventilatory hypocapnia on the ECoG and focal epileptiform activity of the cerebral cortex induced with local application of strychnine was studied in cats with transection of spinal cord at C1. Although both hypoxia and hypocapnia produced synchronization of the cerebral cortex electrical activity, i.e. exerted the same effects on the ECoG, their influence on cortical excitability was quite different: hypoxia suppressed the epileptiform activity whereas hypocapnia facilitated it.

[Mechanism of hyperventilation in exertion (an uncommon factor)].

Electrical stimulation of the corticospinal tract increases the excitability of the phrenic motoneurons directly. As a result the responses of these motoneurons to the impulses from the respiratory centre are facilitated. Until now there were attempts to explain exercise hyperpnea by increasing the activity of the respiratory centre. The experiments permit to suppose that the hyperpnea may be produced by facilitatory action of the corticospinal tract on the respiratory motoneurons, i.e. independently from the respiratory centre.

[Mechanism of the protopathic nature of visceral pains].

Experiments on decerebrated cats and rabbits have shown that local nociceptive stimulation (mechanical pressure and burns) of internal organs causes no reflex responses of the motor system. It is suggested that internal organs are either absolutely devoid of or lack specific nociceptors. Insufficiency of inhibitory mechanisms may account for protopathic nature of visceral pain.

[Hyperventilatory hypocapnia and muscle tonus].

It has been established that hyperventilation hypocapnia inhibits the postural tone in both the unrespiratory and respiratory muscles. However, muscle excitability increases at the same time. As a result phasic as well as tonic reflexes in response to additional nervous stimuli are facilitated in the presence of hypocapnia.

[Results of a 2-year prospective study of patients with ischemic heart disease with stable angina pectoris].

Data of dynamic control of 278 coronary patients with stable angina pectoris are presented. Coronarography was performed in all cases to determine changes in the heart coronary arteries which were of varying markedness. The anginal syndrome, ECG at rest, exercise tolerance, incidence of myocardial infarction and mortality rate were assessed after one and two years of dynamic observation. The incidence of nonfatal myocardial infarction was estimated at an average of 6.3% per year, whereas the mortality rate was 3.8%. The prognosis for patients with stenosing coronary atherosclerosis depends on the number of affected coronary arteries, the functional class of angina pectoris and exercise tolerance. Data on the efficiency of long-term pharmacological treatment of patients with stable angina and stenosing coronary atherosclerosis are obtained.

[Biological significance of physiologic hypoxemia].

Preliminary inhalation of oxygen in conscious rabbits prevents increased respiratory muscle activity caused by airway occlusion. This is accounted for by hyperoxia that removes physiological hypoxemia and therefore depresses the sensitivity of the respiratory center which is needed to perform the reflex caused by airway occlusion. This also explains that hyperoxia prevents respiratory discomfort caused by temporary stoppage of artificial respiration in persons with respiratory paralysis.

[Role of the pyramidal tract in the mechanism of dyspnea and hyperventilation].

Hypoxia evoked by mechanical asphyxia first of all disturbs the generation of potentials in pyramidal tract neurons. Hypoxic work, therefore, causes inadequate contraction of somatic muscles, a difficulty in work performance and respiratory discomfort. Concurrently, the disturbance of the pyramidal tract disinhibits the respiratory center of the brain stem with a result of excessive hyperventilation during hypoxic work.

[Significance of external respiratory indices in the diagnosis of pulmonary hypertension].

The results of the presented study include the parameters of pulmonary ventilation, respiratory volumes, pulmonary diffusion, oxygen saturation of arterial blood in patients with mitral stenosis, aortic valve insufficiency, and primary forms of pulmonary hypertension, as well as the dynamics of several parameters of pulmonary respiration in a group of patients following an alleviation of their circulatory insufficiency. Some pecularities of the changes in the studied parameters were demonstrated with reference to the peculiarities of the haemodynamic shifts in the pulmonary circulation, to the nature of pulmonary hypertension in particular, as well as the correlation between the severity of the functional disorders in the pulmonary respiration system and the severity of circulatory insufficiency. Some pathogenetic mechanisms of the changes of certain pulmonary respiration parameters were analysed in the mentioned pathological conditions. The importance of a compound dynamic study of the pulmonary respiration state is emphasized for an objective assessment of the degree and peculiarities of the functional disorders developing in these conditions, and for an evaluation of the efficacy of the conducted treatment.

[Nature of rhythmic discharges in ventral spinal cord roots].

Rhythmic "locomotor discharges" recorded in the ventral spinal roots by Perret et al proved to increase during asphyxia and disappeared during apnea. Thus, rhythmic discharges in the ventral spinal roots depended on irradiation from the respiratory centre and could not confirm the fact of existence of a special "locomotor pacemaker".
